| 1 | 基于IMRT时代的第八版AJCC/UICC鼻咽癌临床分期建议显示文摘目的准确的分期系统对癌症的治疗至关重要。随着癌症分期和治疗方法的演变,需要不断评价分期的适用性和改进性。方法基于第7版AJCC/UICC分期回顾性分析香港和中国大陆2个肿瘤中心收治的1609例接受调强放射治疗的首诊无转移鼻咽癌患者临床资料,所有患者治疗前均行核磁共振分期评估。结果无其他T3、T4期解剖结构受侵患者中,伴有咀嚼肌间隙(翼内肌和/或翼外肌)侵犯、椎前肌侵犯及咽旁间隙侵犯的三组患者之间0s相近。伴广泛软组织(上述侵犯结构以外的软组织)受侵患者OS与伴有颅内侵犯或颅神经侵犯相似。仅2%患者锁骨上窝以上淋巴结转移者直径〉6cm,其0s率与下颈淋巴结转移者类似。用下颈(环状软骨尾侧缘水平以下)代替锁骨上窝并不影响N分期之间的风险差异性。采用推荐的T、N分期,T。N。、T。N,期0s相近。结论经AJCC/UICC分期筹备委员会审阅后,建议第8版分期应将翼内肌/翼外肌从T。降到T:期,增加椎前肌为L期,用下颈取代锁骨上窝,将淋巴结最大直径〉6cm合并归为N,期,将T。、N,期统一归为Ⅳ。期。这些改变不仅使得相邻分期间风险差异性更好,而且使得临床实践性与全球适用性之间达到最佳平衡。 | 潘建基 Wai Tong Ng 宗井凤 Lucy L.K.Chan Brian O' Sullivan 林少俊 Henry C.K.Sze 陈韵彬 Horace C.W.Choi 郭巧娟 Wai Kuen Kan 肖友平 Xu Wei Quynh Thu Le Christine M.Glastonbury A.Dimtrios Colevas Randal S.Weber Jatin P.Shah Anne W.M.Lee | 2016 | 中华放射肿瘤学杂志2016,25,3: | 65 |
| 2 | Clinicopathological and prognostic analysis of 429 patients with intrahepatic cholangiocarcinoma显示文摘AIM:To understand the clinicopathological characteristics and treatment selections and improve survival and provide valuable information for patients with intrahepatic cholangiocarcinoma(ICC). METHODS:We retrospectively evaluated 5311 liver cancer patients who received resection between October 1999 and December 2003.Of these,429(8.1%)patients were diagnosed with ICC,and their clinicopathological, surgical,and survival characteristics were analyzed. RESULTS:Upper abdominal discomfort or pain(65.0%), no symptoms(12.1%),and hypodynamia(8.2%)were the major causes for medical attention.Laboratory tests showed 198(46.4%)patients were HBsAg positive, 90(21.3%)hadα-fetoprotein>20μg/L,50(11.9%) carcinoembryonic antigen>10μg/L,and 242(57.5%) carbohydrate antigen 19-9(CA19-9)>37 U/mL.Survival data was available for 329(76.7%)patients and their mean survival time was 12.4 mo.The overall survival of the patients with R0,R1 resection and punching exploration were 18.3,6.6 and 5.6 mo,respectively. Additionally,CA19-9>37 U/mL was associated with lymph node metastases,but inversely associated withcirrhosis.Multivariate analysis indicated that radical resection,lymph node metastases,macroscopic tumor thrombi and size,and CA19-9 were associated with prognosis. | 3 | Risk factors for early recurrence of small hepatocellular carcinoma after curative resection显示文摘BACKGROUND:Poorer prognosis is seen in patients with hepatocellular carcinoma (HCC) after curative hepatic resection with early recurrence (≤1 year) than in those with late recurrence (>1 year).This study aimed to identify risk factors for postoperative early recurrence of small HCC (≤3 cm in diameter).METHODS:The study population consisted of 158 patients who underwent curative resection for small HCC between January 2002 and July 2004.Risk factors for early recurrence were analyzed.RESULTS:Thirty-three (20.8%) patients developed early recurrence after surgery.Univariate analysis showed the following significant risk factors for early recurrence in small HCC:serum alpha-fetoprotein (AFP) level >100 ng/ml,lack of tumor capsule formation,microscopic vascular invasion,high Edmonson-Steiner grades,and cytokeratin-19 (CK-19) expression (P<0.05).Multivariate stepwise logistic regression analysis showed that serum AFP level >100 ng/ml (odds ratio 2.561,95% confidence interval 1.057 to 6.206,P=0.037) and microscopic vascular invasion (odds ratio 4.549,95% confidence interval 1.865 to 11.097,P=0.001) were independent factors.CONCLUSIONS:Postoperative early recurrence is related to serum AFP level >100 ng/ml and microscopic vascular invasion in patients with small HCC.Adjuvant therapy and careful follow-up are required for patients with these risk factors. | Zhou, Yan-Ming Yang, Jia-Mei Li, Bin Yin, Zheng-Feng Xu, Feng Wang, Bin Xu, Wen Kan, Tong | 2010 | Hepatobiliary & Pancreatic Diseases International2010,9,1: | 24 |

