1Phacoemulsification versus small incision cataract surgery in patients with uveitis显示文摘AIM: To compare the safety and efficacy of phacoemulsification and small incision cataract surgery(SICS) in patients with uveitic cataract.·METHODS: In a prospective, randomized multi-centric study, consecutive patients with uveitic cataract were randomized to receive phacoemulsification or manual SICS by either of two surgeons well versed with both the techniques. A minimum inflammation free period of 3mo(defined as less than 5 cells per high power field in anterior chamber) was a pre-requisite for eligibility for surgery. Superior scleral tunnel incisions were used for both techniques. Improvement in visual acuity post-operatively was the primary outcome measure and the rate of post-operative complications and surgical time were secondary outcome measures, respectively. Means of groups were compared using t-tests. One way analysis of variance(ANOVA) was used when there were more than two groups. Chi-square tests were used for proportions. Kaplan Meyer survival analysis was done and means for survival time was estimated at 95%confidence interval(CI). A P value of <0.05 was considered statistically significant.·RESULTS: One hundred and twenty-six of 139 patients(90.6%) completed the 6-month follow-up. Seven patients were lost in follow up and another six excluded due to either follow-up less than six months(n=1) or inability implant an intraocular lens(IOL) because of insufficient capsular support following posterior capsule rupture(n=5). There was significant improvement in vision after both the procedures(paired t-test; P <0.001). On first postoperative day, uncorrected distance visual acuity(UDVA) was 20/63 or better in 31(47%)patients in Phaco group and 26(43.3%) patients in SICS group(P=0.384). The mean surgically inducedastigmatism(SIA) was 0.86 ±0.34 dioptres(D) in the phacoemulsification group and 1.16 ±0.28 D in SICS group. The difference between the groups was significant(t-test, P =0.002). At 6mo, corrected distance visual acuity(CDVA) was 20/60 or better in 60(90.9%) patients in Phaco group and 53(88.3%) in the manual SICS group(P =0.478). The mean surgical time was significantly shorter in the manual SICS group(10.8±2.9 versus 13.2±2.6min)(P <0.001). Oral prednisolone, 1 mg/kg body weight was given 7d prior to surgery, continued post-operatively and tapered according to the inflammatory response over 4-6wk in patients with previously documented macular edema, recurrent uveitis, chronic anterior uveitis and intermediate uveitis. Rate of complications like macular edema(Chi-square, P =0.459),persistent uveitis(Chi-square, P =0.289) and posterior capsule opacification(Chi-square, P =0.474) were comparable between both the groups.·CONCLUSION: Manual SICS and phacoemulsification do not differ significantly in complication rates and final CDVA outcomes. However, manual SICS is significantly faster. It may be the preferred technique in settings where surgical volume is high and access to phacoemulsification is limited, such as in eye camps. 2Plant Disease Diagnosis and Image Classification Using Deep Learning显示文摘Indian agriculture is striving to achieve sustainable intensification,the system aiming to increase agricultural yield per unit area without harming natural resources and the ecosystem.Modern farming employs technology to improve productivity.Early and accurate analysis and diagnosis of plant disease is very helpful in reducing plant diseases and improving plant health and food crop productivity.Plant disease experts are not available in remote areas thus there is a requirement of automatic low-cost,approachable and reliable solutions to identify the plant diseases without the laboratory inspection and expert’s opinion.Deep learning-based computer vision techniques like Convolutional Neural Network(CNN)and traditional machine learning-based image classification approaches are being applied to identify plant diseases.In this paper,the CNN model is proposed for the classification of rice and potato plant leaf diseases.Rice leaves are diagnosed with bacterial blight,blast,brown spot and tungro diseases.Potato leaf images are classified into three classes:healthy leaves,early blight and late blight diseases.Rice leaf dataset with 5932 images and 1500 potato leaf images are used in the study.The proposed CNN model was able to learn hidden patterns from the raw images and classify rice images with 99.58%accuracy and potato leaves with 97.66%accuracy.The results demonstrate that the proposed CNN model performed better when compared with other machine learning image classifiers such as Support Vector Machine(SVM),K-Nearest Neighbors(KNN),Decision Tree and Random Forest.Rahul Sharma Amar Singh Kavita N.Z.Jhanjhi Mehedi Masud Emad Sami Jaha Sahil Verma 2022Computers, Materials & Continua2022,,5:4
