4Management of antiplatelet or anticoagulant therapy in endoscopy: A review of literature显示文摘Endoscopic procedures hold a basal risk of bleeding that depends on the type of procedure and patients’comorbidities.Moreover,they are often performed in patients taking antiplatelet and anticoagulants agents,increasing the potential risk of intraprocedural and delayed bleeding.Even if the interruption of antithrombotic therapies is undoubtful effective in reducing the risk of bleeding,the thromboembolic risk that follows their suspension should not be underestimated.Therefore,it is fundamental for each endoscopist to be aware of the bleeding risk for every procedure,in order to measure the risk-benefit ratio for each patient.Moreover,knowledge of the proper management of antithrombotic agents before endoscopy,as well as the adequate timing for their resumption is essential.This review aims to analyze current evidence from literature assessing,for each procedure,the basal risk of bleeding and the risk of bleeding in patients taking antithrombotic therapy,as well as to review the recommendation of American society for gastrointestinal endoscopy,European society of gastrointestinal endoscopy,British society of gastroenterology,Asian pacific association of gastroenterology and Asian pacific society for digestive endoscopy guidelines for the management of antithrombotic agents in urgent and elective endoscopic procedures.Marcello Maida Sandro Sferrazza Carlo Maida Gaetano Cristian Morreale Alessandro Vitello Giovanni Longo Vincenzo Garofalo Emanuele Sinagra 2020World Journal of Gastrointestinal Endoscopy2020,12,6:1

6A LoRa-based protocol for connecting IoT edge computing nodes to provide small-data-based services显示文摘Data is becoming increasingly personal.Individuals regularly interact with a variety of structured data,ranging from SQLite databases on the phone to personal sensors and open government data.The“digital traces left by individuals through these interactions”are sometimes referred to as“small data”.Examples of“small data”include driving records,biometric measurements,search histories,weather forecasts and usage alerts.In this paper,we present a flexible protocol called LoRaCTP,which is based on LoRa technology that allows data“chunks”to be transferred over large distances with very low energy expenditure.LoRaCTP provides all the mechanisms necessary to make LoRa transfer reliable by introducing a lightweight connection setup and allowing the ideal sending of an as-long-as necessary data message.We designed this protocol as communication support for small-data edge-based IoT solutions,given its stability,low power usage,and the possibility to cover long distances.We evaluated our protocol using various data content sizes and communication distances to demonstrate its performance and reliability.Kiyoshy Nakamura Pietro Manzoni Alessandro Redondi Edoardo Longo Marco Zennaro Juan-Carlos Cano Carlos T.Calafate 2022Digital Communications and Networks2022,8,3:0
7Gallbladder hemorrhage–An uncommon surgical emergency:A case report显示文摘BACKGROUND Gallbladder hemorrhage is a life-threatening disorder.Trauma(accidental or iatrogenic such as a percutaneous biopsy or cholecystectomy surgery),cholelithiasis,biliary tract parasitosis,vasculitis,vascular malformations,autoimmune and neoplastic diseases and coagulopathies have been described as causes of hemorrhage within the lumen of the gallbladder.The use of nonsteroidal anti-inflammatory drugs and anticoagulants may represent a risk factor.CASE SUMMARY We report the case of a 76-year-old male patient.An urgent contrast computed tomography scan demonstrated relevant distension of the gallbladder filled with hyperdense non-homogeneous content.The gallbladder walls were of regular thickness.Near the anterior wall a focus of suspected active bleeding was observed.Due to the progressive decrease in hemoglobin despite three blood transfusions,this was an indication for urgent surgery.CONCLUSION Early diagnosis of this potentially fatal pathology is essential in order to plan a strategy and eventually proceed with urgent surgical treatment.Maria Rosaria Valenti Andrea Cavallaro Maria Di Vita Antonio Zanghi Giovanni Longo Trischitta Alessandro Cappellani 2022World Journal of Clinical Cases2022,10,27:0
8Dose escalation of adalimumab as a strategy to overcome anti-drug antibodies:A case report of infantile-onset inflammatory bowel disease显示文摘BACKGROUND Treatment of infantile-onset inflammatory bowel disease(IO-IBD)is often challenging due to its aggressive disease course and failure of standard therapies with a need for biologics.Secondary loss of response is frequently caused by the production of anti-drug antibodies,a well-known problem in IBD patients on biologic treatment.We present a case of IO-IBD treated with therapeutic drug monitoring(TDM)-guided high-dose anti-tumor necrosis factor therapy,in which dose escalation monitoring was used as a strategy to overcome anti-drug antibodies.CASE SUMMARY A 5-mo-old boy presented with a history of persistent hematochezia from the 10th d of life,as well as relapsing perianal abscess and growth failure.Hypoalbuminemia,anemia,and elevated inflammatory markers were also present.Endoscopic assessment revealed skip lesions with deep colic ulcerations,inflammatory anal sub-stenosis,and deep fissures with persistent abscess.A diagnosis of IO-IBD Crohn-like was made.The patient was initially treated with oral steroids and fistulotomy.After the perianal abscess healed,adalimumab(ADA)was administered with concomitant gradual tapering of steroids.Clinical and biochemical steroid-free remission was achieved with good trough levels.After 3 mo,antibodies to ADA(ATA)were found with undetectable trough levels;therefore,we optimized the therapy schedule,first administering 10 mg weekly and subsequently up to 20 mg weekly(2.8 mg/kg/dose).After 2 mo of high-dose treatment,ATA disappeared,with concomitant high trough levels and stable clinical and biochemical remission of the disease.CONCLUSION TDM-guided high-dose ADA treatment as a monotherapy overcame ATA production.This strategy could be a good alternative to combination therapy,especially in very young patients.Silvana Ancona Sara Signa Chiara Longo Giuliana Cangemi Roberta Carfora Enrico Drago Alessandro La Rosa Marco Crocco Andrea Chiaro Paolo Gandullia Serena Arrigo 2023World Journal of Gastroenterology2023,29,38:0
9Observatory science with eXTP显示文摘In this White Paper we present the potential of the enhanced X-ray Timing and Polarimetry(eXTP) mission for studies related to Observatory Science targets. These include flaring stars, supernova remnants, accreting white dwarfs, low and high mass X-ray binaries, radio quiet and radio loud active galactic nuclei, tidal disruption events, and gamma-ray bursts. eXTP will be excellently suited to study one common aspect of these objects: their often transient nature.
