| 1 | Massive duodenal variceal bleed; complication of extra hepatic portal hypertension: Endoscopic management and literature review显示文摘Bleeding from duodenal varices is reported to be a catastrophic and often fatal event. Most of the cases in the literature involve patients with underlying cirrhosis. However, approximately one quarter of duodenal variceal bleeds is caused by extrahepatic portal hypertension and they represent a unique population given their lack of liver dysfunction. The authors present a case where a 61-year-old male with history of remote crush injury presented with bright red blood per rectum and was found to have bleeding from massive duodenal varices. Injection sclerotherapy with ethanolamine was performed and the patient experienced a favorable outcome with near resolution of his varices on endoscopic follow-up. | 2 | Seasonal variations of polybrominated flame retardants bound to car dust under Mediterranean climate显示文摘Polybrominated diphenyl ethers(PBDEs) are commercial flame retardants that have been commonly used in vehicle interior to reduce fire-related hazards. Due to high temperatures and intense insolation that can be attained inside cars parked in the sun, additive PBDEs are prone to leach out and attach to in-vehicle dust, as well as to photo-debrominate. This study examines seasonal variations of concentrations of three common PBDE congeners(BDE-47, BDE-99 and BDE-209) in car dust in Israel. The overall concentrations of these BDEs ranged from 1 to 29 μg/g, and were higher in the summer than in the winter(average of 10.2 and5.3 μg/g, respectively). Congener-specific concentrations showed distinct seasonal pattern,representing the interplay between leaching, evaporation and photodebromination. Photolysis of the three congeners, while adsorbed on glass filters and exposed to solar radiation, revealed first order kinetics with debromination rates on the order of 10-2/min. | 3 | Strategic Analysis of a Regulatory Conflict Using Dempster-Shafer Theory and AHP for Preference Elicitation显示文摘Dempster-Shafer Theory (DST) and the Analytic Hierarchy Process (AHP) are integrated in order to elicit preference information from experts regarding decision makers (DMs) involved in a regulatory conflict. More precisely, DST is used for combining expert knowledge regarding preferences of a specific DM (the regulatory body), and AHP is employed for ranking feasible states in the conflict for this same DM. In order to illustrate how this preference elicitation proposal can be conveniently implemented in practice within the Graph Model for Conflict Resolution (GMCR), it is applied to a real construction dispute located in the city of Ipojuca, Brazil. The conflict is modeled with three DMs: support, opposition, and the regulatory body. Results show that the new preference methodology possesses many inherent advantages including high flexibility the ability to capture uncertainty or even ignorance about preferences, the possibility of combining expert knowledge with respect to missing preferences, and a substantial reduction in the number of pairwise comparisons of states required to express preference information. | Maisa M. | 13 | Prevalence and outcome of acute kidney injury,as defined by the new Kidney Disease Improving Global Outcomes guideline,in very low birth weight infants显示文摘AIM To evaluate the prevalence, risk factors and outcome of acute kidney injury(AKI) in very low birth weight(VLBW) infants. METHODS In this retrospective study of VLBW infants, we analyzed the prevalence of AKI, as defined by changes in serum creatinine and urine output, associated risk factors and outcomes.RESULTS A total of 293 VLBW infants(mean gestational age 28.7 wk) were included, of whom 109 weighed less than 1000 g at birth. The overall prevalence of AKI was 11.6%(22% in infants with a birth weight under 1000 g and 5.4% those heavier). A total of 19(55%) affected infants died, with a mortality rate of 58% in infant less than 1000 g and 50% in those heavier. After adjusting for confounding variables, only necrotizing enterocolitis(NEC) remained associated with AKI, with odds ratio of 4.9(95%CI: 1.9-18.6). Blood pressure and glomerular filtration rate(GFR) were not different between affected infants and the others upon discharge from hospital. A normal GFR was documented in all affected infants at one year of age.CONCLUSION Using Kidney Disease Improving Global Outcomes definition of AKI, it occurred in over 10% of VLBW infants,more commonly in infants with lower birth weight. NEC was an independent associated risk factor.
