| 2 | A meta-analysis of lamivudine for interruption of mother-to-child transmission of hepatitis B virus显示文摘AIM:To determine the therapeutic effect of lamivudine in late pregnancy for the interruption of motherto-child transmission(MTCT) of hepatitis B virus(HBV).METHODS:Studies were identified by searching available databases up to January 2011.Inclusive criteria were HBV-carrier mothers who had been involved in randomized controlled clinical trials(RCTs) with lamivudine treatment in late pregnancy,and newborns or infants whose serum hepatitis B surface antigen(HBsAg),hepatitis B e antigen(HBeAg) or HBV DNA had been documented.The relative risks(RRs) for interruption of MTCT as indicated by HBsAg,HBV DNA or HBeAg of newborns or infants were calculated with 95% confidence interval(CI) to estimate the efficacy of lamivudine treatment.RESULTS:Fifteen RCTs including 1693 HBV-carrier mothers were included in this meta-analysis.The overall RR was 0.43(95% CI,0.25-0.76;8 RCTs;Pheterogeneity = 0.04) and 0.33(95% CI,0.23-0.47;6 RCTs;Pheterogeneity = 0.93) indicated by newborn HBsAg or HBV DNA.The RR was 0.33(95% CI,0.21-0.50;6 RCTs;Pheterogeneity = 0.46) and 0.32(95% CI,0.20-0.50;4 RCTs;Pheterogeneity = 0.33) indicated by serum HBsAg or HBV DNA of infants 6-12 mo after birth.The RR(lamivudine vs hepatitis B immunoglobulin) was 0.27(95% CI,0.16-0.46;5 RCTs;Pheterogeneity = 0.94) and 0.24(95% CI,0.07-0.79;3 RCTs;P heterogeneity = 0.60) indicated by newborn HBsAg or HBV DNA,respectively.In the mothers with viral load < 106 copies/mL after lamivudine treatment,the efficacy(RR,95% CI) was 0.33,0.21-0.53(5 RCTs;Pheterogeneity = 0.82) for the interruption of MTCT,however,this value was not significant if maternal viral load was > 106 copies/mL after lamivudine treatment(P = 0.45,2 RCTs),as indicated by newborn serum HBsAg.The RR(lamivudine initiated from 28 wk of gestation vs control) was 0.34(95% CI,0.22-0.52;7 RCTs;Pheterogeneity = 0.92) and 0.33(95% CI,0.22-0.50;5 RCTs;Pheterogeneity = 0.86) indicated by newborn HBsAg or HBV DNA.The incidence of adverse effects of lamivudine was not higher in the mothers than in controls(P = 0.97).Only one study reported side effects of lamivudine in newborns.CONCLUSION:Lamivudine treatment in HBV carriermothers from 28 wk of gestation may interrupt MTCT of HBV efficiently.Lamivudine is safe and more efficient than hepatitis B immunoglobulin in interrupting MTCT.HBV MTCT might be interrupted efficiently if maternal viral load is reduced to < 106 copies/mL by lamivudine treatment. | Lei Han Hong-Wei Zhang Jia-Xin Xie Qi Zhang Hong-Yang Wang Guang- Wen Cao | 2011 | World Journal of Gastroenterology2011,17,38: | 61 |

