| 2 | The Changing Cold Regions Network:Observation,diagnosis and prediction of environmental change in the Saskatchewan and Mackenzie River Basins,Canada显示文摘Climate change is causing rapid and severe changes to many Earth systems and processes,with widespread cryospheric,ecological,and hydrological impacts globally,and especially in high northern latitudes.This is of major societal concern and there is an urgent need for improved understanding and predictive tools for environmental management.The Changing Cold Regions Network(CCRN)is a Canadian research consortium with a focus to integrate existing and new experimental data with modelling and remote sensing products to understand,diagnose,and predict changing land,water,and climate,and their interactions and feedbacks over the geographic domain of the Mackenzie and Saskatchewan River Basins in Canada.The network operates a set of 14 unique and focused Water,Ecosystem,Cryosphere and Climate(WECC)observatories within this region,which provide opportunities to observe and understand processes and their interaction,as well as develop and test numerical simulation models,and provide validation data for remote sensing products.This paper describes this network and its observational,experimental,and modelling programme.An overview of many of the recent Earth system changes observed across the study region is provided,and some local insights from WECC observatories that may partly explain regional patterns and trends are described.Several of the model products being developed are discussed,and linkages with the local to international user community are reviewed—In particular,the use of WECC data towards model and remote sensing product calibration and validation is highlighted.Some future activities and prospects for the network are also presented at the end of the paper. | Chris M DEBEER Howard S WHEATER William L QUINTON Sean K CAREY Ronald E STEWART Murray D MACKAY Philip MARSH | 2015 | Science China Earth Sciences2015,58,1: | 5 |
| 3 | Early infant male circumcision:Systematic review,risk-benefit analysis,and progress in policy显示文摘AIM To determine whether recent evidence-based United States polices on male circumcision(MC) apply to comparable Anglophone countries,Australia and New Zealand.METHODS Articles in 2005 through 2015 were retrieved from PubM ed using the keyword 'circumcision' together with 36 relevant subtopics.A further PubM ed search was performed for articles published in 2016.Searches of the EMBASE and Cochrane databases did not yield additional citable articles.Articles were assessed for quality and those rated 2+ and above according to the Scottish Intercollegiate Grading System were studied further.The most relevant andrepresentative of the topic were included.Bibliographies were examined to retrieve further key references.Randomized controlled trials,recent high quality systematic reviews or meta-analyses(level 1++ or 1+ evidence) were prioritized for inclusion.A risk-benefit analysis of articles rated for quality was performed.For efficiency and reliability,recent randomized controlled trials,metaanalyses,high quality systematic reviews and large welldesigned studies were used if available.Internet searches were conducted for other relevant information,including policies and Australian data on claims under Medicare for MC.RESULTS Evidence-based policy statements by the American Academy of Pediatrics(AAP) and the Centers for Disease Control and Prevention(CDC) support infant and later age male circumcision(MC) as a desirable public health measure.Our systematic review of relevant literature over the past decade yielded 140 journal articles that met our inclusion criteria.Together,these showed that early infant MC confers immediate and lifelong benefits by protecting against urinary tract infections having potential adverse long-term renal effects,phimosis that causes difficult and painful erections and 'ballooning' during urination,inflammatory skin conditions,inferior penile hygiene,candidiasis,various sexually transmissible infections in both sexes,genital ulcers,and penile,prostate and cervical cancer.Our risk-benefit analysis showed that benefits exceeded procedural risks,which are predominantly minor,by up to 200 to 1.We estimated that more than 1 in 2 uncircumcised males will experience an adverse foreskin-related medical condition over their lifetime.Wide-ranging evidence from surveys,physiological measurements,and the anatomical location of penile sensory receptors responsible for sexual sensation strongly and consistently suggested that MC has no detrimental effect on sexual function,sensitivity or pleasure.United States studies showed that early infant MC is cost saving.The evidence supporting early infant MC has further strengthened since the positive AAP and CDC reviews.CONCLUSION Affirmative MC policies are needed in Australia and New Zealand.Routine provision of accurate,unbiased education,and access in public hospitals,will maximize health and financial benefits. | Brian J Morris Sean E Kennedy Alex D Wodak Adrian Mindel David Golovsky Leslie Schrieber Eugenie R Lumbers David J Handelsman John B Ziegler | 2017 | World Journal of Clinical Pediatrics2017,6,1: | 2 |

| 5 | Use of sodium bicarbonate and blood gas monitoring in diabetic ketoacidosis: A review显示文摘Diabetic ketoacidosis(DKA) is a severe and toocommon complication of uncontrolled diabetes mellitus. Acidosis is one of the fundamental disruptions stemming from the disease process, the complications of which are potentially lethal. Hydration and insulin administration have been the cornerstones of DKA therapy; however, adjunctive treatments such as the use of sodium bicarbonate and protocols that include serial monitoring with blood gas analysis have been much more controversial. There is substantial literature available regarding the use of exogenous sodium bicarbonate in mild to moderately severe acidosis; the bulk of the data argue against significant benefit in important clinical outcomes and suggest possible adverse effects with the use of bicarbonate. However, there is scant data to support or refute the role of bicarbonate therapy in very severe acidosis. Arterial blood gas(ABG) assessment is an element of some treatment protocols, including society guidelines, for DKA. We review the evidence supporting these recommendations. In addition, we review the data supporting some less cumbersome tests, including venous blood gas assessment and routine chemistries. It remains unclear that measurement of blood gas pH, via arterial or venous sampling, impacts management of the patient substantially enough to warrant the testing, especially if sodium bicarbonate administration is not being considered. There are special circumstances when serial ABG monitoring and/or sodium bicarbonate infusion are necessary, which we also review. Despiteits utility in the diagnosis of myocardial injury in this population, CMRI’s impacton patient management is still evolving. This review provides a framework for theuse of CMRI in diagnosis and management of COVID-19 patients from theperspective of a cardiologist. We review the role of CMRI in the management ofboth the acutely and remotely COVID-19 infected patient.
