1Therapeutic efficacy of the Qing Dai in patients with intractable ulcerative colitis显示文摘Ulcerative colitis (UC) is a chronic inflammatory bowel disease that may become intractable when treated with conventional medications such as aminosalicylates, corticosteroids, and azathioprine. The herbal medicine Qing Dai has traditionally been used in Chinese medicine to treat UC patients, but there is a lack of published data on the efficacy of Qing Dai in UC treatment. We report several cases of patients with intractable UC who take Qing Dai in a retrospective observational study. Furthermore, we explore the mechanisms of action of Qing Dai. Nine patients with active UC who received conventional medications but wished to receive Qing Dai as an alternative medication were included in our analysis. The UC severity level was determined based on the clinical activity index (CAI). Additionally, 5 of the 9 patients were endoscopically evaluated according to the Matts grading system. Each patient received 2 g/d of Qing Dai orally and continued taking other medications for UC as prescribed. Electron spin resonance was applied to explore the mechanisms of action of Qing Dai. After 4 mo of treatment with Qing Dai, the CAI score decreased from 8.3 ± 2.4 to 2.4 ± 3.4 (mean ± SD; P < 0.001). Similarly, the endoscopic Matts grade decreased from 3.4 ± 0.5 to 2.2 ± 0.8 (P = 0.02). Six of 7 patients who were on prednisolone upon enrollment in the study were able to discontinue this corticosteroid. Electron spin resonance revealed that Qing Dai possesses strong hydroxyl radical scavenging activity. Qing Dai showed significant clinical and endoscopic efficacy in patients who failed to respond to conventional medications. 2Management of hepatitis B virus infection during treatment for hepatitis B virus-related hepatocellular carcinoma显示文摘Although liver resection is considered the most effective treatment for hepatocellular carcinoma(HCC), treatment outcomes are unsatisfactory because of the high rate of HCC recurrence. Since we reported hepatitis B e-antigen positivity and high serum hepatitis B virus(HBV) DNA concentrations are strong risk factors for HCC recurrence after curative resection of HBV-related HCC in the early 2000 s, many investigators have demonstrated the effects of viral status on HCC recurrence and post-treatment outcomes. These findings suggest controlling viral status is important to prevent HCC recurrence and improve survival after curative treatment for HBV-related HCC. Antiviral therapy after curative treatment aims to improve prognosis by preventing HCC recurrence and maintaining liver function. Therapy with interferon and nucleos(t)ide analogs may be useful for preventing HCC recurrence and improving overall survival in patients who have undergone curative resection for HBV-related HCC. In addition, reactivation of viral replication can occur after liver resection for HBV-related HCC. Antiviral therapy can be recommended for patients to prevent HBV reactivation. 3Effect of Clostridium butyricum on fecal flora in Helicobacter pylori eradication therapy显示文摘AIM: To investigate the effect of probiotic bacterium,Clostridium butyricum MIYAIRI 588 strain (CBM) on the changes of the fecal flora in Helicobacter pylori(H pylori)treatment.METHODS: Thirty-five patients with gastric or duodenal ulcers positive for Hpylori were randomized either to 1 wk amoxicillin, clarithromycin, lansoprazole (Group 1) or to the same regimen supplemented with CBM 7 dahead of the triple therapy (Group 2). Stool samples were collected before and 2, 4, 7, 15, and 22 d after the starting eradication therapy, and were examined intestinal flora. Patients were required to keep a diary record of their condition. RESULTS: Obligate anaerobes decreased significantly on d 2, 4, 8 and 15 in Group 1. On the other hand, they did not decrease significantly in Group 2. The Escherichiacoli was dominant bacterium in Enterobacteriaceae, butthat was replaced by other species such as Klebsiella and Enterobacter after eradication in Group 1. The change was suppressed in Group 2. 7Two cases of adenocarcinoma occurring in sporadic fundic gland polyps observed by magnifying endoscopy with narrow band imaging显示文摘Gastric fundic gland polyps(FGPs) are common nonadenomatous gastric polyps arising from normal fundic mucosa without Helicobacter pylori(H. pylori) infection. Although systemic FGPs associated with familial adenomatous polyposis(FAP) often have dysplasia, there are few reports of dysplasia occurring in sporadic F G P s, e s p e c i a l l y w h e n d e t e c t e d b y m a g n i f y i n g endoscopy with narrow band imaging(ME-NBI). We experienced two cases of adenocarcinoma occurring in sporadic FGPs, and their ME-NBI findings were very useful for differentiating FGP with cancer from