| 7 | Hepatectomy in the treatment of very big primary liver cancer: report of 86 cases显示文摘Objective: To study the indications for resection ofvery big primary liver cancer and the operative re-sults.Methods: From January 1985 to June 1996, 86 pa-tients with very big primary liver cancer (≥15cm indiameter) underwent hepatectomy in our hospital.The volume of bleeding and blood transfusion wasrecorded during the operation. After the operation,the draining quantity from their abdominal cavities,and the days of transfusion and hospitalization wererecorded. The occurrence of complications and sur-vival time of the patient were followed up.Results: The postoperative mortality was 3.48% andthe occurrence rate of complications was 31.40%, whichwas significantly correlated with preoperative lowerlevel of serum albumin or the elevated γ-globulin lev-el and the amount of resected liver tissue. But their liverfunction before operation was fairly good, the 1-, 3-and 5-year survival rates after hepatectomy were58.2%, 35.7% and 17.64%.Conclusions: Patients with very big primary liver can-cer, should be subjected to hepatectomy if their liverfunction before operation are normal and the marginsare distinct between the tumor and liver tissues. | 8 | Excessive gestational weight gain in early pregnancy and insufficient gestational weight gain in middle pregnancy increased risk of gestational diabetes mellitus显示文摘Background:Gestational weight gain(GWG)is associated with the risk of gestational diabetes mellitus(GDM).However,the effect of weight gain in different trimesters on the risk of GDM is unclear.This study aimed to evaluate the effect of GWG on GDM during different trimesters.Methods:A birth cohort study was conducted from 2017 to 2020 in Shenzhen,China.In total,51,205 participants were included comprising two models(early pregnancy model and middle pregnancy model).Gestational weight(kg)was measured at each prenatal clinical visit using a standardized weight scale.Logistic regression analysis was used to assess the risk of GDM.Interaction analysis and mediation effect analysis were performed in the middle pregnancy model.Results:In the early pregnancy model,the risk of GDM was 0.858 times lower(95%confidence interval[CI]:0.786,0.937)with insufficient GWG(iGWG)and 1.201 times higher(95%CI:1.097,1.316)with excessive GWG after adjustment.In the middle pregnancy model,the risk of GDM associated with iGWG increased 1.595 times(95%CI:1.418,1.794)after adjustment;for excessive GWG,no significant difference was found(P=0.223).Interaction analysis showed no interaction between GWG in early pregnancy(GWG-E)and GWG in middle pregnancy(GWG-M)(F=1.268;P=0.280).The mediation effect analysis indicated that GWG-M plays a partial mediating role,with an effect proportion of 14.9%.Conclusions:eGWG-E and iGWG-M are associated with an increased risk of GDM.Strict control of weight gain in early pregnancy is needed,and sufficient nutrition should be provided in middle pregnancy. | Aiqi Yin Fuying Tian Xiaoxia Wu Yixuan Chen Kan Liu Jianing Tong Xiaonian Guan Huafan Zhang Linlin Wu Jianmin Niu | 2022 | Chinese Medical Journal2022,,9: | 5 |

Methods: 72 patients with advanced primary liver cancer with no indication for surgery received intraperitoneal chemotherapy in combination with other therapies including transcatheter arterial chemoem- bolization (TACE), radiofrequency catheter ablation (RFA), percutaneous ethanol injection therapy (PEIT) and radiotherapy. Of them, 29 cases were complicated with hilar or retroperitoneal multiple lymph node metastases, 14 with portal vein em- bolus, 15 with intraperitoneal and diaphragmatic metastases, 6 with chylous ascites, one with cancerous ascites, and 7 with