| 1 | Disaster preparedness, pediatric considerations in primary blast injury, chemical, and biological terrorism显示文摘Both domestic and foreign terror incidents are an unfortunate outgrowth of our modern times from the Oklahoma City bombings, Sarin gas attacks in Japan, the Madrid train bombing, anthrax spores in the mail, to the World Trade Center on September 11 th, 2001. The modalities used to perpetrate these terrorist acts range from conventional weapons to high explosives, chemical weapons, and biological weapons all of which have been used in the recent past. While these weapons platforms can cause significant injury requiring critical care the mechanism of injury, pathophysiology and treatment of these injuries are unfamiliar to many critical care providers. Additionally the pediatric population is particularly vulnerable to these types of attacks. In the event of a mass casualty incident both adult and pediatric critical care practitioners will likely be called upon to care for children and adults alike. We will review the presentation, pathophysiology, and treatment of victims of blast injury, chemical weapons, and biological weapons. The focus will be on those injuries not commonly encountered in critical care practice, primary blast injuries, category A pathogens likely to be used in terrorist incidents, and chemical weapons including nerve agents, vesicants, pulmonary agents, cyanide, and riot control agents with special attention paid to pediatric specific considerations. | Mitchell Hamele William Bradley Poss Jill Sweney | 2014 | World Journal of Critical Care Medicine2014，3，1： | 14 |

| 3 | Rapid assembly of multilayer microfluidic structures via 3D-printed transfer molding and bonding显示文摘A critical feature of state-of-the-art microfluidic technologies is the ability to fabricate multilayer structures without relying on the expensive equipment and facilities required by soft lithography-defined processes.Here,three-dimensional(3D)printed polymer molds are used to construct multilayer poly(dimethylsiloxane)(PDMS)devices by employing unique molding,bonding,alignment,and rapid assembly processes.Specifically,a novel single-layer,two-sided molding method is developed to realize two channel levels,non-planar membranes/valves,vertical interconnects(vias)between channel levels,and integrated inlet/outlet ports for fast linkages to external fluidic systems.As a demonstration,a single-layer membrane microvalve is constructed and tested by applying various gate pressures under parametric variation of source pressure,illustrating a high degree of flow rate control.In addition,multilayer structures are fabricated through an intralayer bonding procedure that uses custom 3D-printed stamps to selectively apply uncured liquid PDMS adhesive only to bonding interfaces without clogging fluidic channels.Using integrated alignment marks to accurately position both stamps and individual layers,this technique is demonstrated by rapidly assembling a six-layer microfluidic device.By combining the versatility of 3D printing while retaining the favorable mechanical and biological properties of PDMS,this work can potentially open up a new class of manufacturing techniques for multilayer microfluidic systems. | Casey C.Glick Mitchell T.Srimongkol Aaron J.Schwartz William S.Zhuang Joseph C.Lin Roseanne H.Warren Dennis R.Tekell Panitan A.Satamalee Liwei Lin | 2016 | Microsystems & Nanoengineering2016，2，1： | 4 |

| 5 | Herbal extracts as hepatoprotectants against acetaminophen hepatotoxicity显示文摘Many plant-derived natural products have the potential to be hepatoprotective and therefore can be used to treat acute and chronic liver diseases. The challenge is to identify the most promising compounds and evaluate their protective mechanism. In a recently published article, Wang et al evaluated extracts of the plant Gentiana manshurica Kitagawa (GM) in a model of acetaminophen hepatotoxicity. The authors concluded that GM is hepatoprotective against acetaminopheninduced liver injury due to its antioxidant properties and anti-apoptotic capacity. | 10 | 采用间接复位、钢板固定、不植骨的方法治疗稳定的股骨干髓内假体周围骨折显示文摘背景:间接复位技术相对于较早期的开放复位技术具有能够提高骨愈合率及减少植骨的优势,我们对以间接复位治疗股骨干假体周围骨折的结果进行了研究。
方法:连续对50例稳定的股骨干髓内假体周围骨折(Vancourvr B1型)进行治疗,根据病例记录包括切开复位、间接复位、外侧单钢板固定且不行同种异体或其他植骨。4例术后早期死亡,5例随访时间短,对其余41例(平均年龄72岁)进行了平均24个月的随访,并进行临床和影像学评估。
结果:所有骨折平均12周(7~23周)愈合,对位满意。1例有1根钢丝断裂,2例有1枚螺钉断裂,骨折最终愈合且无明显的内置物松动和对位不良。1例发生围手术期深部感染。41例中30例行走功能恢复到骨折前的状态。
结论:结果证实间接复位骨膜外侧单钢板固定且不进行植骨的方法对于稳定的股骨干髓内假体周围骨折的疗效是应该肯定的。

| 11 | Factors associated with refractory pain in emergency patients admitted to emergency general surgery显示文摘BACKGROUND: Oligoanalgesia in emergency departments (EDs) is multifactorial. A previousstudy reported that emergency providers did not adequately manage patients with severe paindespite objective findings for surgical pathologies. Our study aims to investigate clinical andlaboratory factors, in addition to providers’ interventions, that might have been associated witholigoanalgesia in a group of ED patients with moderate and severe pains due to surgical pathologies.METHODS: We conducted a retrospective study of adult patients who were transferred directlyfrom referring EDs to the emergency general surgery (EGS) service at a quaternary academic centerbetween January 2014 and December 2016. Patients who were intubated, did not have adequaterecords, or had mild pain were excluded. The primary outcome was refractory pain, which wasdefi ned as pain reduction <2 units on the 0–10 pain scale between triage and ED departure.RESULTS: We analyzed 200 patients, and 58 (29%) had refractory pain. Patients with refractory painhad signifi cantly higher disease severity, serum lactate (3.4±2.0 mg/dL vs. 1.4±0.9 mg/dL, P=0.001), and lessfrequent pain medication administration (median [interquartile range], 3 [3–5] vs. 4 [3–7], P=0.001), whencompared to patients with no refractory pain.