3A Comprehensive Review of Animal Models for Coronaviruses: SARSCoV-2, SARS-CoV, and MERS-CoV显示文摘The recent outbreak of coronavirus disease(COVID-19) caused by the novel severe acute respiratory syndrome coronavirus 2(SARS-CoV-2) has already affected a large population of the world. SARS-CoV-2 belongs to the same family of severe acute respiratory syndrome coronavirus(SARS-CoV) and Middle East respiratory syndrome coronavirus(MERSCoV). COVID-19 has a complex pathology involving severe acute respiratory infection, hyper-immune response, and coagulopathy. At present, there is no therapeutic drug or vaccine approved for the disease. There is an urgent need for an ideal animal model that can reflect clinical symptoms and underlying etiopathogenesis similar to COVID-19 patients which can be further used for evaluation of underlying mechanisms, potential vaccines, and therapeutic strategies. 5Feedback linearization based control for weak grid connected PV system under normal and abnormal conditions显示文摘This paper proposes a control strategy for interface of distributed energy sources into the weak grid system with a focus on the energy and ancillary services.A novel controller has been designed and implemented to tackle the challenges of coupling terms in the LCL filter,the transient behavior under sudden changes,and the voltage support under fault condition using the feedback linearization technique.The controller proposed has been implemented on the PV system connected with the weak grid using the LCL filter and the performance of the controller has been verified using Matlab/Simulink through simulation under different conditions.The results of the controller proposed have been compared with the conventional PI dual loop controller.The simulation results obtained demonstrate the effectiveness and simplicity of the controller design strategy.Rahul SHARMA Sathans SUHAG 2020Frontiers in Energy2020,14,2:2
6Erythropoietin therapy after out-of-hospital cardiac arrest:A systematic review and meta-analysis显示文摘AIM To assess safety and efficacy of early erythropoietin(Epo) administration in patients with out-of-hospital cardiac arrest(OHCA).METHODS A systematic literature search was performed using PubM ed,MEDLINE,EMBASE,EBSCO,CINAHL,Web of Science and Cochrane databases,of all studies published from the inception through October 10,2016.Inclusion criteria included:(1) Adult humans with OHCA and successful sustained return of spontaneous circulation;and(2) studies including mortality/brain death,acute thrombotic events as their end points.Primary efficacyoutcome was 'brain death or Cerebral Performance Category(CPC) score of 5'.Secondary outcomes were 'CPC score 1,and 2-4','overall thrombotic events' and 'acute coronary stent thrombosis'.RESULTS We analyzed a total of 606 participants(n = 276 received Epo and n = 330 with standard of care alone) who experienced OHCA enrolled in 3 clinical trials.No significant difference was observed between the Epo and no Epo group in brain death or CPC score 5(OR = 0.77;95%CI:0.42-1.39),CPC score 1(OR = 1.16,95%CI:0.82-1.64),and CPC score 2-4(OR = 0.77,95%CI:0.44-1.36).Epo group was associated with increased thrombotic complications(OR = 2.41,95%CI:1.26-4.62) and acute coronary stent thrombosis(OR = 8.16,95%CI:1.39-47.99).No publication bias was observed.CONCLUSION Our study demonstrates no improvement in neurological outcomes and increased incidence of thrombotic events and acute coronary stent thrombosis in OHCA patients who were treated with Epo in addition to standard therapy.Rahul Chaudhary Jalaj Garg Parasuram Krishnamoorthy Kevin Bliden Neeraj Shah Nayan Agarwal Rahul Gupta Abhishek Sharma Karl B Kern Nainesh C Patel Paul Gurbel 2017World Journal of Cardiology2017,9,12:2