suspected cancerous ascites (referring to large amounts of ascites without hypoproteinemia while exfoliative cytology of the ascites was positive). The mean maximum tumor size was 8.2 cm in diameter. Liver function at the initial treatment was Child A in 53 cases, and Child B in 19 cases. Intraperitoneal chemotherapy was performed in all these patients. The intraperitoneal chemotherapy protocols included: 5-FU 0.5-0.75 g/d for 10-15 consecutive days, with a total dosage of 5-12.5 g, and at the last day of chemotherapy 10 mg mitomycin (MMC) or 100 mg carboplatin was injected. For 7 cases of cholangiocarcinoma, Gemzar 800-1000 mg was administered additionally. A majority of all these patients received another one or two therapy methods followed by intraperitoneal chemotherapy. TACE was performed in the patients with multiple tumors or nodule more than 5 cm in diameter in the liver, RFA or PEIT with nodule fewer than 4 in number and 5 cm or less than 5 cm in diameter and radiotherapy, only for metastases, with metastatic lymph nodes, localized metastasis within the abdominal cavity or portal vein embolus. Interval time between two methods was one month or so. Two months after the sequential therapy, repeated treatment would be given if general medical condition and liver function were perfect at that time. Results: The median sur- vival time of the group was 13.97 ± 6.27 months. The 1- and 2-year survival rates were 59.7% and 30.6% respectively. The mean survival time of the patients with liver function Child A was 15.91 ± 5.49 months, and that of the patients with Child B was 8.55 ± 5.09 months. The difference was statistically significant (P < 0.05). Conclusion: Intraperitoneal chemotherapy or in combination with other therapies in patients with advanced primary liver cancer with metastases to abdominal cavity is an effective method. | 19 | High-mobility induced high-performance self-powered ultraviolet photodetector based on single ZnO microwire/PEDOT:PSS heterojunction via slight ga-doping显示文摘Semiconductor micro/nanostructures with broad bandgap can provide powerful candidates for fabricating ultraviolet photodetectors(PDs)due to their proper bandgap,unique optoelectronic properties,large surface-to-volume ratio and good integration.However,semiconducting micro/nanostructures suffer from low electron conductivity and abundant surface defects,which greatly limits their practical application in developing PDs.In this work,an ultraviolet PD consisting of single Ga-doped ZnO microwire(ZnO:Ga MW)and p-type poly(3,4 ethylenedioxythiophene):poly(styrenesulfonate)(PEDOT:PSS)was designed.When exposed to ultraviolet illumination,the PD exhibits excellent performance(responsivity~185 m A/W,detectivity~2.4×10^(11) Jones,and fast response speed of~212μs for rise time and~387μs for decay time)under self-driven conditions.Compared with that of an undoped Zn O MWbased PD,the responsivity and detectivity of ZnO:Ga MW/PEDOT:PSS PD are significantly enhanced over 400%and 600%,respectively.Due to the incorporation of Ga element,the charge transport properties of a ZnO:Ga MW,specifically for the mobility,are effectively enhanced,which can substantially facilitate the generation,separation,transport and harvest efficiency of photo-generated carriers in the asfabricated PD.Besides,the Ga-incorporation improves the crystalline quality of MWs and reduces surface state density,further suggesting a high-quality ZnO:Ga MW/PEDOT:PSS heterojunction.This work provides a potential approach for designing high-performance self-powered ultraviolet PDs from the aspect of enhancing carrier transport through fine doping. | Peng Wan Mingming Jiang Tong Xu Yang Liu Caixia Kan | 2021 | Journal of Materials Science & Technology2021,,34: | 1 |
