| 2 | Adverse Effects of Exposure to Fine Particulate Matters and Ozone on Gestational Hypertension显示文摘Gestational hypertension(GH)is a common complication during pregnancy.GH is regarded as a potential public health challenge for pregnant women and infants.Limited evidence has linked ambient air pollution to an increased GH risk.However,most of the studies were conducted in developed countries,with inconsistent results obtained.The present study was performed to explore whether exposure to particulate matters with an aerodynamic diameter≤2.5(PM2.5)and ozone(O3)was related to elevated odds of GH in a Chinese population.This population-based cohort study involved 38115 pregnant women in Wuhan,China.All information was collected from the Wuhan Maternal and Child Health Management Information System,using standardized quality control.The daily air pollutant data for PM2.5 and O3 were obtained from the 20 monitoring stations of the Wuhan Environmental Monitoring Center during 2014.The nearest monitor approach was applied to individual exposure assessment of PM2.5 and O3 for each participant.After adjusting for major confounders and other air pollutants,a 10μg/m^3 increase in PM25 and O3 concentrations was found to correlate to a 1.14-fold[95%confidence interval(95%CI):1.09,1.20]and a 1.05-fold(95%CI:1.02,1.07)increase in GH risk,respectively.Additionally,stronger relationships between GH risk and PM25 and O3 exposure were observed in women who conceived in winter and summer,respectively.These findings suggest that air pollutants may contribute to the development of GH. | Rong YANG Dan LUO Yi-ming ZHANG Ke HU Zheng-min QIAN Li-qin HU Long-jiao SHEN Hong XIAN Juliet Iwelunmor Su-rong MEI | 2019 | Current Medical Science2019，39，6： | 7 |

| 4 | Phytoestrogens and prevention of breast cancer: The contentious debate显示文摘Phytoestrogens have multiple actions within target cells, including the epigenome, which could be beneficial to the development and progression of breast cancer. In this brief review the action of phytoestrogens on oestrogen receptors, cell signalling pathways, regulation of the cell cycle, apoptosis, steroid synthesis and epigenetic events in relation to breast cancer are discussed. Phytoestrogens can bind weakly to oestrogen receptors(ERs) and some have a preferential affinity for ERβ which can inhibit the transcriptional growthpromoting activity of ERα. However only saturating doses of phytoestrogens, stimulating both ERα and β, exert growth inhibitory effects. Such effects on growth may be through phytoestrogens inhibiting cell signalling pathways. Phytoestrogens have also been shown to inhibit cyclin D1 expression but increase the expression of cyclin-dependent kinase inhibitors(p21 and p27) and the tumour suppressor gene p53. Again these effects are only observed at high(> 10) μmol/L doses of phytoestrogens. Finally the effects of phytoestrogens on breast cancer may be mediated by their ability toinhibit local oestrogen synthesis and induce epigenetic changes. | 6 | Intestinal schistosomiasis in Uganda at high altitude(>1400 m):malacological and epidemiological surveys on Mount Elgon and in Fort Portal crater lakes reveal extra preventive chemotherapy needs显示文摘Background:Intestinal schistosomiasis is of public health importance in Uganda but communities living above 1400 m are not targeted for control as natural transmission is thought unlikely.To assess altitudinal boundaries and at-risk populations,conjoint malacological and epidemiological surveys were undertaken on Mount Elgon(1139 m-3937 m),in Fort Portal crater lakes and in the Rwenzori Mountains(1123 m-4050 m).Methods:Seventy freshwater habitats[Mount Elgon(37),Fort Portal crater lakes(23),Rwenzori Mountains(8)and Lake Albert(2)]were inspected for Biomphalaria species.Water temperature,pH and conductivity were recorded.A parasitological examination of 756 schoolchildren[Mount Elgon(300),Fort Portal crater lakes(456)]by faecal microscopy of duplicate Kato-Katz smears from two consecutive stool samples was bolstered by antigen(urine-CCA dipstick)and antibody(SEA-ELISA)diagnostic assays.Results:Biomphalaria spp.was found up to 1951 m on Mount Elgon and 1567 m in the Fort Portal crater lakes.Although no snail from Mount Elgon shed cercariae,molecular analysis judged 7.1%of snails sampled at altitudes above 1400 m as having DNA of Schistosoma mansoni;in Fort Portal crater lakes three snails shed schistosome cercariae.Prevalence of intestinal schistosomiasis as measured in schoolchildren by Kato-Katz(Mount Elgon=5.3%v.Fort Portal crater lakes=10.7%),CCA urine-dipsticks(18.3%v.34.4%)and SEA-ELISA(42.3%v.63.7%)showed negative associations with increasing altitude with some evidence of infection up to 2000 m.Conclusions:Contrary to expectations,these surveys clearly show that natural transmission of intestinal schistosomiasis occurs above 1400 m,possibly extending up to 2000 m.Using spatial epidemiological predictions,this now places some extra six million people at-risk,denoting an expansion of preventive chemotherapy needs in Uganda. | Michelle CStanton Moses Adriko Moses Arinaitwe Alison Howell Juliet Davies Gillian Allison EJames LaCourse Edridah Muheki Narcis BKabatereine J.Russell Stothard | 2017 | Infectious Diseases of Poverty2017，6，1： | 3 |
