1Management of chronic hepatitis B infection: Current treatment guidelines, challenges, and new developments显示文摘Chronic hepatitis B(CHB)virus infection is a global public health problem,affecting more than 400 million people worldwide.The clinical spectrum is wide,ranging from a subclinical inactive carrier state,to progressive chronic hepatitis,cirrhosis,decompensation,and hepatocellular carcinoma.However,complications of hepatitis B virus(HBV)-related chronic liver disease may be reduced by viral suppression.Current international guidelines recommend first-line treatment of CHB infection with pegylated interferon,entecavir,or tenofovir,but the optimal treatment for an individualpatient is controversial.The indications for treatment are contentious,and increasing evidence suggests that HBV genotyping,as well as serial on-treatment measurements of hepatitis B surface antigen and HBV DNA kinetics should be used to predict antiviral treatment response.The likelihood of achieving a sustained virological response is also increased by extending treatment duration,and using combination therapy.Hence the paradigm for treatment of CHB is constantly evolving.This article summarizes the different indications for treatment,and systematically reviews the evidence for the efficacy of various antiviral agents.It further discusses the shortcomings of current guidelines,use of rescue therapy in drug-resistant strains of HBV,and highlights the promising clinical trials for emerging therapies in the pipeline.This concise overview presents an updated practical approach to guide the clinical management of CHB.Ceen-Ming Tang Tung On Yau Jun Yu 2014World Journal of Gastroenterology2014,20,20:41
2Evolution of systemic therapy of advanced hepatocellular carcinoma显示文摘Hepatocellular carcinoma (HCC) commonly occurs in hepatitis B endemic areas, especially in Asian countries. HCC is highly refractory to cytotoxic chemotherapy. This resistance is partly related to its tumor biology, pharmacokinetic properties, and both intrinsic and acquired drug resistance. There is no convincing evidence thus far that systemic chemotherapy improves overall survival in advanced HCC patients. Other systemic approaches, such as hormonal therapy and immunotherapy, have also disappointing results. Recently, encouraging results have been shown in using sorafenib in the treatment of advanced HCC patients. 5Preliminary experience with drug-coated balloon angioplasty in primary percutaneous coronary intervention显示文摘We evaluated the clinical feasibility of using drugcoated balloon(DCB) angioplasty in patients undergoingprimary percutaneous coronary intervention(PPCI). Between January 2010 to September 2014,89 STelevation myocardial infarction patients(83% male,mean age 59 ± 14 years) with a total of 89 coronary lesions were treated with DCB during PPCI. Clinical outcomes are reported at 30 d follow-up. Left anterior descending artery was the most common target vessel for PCI(37%). Twenty-eight percent of the patients had underlying diabetes mellitus. Mean left ventricular ejection fraction was 44% ± 11%. DCB-only PCI was the predominant approach(96%) with the remaining 4% of patients receiving bail-out stenting. Thrombolysis in Myocardial Infarction(TIMI) 3 flow was successfully restored in 98% of patients. An average of 1.2 ± 0.5 DCB were used per patient,with mean DCB diameter of 2.6 ± 0.5 mm and average length of 23.2 ± 10.2 mm. At 30-d follow-up,there were 4 deaths(4.5%). No patients experienced abrupt closure of the infarctrelated artery and there was no reported target-lesion failure. 6Energy-efficient Virtual Machine Allocation Technique Using Flower Pollination Algorithm in Cloud Datacenter:A Panacea to Green Computing显示文摘Cloud computing has attracted significant interest due to the increasing service demands from organizations offloading computationally intensive tasks to datacenters.Meanwhile,datacenter infrastructure comprises hardware resources that consume high amount of energy and give out carbon emissions at hazardous levels.In cloud datacenter,Virtual Machines(VMs)need to be allocated on various Physical Machines(PMs)in order to minimize resource wastage and increase energy efficiency.Resource allocation problem is NP-hard.Hence finding an exact solution is complicated especially for large-scale datacenters.In this con text,this paper proposes an Energy-oriented Flower Pollination Algorithm(E-FPA)for VM allocation in cloud datacenter environments.A system framework for the scheme was developed to enable energy-oriented allocation of various VMs on a PM.The allocation uses a strategy called Dynamic Switching Probability(DSP).The framework finds a near optimal solution quickly and balances the exploration of the global search and exploitation of the local search.It considers a processor,storage,and memory constraints of a PM while prioritizing energy-oriented allocation for a set of VMs.Simulations performed on MultiRecCloudSim utilizing planet workload show that the E-FPA outperforms the Genetic Algorithm for Power-Aware(GAPA)by 21.8%,Order of Exchange Migration(OEM)ant colony system by 21.5%,and First Fit Decreasing(FFD)by 24.9%.Therefore,E-FPA significantly improves datacenter performance and thus,enhances environmental sustainability.Mohammed Joda Usman Abdul Samad Ismail Hassan Chizari Gaddafi Abdul-Salaam Ali Muhammad Usman Abdulsalam Yau Gital Omprakash Kaiwartya Ahmed Aliyu 2019Journal of Bionic Engineering2019,16,2:13
