1Hepatocellular carcinoma in non-cirrhotic liver: A comprehensive review显示文摘Hepatocellular carcinoma(HCC) is the most common type of primary liver cancer, which in turns accounts for the sixth most common cancer worldwide.Despite being the 6 th most common cancer it is the second leading cause of cancer related deaths. HCC typically arises in the background of cirrhosis, however,about 20% of cases can develop in a non-cirrhotic liver. This particular subgroup of HCC generally presents at an advanced stage as surveillance is not performed in a non-cirrhotic liver. HCC in non-cirrhotic patients is clinically silent in its early stages because of lack of symptoms and surveillance imaging; and higher hepatic reserve in this population. Interestingly, F3 fibrosis in non-alcoholic fatty liver disease, hepatitis B virus and hepatitis C virus infections are associated with high risk of developing HCC. Even though considerable progress has been made in the management of this entity, there is a dire need for implementation of surveillance strategies in the patient population at risk, to decrease the disease burden at presentation and improve the prognosis of these patients. 4Probiotics for gastrointestinal disorders: Proposed recommendations for children of the Asia-Pacific region显示文摘Recommendations for probiotics are available in several regions. This paper proposes recommendations for probiotics in pediatric gastrointestinal diseases in the Asia-Pacific region. Epidemiology and clinical patterns of intestinal diseases in Asia-Pacific countries were discussed. Evidence-based recommendations and randomized controlled trials in the region were revised. Cultural aspects,health management issues and economic factors were also considered. Final recommendations were approved by applying the Likert scale and rated using the GRADE system. Saccharomyces boulardii CNCM I-745(Sb) and Lactobacillus rhamnosus GG(LGG) were strongly recommended as adjunct treatment to oral rehydration therapy for gastroenteritis. Lactobacillus reuteri could also be considered. Probiotics may be considered for prevention of(with the indicated strains): antibiotic-associated diarrhea(LGG or Sb); Clostridium difficile-induced diarrhea(Sb); nosocomial diarrhea(LGG); infantile colic(L reuteri) and as adjunct treatment of Helicobacter pylori(Sb and others). Specific probiotics with a history of safe use in preterm and term infants may be considered in infants for prevention of necrotizing enterocolitis. There is insufficient evidence for recommendations in other conditions. Despite a diversity of epidemiological,socioeconomical and health system conditions,similar recommendations apply well to Asia pacific countries. 5Endothelial progenitor cells:Exploring the pleiotropic effects of statins显示文摘Statins have become a cornerstone of risk modification for ischaemic heart disease patients. A number of studies have shown that they are effective and safe. However studies have observed an early benefit in terms of a reduction in recurrent infarct and or death after a myocardial infarction,prior to any significant change in lipid profile. Therefore,pleiotropic mechanisms,other than lowering lipid profile alone,must account for this effect. One such proposed pleiotropic mechanism is the ability of statins to augment both number and function of endothelial progenitor cells. The ability to augment repair and maintenance of a functioning endothelium may have profound beneficial effect on vascular repair and potentially a positive impact on clinical outcomes in patients with cardiovascular disease. 6Clinically unrecognized mitral regurgitation is prevalent in lone atrial fibrillation显示文摘AIM:To investigate the prevalence of clinically unrecognized mitral regurgitation(MR) in lone atrial fibrillation(AF).METHODS:We studied the prevalence and severity of MR by transesophageal echocardiography(TEE) in patients with 'lone' AF as compared to a matched cohort of patients in normal sinus rhythm(NSR) undergoing TEE for other indications besides recognized valvular heart disease.RESULTS:A total of 157 subjects(57 in the AF group and 100 in the NSR group) with structurally normal cardiac valves were included in the study.In the AF group,moderate MR or more was noted in 66% of thepatients,mild MR in 18%,trace or no MR in 16%.In the control group,moderate MR was noted in 6% of patients,mild MR 31%,trace or no MR in 63 % of patients.Moderate MR or greater was significantly more prevalent in the AF group compared to the NSR group(66% vs 6%,P < 0.0001).CONCLUSION:Clinically unrecognized moderate MR is prevalent in 'lone' AF-either as an etiologic factor leading to 'lone' AF or developing after onset of AF.Sanjiv Sharma Joel Lardizabal Mark Monterroso Neil Bhambi Rohan Sharma Rasham Sandhu Sarabjeet Singh 2012World Journal of Cardiology2012,4,5:5
