| 9 | Direct-acting antivirals for chronic hepatitis C treatment: The experience of two tertiary university centers in Brazil显示文摘BACKGROUND Hepatitis C virus(HCV)treatment has undergone major changes in recent years.Previous interferon-based therapies have been replaced by oral direct-acting antivirals(DAA)regimens,with high sustained virologic response(SVR)rates,and a lower incidence of adverse events(AEs).AIM To evaluate the efficacy and safety of DAAs for HCV treatment in subjects from two tertiary university centers in Brazil.METHODS This is a multicenter retrospective cohort study of 532 patients with chronic hepatitis C(CHC),undergoing treatment with interferon-free regimens from November 2015 to November 2019.The therapeutic regimen was defined by the current Brazilian guidelines for HCV management at the time of treatment.Demographic,anthropometric,clinical,and laboratory variables were evaluated.SVRs were assessed at 12 to 24 wk after therapy by intention-to-treat(ITT),and modified ITT(m-ITT)analysis.AEs and serious adverse events(SAEs)were registered.In the statistical analysis,a P value of<0.05 was considered significant.RESULTS The mean age was 56.88 years,with 415(78.5%)being HCV genotype 1,followed by genotype 3(20.1%).Moreover,306(57.5%)subjects had cirrhosis,and a third of them had decompensated cirrhosis.Sofosbuvir(SOF)plus daclatasvir±ribavirin was the most frequently used treatment(66.9%),followed by SOF plus simeprevir(21.2%).The overall ITT SVR was 92.6%(493/532),while the m-ITT SVR was 96.8%(493/509).Variables associated with treatment failure via ITT evaluation were hepatic encephalopathy(OR:4.320;95%CI:1.920-9.721,P=0.0004),presence of esophageal varices(OR:2.381;95%CI:1.137-4.988,P=0.0215),previous portal hypertensive bleeding(OR:2.756;95%CI:1.173-6.471,P=0.02),higher model for end-stage liver disease scores(OR:1.143,95%CI:1.060–1.233,P=0.0005),lower serum albumin levels(OR:0.528,95%CI:0.322-0.867,P=0.0115),higher serum creatinine(OR:1.117,95%CI:1.056-1.312,P=0.0033),and international normalized ratio(INR)levels(OR:5.542,95%CI:2.023-15.182,P=0.0009).AEs were reported in 41.1%(211/514)of patients,and SAEs in 3.7%.The female gender,higher body mass index,esophageal varices,higher INR values,and longer treatment duration were independently associated with AE occurrence.CONCLUSION Treatment with oral DAAs attains a high SVR rate,with fewer SAEs in a real-life cohort of subjects with CHC,from two tertiary university centers in Brazil. | Mariana Sandoval Lourenço Patricia Momoyo Y Zitelli Marlone Cunha-Silva Arthur Ivan N Oliveira Cláudia P Oliveira Tiago Sevá-Pereira Flair JoséCarrilho Mario G Pessoa Daniel F Mazo | 2022 | World Journal of Hepatology2022,14,1: | 1 |

| 15 | Use of hybrid chitosan membranes and human mesenchymal stem cells from the Wharton jelly of umbilical cord for promoting nerve regeneration in an axonotmesis rat model显示文摘Many studies have been dedicated to the development of scaffolds for improving post-traumatic nerve regeneration. The goal of this study was to assess the effect on nerve regeneration, associating a hybrid chitosan membrane with non-differentiated human mesenchymal stem cells isolated from Wharton's jelly of umbilical cord, in peripheral nerve reconstruction after crush injury. Chromosome analysis on human mesenchymal stem cell line from Wharton's jelly was carried out and no structural alterations were found in metaphase. Chitosan membranes were previously tested in vitro, to assess their ability in supporting human mesenchymal stem cell survival, expansion, and differentiation. For the in vivo testing, Sasco Sprague adult rats were divided in 4 groups of 6 or 7 animals each:Group 1, sciatic axonotmesis injury without any other intervention (Group 1-Crush); Group 2, the axonotmesis lesion of 3 mm was infiltrated with a suspension of 1 250-1 500 human mesenchymal stem cells (total volume of 50 μL) (Group 2-CrushCell); Group 3, axonotmesis lesion of 3 mm was enwrapped with a chitosan type III membrane covered with a monolayer of non-differentiated human mesenchymal stem cells (Group 3-CrushChitIIICell) and Group 4, axonotmesis lesion of 3 mm was enwrapped with a chitosan type III membrane (Group 4-CrushChitIII). Motor and sensory functional recovery was evaluated throughout a healing period of 12 weeks using sciatic functional index, static sciatic index, extensor postural thrust, and withdrawal reflex latency. Stereological analysis was carried out on regenerated nerve fibers. Results showed that infiltration of human mesenchymal stem cells, or the combination of chitosan membrane enwrapment and human mesenchymal stem cell enrichment after nerve crush injury provide a slight advantage to post-traumatic nerve regeneration. Results obtained with chitosan type III membrane alone confirmed that they significantly improve post-traumatic axonal regrowth and may represent a very promising clinical tool in peripheral nerve reconstructive surgery. | 20 | Lithic soils in the semi-arid region of Brazil:edaphic characterization and susceptibility to erosion显示文摘Soils(Leptosols or Epileptic Regosols)with lithic contact at a depth of 50 cm occupy almost 20%of the Brazilian semi-arid region.These lithic soils are susceptible to erosion due to faster saturation of water-holding capacity during rainfall,which accelerates the beginning of runoff.However,erosion traits of lithic soils in the semi-arid region of Brazil are less studied.The aim of this study was to characterize the soil and landscape attributes in areas with Neossolos Litólicos(Entisols)in the Caatinga biome to identify region of high susceptibility to erosion.Results showed that the soils were characterized by a sandy texture,soil structure with poor development and low content of organic carbon.These attributes increase susceptibility to erosion and reduce water storage capacity,especially in the states of Cearáand Sergipe.In these states,the content of rock fragments in the soil reaches 790 g/kg.High contents of silt and fine sand,high silt/clay ratio,predominance of Leptosols and strong rainfall erosivity were observed in Piauíand northwestern Ceará.A very high degree of water erosion was observed in the states of Pernambuco and Paraíba.Despite the low degree of erosion observed in the state of Bahia,it is highly susceptible to erosion due to the predominance of very shallow soils,rugged relief and high values of rainfall erosivity.Lower vulnerability was observed in the state of Alagoas because of its more smoothed relief,greater effective soil depth,thicker A horizon of soil and lower rainfall erosivity.In general,the characteristics that intensify the susceptibility to erosion in the Caatinga biome are those soil structures with poor development or without aggregation,low contents of organic carbon,high contents of silt and fine sand,high values of silt/clay ratio and rugged relief in some regions.This study collected information contributing to a better characterization of soils with lithic contact in the semi-arid region of Brazil.In addition,regions with a higher susceptibility to erosion were identified,revealing insights that could help develop strategies for environmental risk mitigation. | Carlos R PINHEIRO JUNIOR Conan A SALVADOR Tiago R TAVARES Marcel C ABREU Hugo S FAGUNDES Wilk S ALMEIDA Eduardo C SILVA NETO Lúcia H C ANJOS Marcos G PEREIRA | 2022 | Journal of Arid Land2022,14,1: | 0 |