11Universal converse flexoelectricity in dielectric materials via varying electric field direction显示文摘Flexoelectricity is a symmetry independent electromechanical cou-pling phenomenon that outperforms piezoelectricity at micro and nanoscales due to its size-dependent behavior arising from gradi-ent terms in its constitutive relations.However,due to this gradient term flexoelectricity,to exhibit itself,requires specially designed geometry or material composition of the dielectric material.First of its kind,the present study put forward a novel strategy of achieving electric field gradient and thereby converse flexoelectri-city,independent of geometry and material composition of the material.The spatial variation of the electric field is established inside the dielectric material,Ba_(0.67)Sr_(0.33)TiO_(3)(BST),by manipulating electrical boundary conditions.Three unique patterns of electrode placement are suggested to achieve this spatial variation.This varying direction of electric field gives rise to electric field gradient,the prerequisite of converse flexoelectricity.A multi-physics cou-pling based theoretical framework is established to solve the flexo-electric actuation by employing isogeometric analysis(IGA).Electromechanically coupled equations of flexoelectricity are solved to obtain the electric field distribution and the resulting displace-ments thereby.The maximum displacements of 0.2 nm and 2.36 nm are obtained with patterns I and II,respectively,while pattern III can yield up to 85 nm of maximum displacement.Saurav Sharma Rajeev Kumar Rahul Vaish 2021International Journal of Smart and Nano Materials2021,12,1:1

15Critical care practice in India:Results of the intensive care unit need assessment survey(ININ2018)显示文摘BACKGROUND A diverse country like India may have variable intensive care units(ICUs)practices at state and city levels.AIM To gain insight into clinical services and processes of care in ICUs in India,this would help plan for potential educational and quality improvement interventions.METHODS The Indian ICU needs assessment research group of diverse-skilled individuals was formed.A pan-India survey'Indian National ICU Needs'assessment(ININ 2018-I)was designed on google forms and deployed from July 23rd-August 25th,2018.The survey was sent to select distribution lists of ICU providers from all 29 states and 7 union territories(UTs).In addition to emails and phone calls,social medial applications-WhatsApp™,Facebook™and LinkedIn™were used to remind and motivate providers.By completing and submitting the survey,providers gave their consent for research purposes.This study was deemed eligible for category-2 Institutional Review Board exempt status.RESULTS There were total 134 adult/adult-pediatrics ICU responses from 24(83%out of 29)states,and two(28%out of 7)UTs in 61 cities.They had median(IQR)16(10-25)beds and most,were mixed medical-surgical,111(83%),with 108(81%)being adult-only ICUs.Representative responders were young,median(IQR),38(32-44)years age and majority,n=108(81%)were males.The consultants were,n=101(75%).A total of 77(57%)reported to have 24 h in-house intensivist.A total of 68(51%)ICUs reported to have either 2:1 or 2≥:1 patient:nurse ratio.More than 80%of the ICUs were open,and mixed type.Protocols followed regularly by the ICUs included sepsis care,ventilator-associated pneumonia(83%each);nutrition(82%),deep vein thrombosis prophylaxis(87%),stress ulcer prophylaxis(88%)and glycemic control(92%).Digital infrastructure was found to be poor,with only 46%of the ICUs reporting high-speed internet availability.CONCLUSION In this large,national,semi-structured,need-assessment survey,the need for improved manpower including;in-house intensivists,and decreasing patient-tonurse ratios was evident.Sepsis was the most common diagnosis and quality and research initiatives to decrease sepsis mortality and ICU length of stay could be prioritized.Additionally,subsequent surveys can focus on digital infrastructure for standardized care and efficient resource utilization and enhancing compliance with existing protocols.Rahul Kashyap Kirtivardhan Vashistha Chetan Saini Taru Dutt Dileep Raman Vikas Bansal Harpreet Singh Geeta Bhandari Nagarajan Ramakrishnan Harshit Seth Divya Sharma Premkumar Seshadri Mradul Kumar Daga Mohan Gurjar Yash Javeri Salim Surani Joseph Varon 2020World Journal of Critical Care Medicine2020,9,2:1