7Landscape changes have greater effects than climate changes on six insect pests in China显示文摘In recent years, global changes are the major causes of frequent, widespread outbreaks of pests in mosaic landscapes, which have received substantial attention worldwide. We collected data on global changes(landscape and climate) and economic damage caused by six main insect pests during 1951–2010 in China. Landscape changes had significant effects on all six insect pests. Pest damage increased significantly with increasing arable land area in agricultural landscapes. However, climate changes had no effect on damage caused by pests, except for the rice leaf roller(Cnaphalocrocis medinalis Guenee) and armyworm(Mythimna separate(Walker)), which caused less damage to crops with increasing mean temperature. Our results indicate that there is slight evidence of possible offset effects of climate changes on the increasing damage from these two agricultural pests. Landscape changes have caused serious outbreaks of several species, which suggests the possibility of the use of landscape design for the control of pest populations through habitat rearrangement. 9Gastrointestinal and hepatic manifestations of COVID-19:A comprehensive review显示文摘The severe acute respiratory syndrome-coronavirus-2(SARS-CoV-2)that causes coronavirus disease-2019(COVID-19)is a global pandemic,manifested by an infectious pneumonia.Although patients primarily present with fever,cough and dyspnea,some patients also develop gastrointestinal(GI)and hepatic manifestations.The most common GI symptoms reported are diarrhea,nausea,vomiting,and abdominal discomfort.Liver chemistry abnormalities are common and include elevation of aspartate transferase,alanine transferase,and total bilirubin.Studies have shown that SARS-CoV-2 infects the GI tract via its viral receptor angiotensin converting enzyme Ⅱ,which is expressed on enterocytes of the ileum and colon.Viral RNA has also been isolated from stool specimens of COVID-19 patients,which raised the concern for fecal-oral transmission in addition to droplet transmission.Although indirect evidence has suggested possible fecal-oral transmission of SARS-CoV-2,more effort is needed to establish the role of the fecal-oral transmission route.Further research will help elucidate the association between patients with underlying GI diseases,such as chronic liver disease and inflammatory bowel disease,and severity of COVID-19.In this review,we summarize the data on GI involvement to date,as well as the impact of COVID-19 on underlying GI diseases.Ming Han Cha Miguel Regueiro Dalbir S Sandhu 2020World Journal of Gastroenterology2020,26,19:4

13Imaging-based algorithmic approach to gallbladder wall thickening显示文摘Gallbladder(GB) wall thickening is a frequent finding caused by a spectrum of conditions. It is observed in many extracholecystic as well as intrinsic GB conditions. GB wall thickening can either be diffuse or focal. Diffuse wall thickening is a secondary occurrence in both extrinsic and intrinsic pathologies of GB, whereas, focal wall thickening is mostly associated with intrinsic GB pathologies. In the absence of specific clinical features, accurate etiological diagnosis can be challenging. The survival rate in GB carcinoma(GBC) can be improved if it is diagnosed at an early stage, especially when the tumor is confined to the wall. The pattern of wall thickening in GBC is often confused with benign diseases, especially chronic cholecystitis, xanthogranulomatous cholecystitis, and adenomyomatosis. Early recognition and differentiation of these conditions can improve the prognosis. In this minireview, the authors describe the patterns of abnormalities on various imaging modalities(conventional as well as advanced) for the diagnosis of GB wall thickening. 16The benefit of curative liver resection with a selective bile duct preserving approach for hepatocellular carcinoma with macroscopic bile duct tumor thrombus显示文摘Background:Hepatocellular carcinoma(HCC)presenting with macroscopic bile duct tumor thrombus(BDTT)is an uncommon event.The role of a curative hepatic resection and associated long-term outcomes remain controversial.In addition the necessity for bile duct resection is still unclear.The aim of this study was to evaluate outcomes of hepatectomy with a selective bile duct preservation approach for HCC with BDTT in comparison to outcomes without BDTT.Methods:A total of 22 HCC with BDTT patients who had undergone curative hepatic resection with a selective bile duct preservation approach at our institute were retrospectively reviewed.These were compared to group of 145 HCC without BDTT patients.The impact of curative surgical resection and BDTT on clinical outcomes and survival after surgical resection were analyzed.Results:All HCC with BDTT cases underwent major hepatectomy vs.32.4%in the comparative group.Bile duct preservation rate was 56.5%.The 1-,3-and 5-year survival rates of HCC with BDTT patients in comparison to the HCC without BDTT group were 81.8%,52.8%and 52.8%vs.73.6%,55.6%and 40.7%(P=0.804)respectively.Positive resection margin,tumor size≥5 cm and AFP≥200 IU/mL were significant risk factors regarding overall survival.However,it is unclear whether presence of a bile duct tumor thrombus has an adverse impact on either recurrence free survival or overall survival.Conclusions:Bile duct obstruction from tumor thrombus did not necessarily indicate an advanced form of disease.Tumor size and AFP had greater impact on long-term outcomes than bile duct tumor thrombus.Major liver resection with a selective bile duct preserving approach in HCC with BDTT can achieve favorable outcomes comparable to those of HCC without BDTT in selected patients.Anon Chotirosniramit Akkaphod Liwattanakun Sunhawit Junrungsee Wasana Ko-iam Trichak Sandhu Worakitti Lapisatepun 2020Hepatobiliary Surgery and Nutrition2020,9,6:2