nondysplastic FGP. A 68-year-old man and a 63-year-old woman were referred to our institution for medical checkup. H. pylori was negative in both patients. Endoscopic examination revealed a small reddishpolypoid lesion on the anterior wall of the upper gastric body and several FGPs. ME-NBI showed an irregular microvascular architecture composed of closed loop- or open loop-type vascular components, plus an irregular microsurface structure composed of oval-type surface components which was different from that of FGPs. FAP was denied because of the absence of colon polyps and no familial history of FAP. 8Co-effect of Demand-control-support Model and Effort-reward Imbalance Model on Depression Risk Estimation in Humans: Findings from Henan Province of China显示文摘Objective To investigate the co-effect of Demand-control-support(DCS) model and Effort-reward Imbalance(ERI) model on the risk estimation of depression in humans in comparison with the effects when they are used respectively. Methods A total of 3 632 males and 1 706 females from 13 factories and companies in Henan province were recruited in this cross-sectional study. Perceived job stress was evaluated with the Job Content Questionnaire and Effort-Reward Imbalance Questionnaire(Chinese version). Depressive symptoms were assessed by using the Center for Epidemiological Studies Depression Scale(CES-D). Results DC(demands/job control ratio) and ERI were shown to be independently associated with depressive symptoms. The outcome of low social support and overcommitment were similar. High DC and low social support(SS), high ERI and high overcommitment, and high DC and high ERI posed greater risks of depressive symptoms than each of them did alone.ERI model and SS model seem to be effective in estimating the risk of depressive symptoms if they are used respectively. Conclusion The DC had better performance when it was used in combination with low SS. The effect on physical demands was better than on psychological demands. 9Basket pattern blood flow signals discovered in a case of splenic hamartoma by power Doppler ultrasonography显示文摘We present the gray-scale ultrasonography (GSUS), power Doppler ultrasonography (PDUS), abdominal computed tomography (CT), and magnetic resonance imaging (MRI) findings for a case of splenic hamartoma in a 27-year-old man, showing a ψ 50 mm homogeneous, iso- and hypo-echoic splenic mass with evidence of a small plural cystic lesion.This splenic hamartoma showed increased vascularity on power Doppler sonograms. PDUS showed multiple circular blood flow signals inside the mass (i.e. a basket pattern),which was consistent with the small plural cystic lesion shown by GSUS. Spectral analysis also confirmed arterial and venous flow. CT scans showed that the mass had lowdensity relative to the normal spleen and MRI showed that the mass was isodense, relative to the normal spleen.Therefore, CT and MRI are not useful for the diagnosis of splenic hamartoma. 10Characterization of CO_(2) flux in three Kobresia meadows differing in dominant species显示文摘Aims Kobresia meadows,the dominant species of which differ in different habitats,cover a large area of alpine grassland on the QinghaiTibetan Plateau and act as potential CO_(2) sinks.Kobresia meadows with different dominant species may differ in carbon sink strength.We aimed to test the hypothesis and to clarify the differences in CO_(2) sink strength among three major Kobresia meadows on the plateau and the mechanisms underlying these differences.Methods We measured the net ecosystem exchange flux(NEE),ecosystem respiration flux(ER),aboveground biomass(AGB)and environmental variables in three Kobresia meadows,dominated by K.pygmaea,K.humilis,or K.tibetica,respectively,in Haibei,Qinghai.NEE and ER were measured by a closed-chamber method.Environmental variables,including photosynthetic photon flux density(PPFD),air and soil temperature and air and soil moisture,were monitored during the above flux measurements.Important findings The measured peak AGB increased with soil water content and was 365,402 and 434 g dry weight m2 for K.pygmaea,K.humilis and K.tibetica meadow,respectively.From the maximum ecosystem photosynthetic rate in relation to PPFD measured during the growing season,we estimated gross ecosystem photosynthetic potential(GEPmax)as 22.2,29.9 and 37.8 lmol CO_(2) m2 s1 for K.pygmaea,K.humilis and K.tibetica meadow,respectively.We estimated the respective gross primary production(GPP)values as 799,1063 and 1158 g C m^(-2) year^(-1) and ER as 722,914 and 1011 g C m^(-2) year^(-1).Average net ecosystem production(NEP)was estimated to be 76.9,149.4 and 147.6 g C m^(-2) year^(-1) in K.pygmaea,K.humilis and K.tibetica meadows,respectively.The results indicate that(i)the three meadows were CO_(2) sinks during the study period and(ii)Kobresia meadows dominated by different species can differ considerably in carbon sink strength even under the same climatic conditions,which suggests the importance of characterizing spatial heterogeneity of carbon dynamics in the future.Pengcheng Zhang Mitsuru Hirota Haihua Shen Akinori Yamamoto Shigeru Mariko Yanhong Tang 2009Journal of Plant Ecology2009,2,4:4

