| 1 | Quantitative investigation on collapse margin of steel high-rise buildings subjected to extremely severe earthquakes显示文摘Reponses of structures subjected to severe earthquakes sometimes significantly surpass what was considered in the design.It is important to investigate the failure mechanism and collapse margin of structures beyond design,especially for high-rise buildings.In this study,steel high-rise buildings using either square concrete-filled-tube(CFT) columns or steel tube columns are designed.A detailed three-dimensional(3 D) structural model is developed to analyze the seismic behavior of a steel high-rise towards a complete collapse.The effectiveness is verified by both component tests and a full-scale shaking table test.The collapse margin,which is defined as the ratio of PGA between the collapse level to the design major earthquake level(Level 2),is quantified by a series of numerical simulations using incremental dynamic analyses(IDA).The baseline building using CFT columns collapsed with a weak first story mechanism and presented a collapse margin ranging from 10 to 20.The significant variation in the collapse margin was caused by the different characteristics of the input ground motions.The building using equivalent steel columns collapsed earlier due to the significant shortening of the locally buckled columns,exhibiting only 57% of the collapse margin of the baseline building.The influence of reducing the height of the first story was quite significant.The shortened first story not only enlarged the collapse margin by 20%,but also changed the collapse mode. | Lin Xuchuan Mikiko Kato Zhang Lingxin Masayoshi Nakashima | 2018 | Earthquake Engineering and Engineering Vibration2018，17，3： | 9 |
| 2 | Expression of β-catenin in hepatocellular carcinoma显示文摘AIM: The β-catenin has been recognized as a critical member of the Wnt signaling pathway and plays an important role in the generation/differentiation of many tissues. Inappropriate activation of this pathway has been implicated in carcinogenesis. The mechanism underlying the development as well as its prognosis of hepatocellular carcinoma (HCC) has remained unclear. The purpose of this study is to analyze the expression of β-catenin in HCC in relation to histological grades and viral hepatitis backgrounds.METHODS: Thirty-two sections were selected at random from autopsy and surgical cases of HCC. Immuohistologically,the location and positivity of β-catenin expression in HCC was examined.RESULTS: Normal hepatocytes did not express β-catenin.In 78% of HCC β-catenin was expressed at the membrane of the cells, with or without cytoplasmic and/or nuclear expression. The tumor cells with well- and moderatelydifferentiated grades expressed frequently at the membrane and cytoplasm compared with poorly-differentiated type.Nuclear expression of β-catenin was prone to occur in the tumor cells of poorly-differentiated grade. There were 15% of hepatitis C virus (HCV) backgrounds with nuclear expression. In contrast, there was 38% with nuclear expression in hepatitis B virus (HBV) backgrounds. | 3 | Acquired amegakaryocytic thrombocytopenia previously diagnosed as idiopathic thrombocytopenic purpura in a patient with hepatitis C virus infection显示文摘We report the first case of a patient with hepatitis C virus(HCV) infection and idiopathic thrombocytopenic purpura(ITP), who later developed acquired amegakaryocytic thrombocytopenia(AAMT), with autoantibodies to the thrombopoietin(TPO) receptor(c-Mpl). A 64-year-old woman, with chronic hepatitis C, developed severe thrombocytopenia and was diagnosed with ITP. She died of liver failure. Autopsy revealed cirrhosis and liver carcinoma. In the bone marrow, a marked reduction in the number of megakaryocytes was observed, while other cell lineages were preserved. Therefore, she was diagnosed with AAMT. Additionally, autoantibodies to c-Mpl were detected in her serum. Autoantibodies to c-Mpl are one of the causes of AAMT, acting through inhibition of TPO function, megakaryocytic maturation, and platelet formation. HCV infection induces several autoantibodies. HCV infection might also induce autoantibodies to c-Mpl, resulting in the development of AAMT. | 16 | Follow-up outcomes in patients with negative initial colon capsule endoscopy findings显示文摘BACKGROUND Colon capsule endoscopy(CCE),which became clinically applicable in 2006,is a simple and noninvasive procedure to evaluate colonic diseases;the accuracy of second-generation CCE,introduced in 2009,has dramatically improved.Currently,CCE is used as an alternative method for colorectal cancer screening,as well as for evaluating the mucosal lesions of inflammatory bowel disease,in cases where performing colonoscopy(CS)is difficult.However,the outcomes of CCE are uncertain.AIM To investigate the outcomes of Japanese patients with negative findings(no polyps or colorectal cancer)on initial CCE.METHODS This retrospective,single-center study was conducted at the Endoscopic Center at Aishinkai Nakae Hospital.This study included patients who underwent continuous CCE between November 2013 and August 2019,that exhibited no evidence of polyps or colorectal cancer at the initial CCE,and could be followed up using either the fecal immunochemical test(FIT),CS,or CCE.The observational period,follow-up method,presence or absence of polyps and colorectal cancer,pathological diagnosis,and number of colorectal cancer deaths were evaluated.RESULTS Thirty-one patients(mean age,60.4±15.6 years;range,28–84 years;14 men and 17 women)were enrolled in this study.The reasons for performing the first CCE were screening in 12,a positive FIT in six,lower abdominal pain in nine,diarrhea in two,and anemia in two patients.The mean total water volume at the time of examination was 3460±602 mL(2250–4800 mL),and a total CS was performed in 28 patients(90%).The degree of cleanliness was excellent in 15 patients and good in 16,and no poor cases were observed.No adverse events,such as retention or capsule aspiration,were observed in any of the patients.The mean follow-up period was 3.1±1.5 years(range,0.3–5.5 years).Follow-up included FIT in nine,CS in 20,and CCE in four patients(including duplicate patients).The FIT was positive in two patients,while CS revealed five polyp lesions(three in the ascending colon,one in the transverse colon,and one in the descending colon),with sizes ranging between 2 mm and 8 mm.Histopathological findings revealed a hyperplastic polyp in one patient,and adenoma with low grade dysplasia in four patients;colorectal cancers were not recognized.In the follow-up example by CCE,polyps and colorectal cancer could not be recognized.During the follow-up period,there were no deaths due to colorectal cancer in any of the patients.CONCLUSION We determined the outcomes in patients with negative initial CCE findings. | Konosuke Nakaji Mitsutaka Kumamoto Mikiko Yodozawa Kazuki Okahara Shigeo Suzumura Yukinori Nakae | 2021 | World Journal of Gastrointestinal Endoscopy2021，13，10： | 1 |
