1Resection of pancreatic cystic neoplasms in recurrent acute pancreatitis prevents recurrent pancreatitis but does not identify more malignancies显示文摘BACKGROUND Recurrent acute pancreatitis(RAP)may be a presenting feature of and an indication for resection of pancreatic cysts,including intra-ductal papillary mucinous neoplasm(IPMN).Few data are available regarding the prevalence of malignancy and post-operative RAP in this population.AIM To study the role of resection to help prevent RAP and analyze if presentation as RAP would be a predictor for malignancy.METHODS This retrospective study assessed 172 patients who underwent surgical resection of pancreatic cystic neoplasms at a university hospital between 2002 and 2016.The prevalence of preoperative high-risk cyst features,and of neoplasia was compared between patients with and without RAP.To identify the cause of pancreatitis,all the patients had a detailed history of alcohol,smoking,medications obtained,and had cross-sectional imaging(contrast-enhanced computed tomography/magnetic resonance imaging)and endoscopic ultrasound to look for gallstone etiology and other structural causes for pancreatitis.The incidence of RAP post-resection was the primary outcome.RESULTS IPMN accounted for 101 cases(58.7%){[branch duct(BD)59(34.3%),main duct(MD)42](24.4%)}.Twenty-nine(16.9%)presented with RAP(mean 2.2 episodes):15 had BD-IPMN,8 MD-IPMN,5 mucinous cystic neoplasm and 1 serous cystic neoplasm.Malignancy was similar among those with vs without RAP for all patients[6/29(20.7%)vs 24/143(16.8%)]and IPMN patients[6/23(26.1%)vs 23/78(29.5%)],although tended to be higher with RAP in BD-IPMN,[5/15(33.3%)vs 3/44(6.8%),P=0.04].At mean follow-up of 7.2 years,1(3.4%)RAP patient had post-resection RAP.The mean episodes of acute pancreatitis before vs after surgery were 3.4 vs 0.02(P<0.0001).CONCLUSION Malignancy was not increased in patients with pancreatic cystic neoplasms who have RAP compared to those without RAP.In addition,specific cyst characteristics were not clearly associated with RAP.The incidence of RAP was markedly decreased in almost all patients following cyst resection.Thiruvengadam Muniraj Harry R Aslanian Loren Laine Priya A Jamidar James F Farrell Kisha A Mitchell Ronald R Salem 2021World Journal of Gastroenterology2021,27,15:3

17Mesenchymal stem cells for kidney transplantation显示文摘The long term consequence of immunosuppressive therapy in kidney transplantation has prompted investigation of alternative means to modify the immune response to the allograft. Cell based therapies are potentially attractive as they may provide a long lasting immunomodulatory effect, may repair tissues and reduce the necessity to take immunosuppressive drug therapy. Of the current cell therapies, mesenchymal stem cells have now been trialled in small numbers of human kidney transplantation with apparent safety and potential efficacy. 18Vitreous microparticles contain apoptotic signals suggesting a diabetic vitreopathy显示文摘AIM:To evaluate differences in microparticle profiles in vitreous samples between diabetic and non-diabetic eyes undergoing vitrectomy.METHODS:Un-masked cross-sectional series of 34 eyes undergoing vitrectomy.Vitreous specimens were collected and processed to evaluate for membrane integrity(DAPI),apoptosis(Annexin-V),and endothelial-cell origin(V-Cadherin).A BD LSR II flow cytometer was used for analysis and standardized sub-micron-sized beads were used for size comparison.RESULTS:Thirty-four specimens underwent analysis.Greater levels of Annexin-V were found on microparticles from specimens in which blood had entered the vitreous(n=12)compared to those without blood(n=22;52.3%±30.7%vs 19.6%±27.2%,P=0.002).Patients with diabetes having surgery with hemorrhage(n=7)had greater expression of Annexin-V than those without hemorrhage(n=8;62.1%±31.7%vs 18.9%±20.9%,P=0.009).However,in patients with non-diabetic vitreous hemorrhage,the level of Annexin-V expression was not significantly different compared to other disease processes(38.6%±25.7%,n=5 vs 20.0%±30.9%,n=14,P=0.087).CONCLUSION:Increased expression of the apoptotic marker,Annexin-V is detected on vitreous microparticles in diabetes-related vitreous hemorrhage.When evaluating vitreous hemorrhage in patients without diabetes,the apoptotic signal is not significantly different.Vitrectomy in patients with diabetes,and improvement in visual outcomes,may be related to the removal of a serumderived,pro-apoptotic vitreous.Further investigation is warranted in order to identify the molecular characteristics of microparticles that regulate disease.Harris Sultan Rithwick Rajagopal Prabakar Kumar Rao Kisha Deslee Piggott Michael A Paley Lynn Marisa Hassman Albert S.Li Brigid Marshall Rajendra Shridhar Apte 2022International Journal of Ophthalmology(English edition)2022,15,1:0