13Apoptosis is not an invariable component of in vitro models of cortical cerebral ischaemia显示文摘Characterising the mechanisms of cell death following focal cerebral ischaemia has been hampered by a lack of an in vitro assay emulating both the apoptotic and necrotic features observed in vivo. The present study systematically characterised oxygen-glucose-deprivation (OGD) in primary rat cortical neurones to establish a reproducible model with components of both cell-death endpoints. OGD induced a time-dependent reduction in cell viability, with 80% cell death occurring 24 h after 3 h exposure to 0% O2 and 0.5 mM glucose. Indicative of a necrotic component to OGDinduced cell death, N-methyl-D-aspartate (NMDA) receptor inhibition with MK-801 attenuated neuronal loss by 60%.The lack of protection by the caspase inhibitors DEVD-CHO and z-VAD-fmk suggested that under these conditions neurones did not die by an apoptotic mechanism. Moderating the severity of the insult by decreasing OGD exposure to 60 min did not reduce the amount of necrosis, but did induce a small degree of apoptosis (a slight reduction in cell death was observed in the presence of 10 μtM DEVD-CHO). In separate experiments purported to enhance the apoptotic component, cells were gradually deprived of O2, exposed to 4% O2 (as opposed to 0%) during the OGD period, or maintained in serum-containing media throughout. While NMDA receptor antagonism significantly reduced cortical cell death under all conditions, a caspase-inhibitor sensitive component of cell death was not uncovered. 15Experimental Analysis of Flow Structure in Contra-Rotating Axial Flow Pump Designed with Different Rotational Speed Concept显示文摘As a high specific speed pump, the contra-rotating axial flow pump distinguishes itself in a rear rotor rotating in the opposite direction of the front rotor, which remarkably contributes to the energy conversion, the reduction of the pump size, better hydraulic and cavitation performances. However, with two rotors rotating reversely, the significant interaction between blade rows was observed in our prototype contra-rotating rotors, which highly affected the pump performance compared with the conventional axial flow pumps. Consequently, a new type of rear rotor was designed by the rotational speed optimization methodology with some additional considerations, aiming at better cavitation performance, the reduction of blade rows interaction and the secondary flow suppression. The new rear rotor showed a satisfactory performance at the design flow rate but an unfavorable positive slope of the head-flow rate curve in the partial flow rate range less than 40% of the design flow rate, which should be avoided for the reliability of pump-pipe systems. In the present research, to understand the internal flow field of new rear rotor and its relation to the performances at the partial flow rates, the velocity distributions at the inlets and outlets of the rotors are firstly investigated. Then, the boundary layer flows on rotor surfaces, which clearly reflect the secondary flow inside the rotors, are analyzed through the limiting streamline observations using the multi-color oil-film method. 17Removal of trace arsenic(V) and phosphate from water by a highly selective ligand exchange adsorbent显示文摘A highly selective ligand exchange type adsorbent was developed for the removal of trace arsenic(V) (As(V)) and phosphate from water. This adsorbent was prepared by loading zirconium(IV) on monophosphonic acid resin. This adsorbent was able to remove toxi canions efficiently at wide pH ranges. However, low pH was preferable for maximum breakthrough capacity in an adsorption operation.The effect of a large amount of competing anions such as chloride, bicarbonate, and sulfate on the adsorption systems of As(V) andphosphate anions was investigated. The experimental findings revealed that the As(V) and phosphate uptakes were not affiected by thesecompeting anions despite the enhancement of the breakthrough points and total adsorption. Phosphate anion was slightly preferablethan As(V) in their competitive adsorption by the adsorbent. The adsorbed As(V) and phosphate on the Zr(IV)-loaded resin werequantitatively eluted with 0.1 mol/L sodium hydroxide solution, and the adsorbent was regenerated by 0.5 mol/L sulfuric acid. Duringseveral cycles of adsorption-elution-regeneration operations, no Zr(IV) was detected in the column efluents.