| 4 | Tobacco curly shoot virus isolated in Yunnan is a distinct species of Begomovirus显示文摘Virus isolate Y1 was obtained from tobacco showing curly shoot symptoms in Baoshan, Yunnan Province. Whitefly transmission test and virion morphology observa-tion showed that it is a begomovirus. In reactions with 14 monoclonal antibodies raised against begomoviruses, Y1 was readily differentiated from begomoviruses reported in China, Pakistan and India. The complete nucleotide sequence of DNA-A was determined, it contains 2746 nucleotides, with two ORFs in virion-sense DNA and four ORFs in comple-mentary-sense DNA. Comparisons with total DNA-A, inter-genie region and deduced amino acid sequences of individual ORFs showed that Y1 is a distinct Begomovirus species, for which the name Tobacco curly shoot virus (TCSV) is pro-posed. The total DNA-A of TCSV is most closely related to that of Tomato leaf curl virus from India (85% sequence identity). The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. | 12 | Pathological changes at early stage of multiple organ injury in a rat model of severe acute pancreatitis显示文摘BACKGROUND:Severe acute pancreatitis (SAP) is a commonly seen acute abdominal syndrome characterized by sudden onset,rapid progression and high mortality rate.The damage in peripheral organs may be more severe than that in the pancreas,and can even lead to multiple organ dysfunction.It is critical to recognize early pathological changes in multiple organs.This study aimed to assess the early pathological features of damaged organs in a rat model of SAP.METHODS:Thirty clean grade healthy male Sprague-Dawley rats weighing 250-300 g were randomly divided into a model control group (n=15) and a sham-operated group (n=15).The SAP rat model was induced by sodium taurocholate.Samples of blood and from multiple organs were collected 3 hours after operation.We assessed the levels of IL-6,TNF-α,PLA2,NO,ET-1,MDA,amylases and endotoxin in blood and observed the early pathological changes in multiple damaged organs.RESULTS:Levels of IL-6,TNF-α,PLA2,NO,ET-1 and MDA in serum and of amylase and endotoxin in plasma of the model control group rats were significantly higher than those of the sham-operated group (P<0.01).Different degrees of pathological change were observed in multiple damaged organs.CONCLUSION:Multiple organ injury may occur at the early stage of SAP in rats. | Zhang, Xi-Ping Zhang, Jie Ma, Mei-Li Cai, Yang Xu, Ru-Jun Xie, Qi Jiang, Xin-Ge Ye, Qian | 2010 | Hepatobiliary & Pancreatic Diseases International2010,9,1: | 33 |

| 15 | Urinary trypsin inhibitor attenuates hepatic ischemia-reperfusion injury by reducing nuclear factor-kappa B activation显示文摘BACKGROUND:Urinary trypsin inhibitor(UTI)inhibits the inflammatory response and protects against ischemia- reperfusion(I/R)injury.The inflammatory response is mediated by nuclear factor-kappa B(NF-κB)and its related target genes and products such as vascular endothelial cell adhesion molecule and CXC chemokines.We aimed to assess the roles of those mediators in a UTI-treated mouse model of hepatic I/R injury. METHODS:Treatment group 1(UTI given 5 minutes prior to liver ischemia),treatment group 2(UTI given 5 minutes after the anhepatic phase)and a control group were investigated.Blood and liver samples were obtained and compared at 1,3,6 and 24 hours after reperfusion. RESULTS:Attenuation of pathological hepatocellular damage was greater in the treatment groups than in the control group(P<0.05).Compared with the control group, the UTI treatment groups showed significantly lower serum alanine aminotransferase and aspartate aminotransferase levels,decreased myeloperoxidase activity,and reduced NF- κB activation.Also downregulated was the expression of tumor necrosis factor-alpha,cytokine-induced neutrophil chemoattractant,and macrophage inflammatory protein-2 at the mRNA level.P-selectin protein and intercellular adhesion molecule-1 protein expression were also downregulated.In addition,the treatment group 1 showed a better protective effect against I/R injury than the treatment group 2.CONCLUSIONS:UTI reduces NF-κB activation and downregulates the expression of its related mediators, followed by the inhibition of neutrophil aggregation and infiltration in hepatic I/R injury.The protective role of UTI is more effective in prevention than in treatment. | Wu, Yi-Jun Ling, Qi Zhou, Xin-Hui Wang, Yan Xie, Hai-Yang Yu, Ji-Ren Zheng, Shu-Sen | 2009 | Hepatobiliary & Pancreatic Diseases International2009,8,1: | 28 |

This study reviews China's food security prospects and their implications, focusing on international trade in the coming decade. The results show that China's policies for ensuring food security will be enhanced and China will move to sustainable agriculture. Most studies anticipate that China will increase its food and feed imports in the coming decade. China's overall food self-sufficiency is likely to fall from 94.5% in 2015 to around 91% by 2025. The greatest increases in imports are likely to be soybean, maize, sugar, and dairy products. However, within the production capacity of the major exporting countries and of many food-importing developing countries, China's additional imports of 3 to 5% of its total food consumption in the coming decade are unlikely to threaten global food security.