7“原位开窗”技术重建主动脉弓上分支血管的Meta分析显示文摘目的系统评价应用'原位开窗'技术治疗胸主动脉(夹层)动脉瘤重建主动脉弓上分支血管的安全性及有效性。方法收集截止2018年8月前发表的关于应用'原位开窗'技术腔内修复主动脉弓部病变(夹层/动脉瘤),同时保留弓上血管血供的文献,回顾性分析技术成功率、内漏发生率、并发症发生率及病死率等近中期临床结果。结果共纳入11篇文献的262例胸主动脉(夹层)动脉瘤患者,技术成功率为94%,平均随访时间为16.85个月,共13例患者出现术后支架内漏(Ⅰ型内漏10例,Ⅱ型内漏3例),术后内漏总发生率为8%,30天病死率和随访期手术相关病死率分别为3.44%和0。结论应用'原位开窗'技术重建弓上分支血管技术成功率较高,并发症发生率及病死率较低。周楚 Tung On Yau 乔彤 2018中国血管外科杂志(电子版)2018,10,3:11
8Epigenetic dysregulation in Epstein-Barr virus-associated gastric carcinoma: Disease and treatments显示文摘Epstein-Barr virus(EBV)-associated gastric carcinoma(EBVaGC)comprises nearly 10%of gastric carcinoma cases worldwide.Recently,it was recognised to have unique clinicopathologic characteristics,including male predominance,lower rates of lymph node involvement,and better prognosis.EBVaGC is further characterised by abnormal hypermethylation of tumour suppressor gene promoter regions,causing down-regulation of their expression.In the present review,we critically discuss the role of EBV in gastric carcinogenesis,summarising the role of viral proteins and microRNAs with respect to aberrant methylation in EBVaGC.Given the role of epigenetic dysregulation in tumourigenesis,epigenetic modifiers may represent a novel therapeutic strategy.Tung On Yau Ceen-Ming Tang Jun Yu 2014World Journal of Gastroenterology2014,20,21:11

10A Geometric Understanding of Deep Learning显示文摘This work introduces an optimal transportation(OT)view of generative adversarial networks(GANs).Natural datasets have intrinsic patterns,which can be summarized as the manifold distribution principle:the distribution of a class of data is close to a low-dimensional manifold.GANs mainly accomplish two tasks:manifold learning and probability distribution transformation.The latter can be carried out using the classical OT method.From the OT perspective,the generator computes the OT map,while the discriminator computes the Wasserstein distance between the generated data distribution and the real data distribution;both can be reduced to a convex geometric optimization process.Furthermore,OT theory discovers the intrinsic collaborative-instead of competitive-relation between the generator and the discriminator,and the fundamental reason for mode collapse.We also propose a novel generative model,which uses an autoencoder(AE)for manifold learning and OT map for probability distribution transformation.This AE–OT model improves the theoretical rigor and transparency,as well as the computational stability and efficiency;in particular,it eliminates the mode collapse.The experimental results validate our hypothesis,and demonstrate the advantages of our proposed model.Na Lei Dongsheng An Yang Guo Kehua Su Shixia Liu Zhongxuan Luo Shing-Tung Yau Xianfeng Gu 2020Engineering2020,6,3:10

12Geometric aspects of the moduli space of Riemann surfaces显示文摘We describe some recent progress in the study of moduli space of Riemann surfaces in this survey paper. New complete Kahler metrics were introduced on the moduli space and Teichmuller space. Their curvature properties and asymptotic behavior were studied in details. These natural metrics served as bridges to connect all the known canonical metrics, especially the Kahler-Einstein metric. We showed that all the known complete metrics on the moduli space are equivalent and have Poincare type growth. Furthermore,the Kahler-Einstein metric has strongly bounded geometry. 13Fat cell-secreted adiponectin mediates physical exercise-induced hippocampal neurogenesis: an alternative anti-depressive treatment?显示文摘Psychological depression is drawing accumulating attention nowadays, due to the skyrocketing incidence worldwide and the enormous burdens it incurs. Physical exercise has been long recognized for its therapeutic effects on depressive disorders, although knowledge of the underlying mechanisms remains limited. Suppressed hippocampal neurogenesis in adult brains has been regarded, at least partly, contributive to depression, whereas physical exercise that restores neurogenesis accordingly exerts the anti-depressive action. Several recent publications have suggested the potential role of adiponectin, a protein hormone secreted by peripheral mature adipocytes, in mediating physical exercise-triggered enhancement of hippocampal neurogenesis and alleviation of depression.
