| 1 | 年龄相关性黄斑变性自我管理第6个月时的随访 一项随机对照试验显示文摘目的:通过测试情绪和功能以评估一项年龄相关性黄斑变性(AMD)的自我管理方案6个月时的有效性。该方案由健康教育和旨在提高生活质量的训练组成。方法:将252例患有进展性AMD的老年成人志愿者(平均年龄,80.8岁)中的214例随机分为3组,一组为12h自我管理方案(n=82),一组为12小时录像健康教育(n=66),一组为静态观察(n=66),分析6个月时随访的数据。主要结果测试是精神痛苦(美国情绪量表)。次要结果测试包括功能(国立眼科研究所视功能调查表),对日常生活中AMD挑战的自身把握能力(AMD自身效应调查表),以及运用精神障碍诊断及统计手册第4版检测抑郁情况。结果:在第6个月随访时,与对照组相比,自我管理组受试者的精神痛苦明显较小(P=0.008),功能更好(P=0.05);自身效应明显提高(P=0.006)。与非抑郁病例相比,在抑郁病例中迟发效应更为常见。最后,在第6个月随访时自我管理组的临床抑郁症发病率明显低于对照组(P=0.05)。结论:在第6个月随访时持续的积极效果证实,对于视力低下的老年AMD患者,AMD自我管理方案可以有效地减少痛苦和病残,提高自身效应,以及防止忧郁症的发生。 | Barbara L.
