| 1 | Lymph node imaging in initial staging of prostate cancer:An overview and update显示文摘Accurate nodal staging at the time of diagnosis of prostate cancer is crucial in determining a treatment plan for the patient.Pelvic lymph node dissection is the most reliable method,but is less than perfect and has increased morbidity.Cross sectional imaging with computed tomography(CT) and magnetic resonance imaging(MRI) are non-invasive tools that rely on morphologic characteristics such as shape and size of the lymph nodes.However,lymph nodes harboring metastatic disease may be normal sized and non-metastatic lymph nodes may be enlarged due to reactive hyperplasia.The optimal strategy for preoperative staging remains a topic of ongoing research.Advanced imaging techniques to assess lymph nodes in the setting of prostate cancer utilizing novel MRI contrast agents as well as positron emission tomography(PET) tracers have been developed and continue to be studied.Magnetic resonance lymphography utilizing ultra-small super paramagnetic iron oxide has shown promising results in detection of metastatic lymph nodes.Combining MRL with diffusion-weighted imaging may also improve accuracy.Considerable efforts are being made to develop effective PET radiotracers that are performed using hybrid-imaging systems that combine PET with CT or MRI.PET tracers that will be reviewed in this article include [^(18)F]fluoro-D-glucose,sodium [^(18)F]fluoride,[^(18)F]choline,[^(11)C]choline,prostate specific membrane antigen binding ligands,[^(11)C]acetate,[^(18)F]fluciclovine,gastrin releasing peptide receptor ligands,and androgen binding receptors.This article will review these advanced imaging modalities and ability to detect prostate cancer metastasis to lymph nodes.While more research isneeded,these novel techniques to image lymph nodes in the setting of prostate cancer show a promising future in improving initial lymph node staging. | Jessica G Zarzour Sam Galgano Jonathan McConathy John V Thomas Soroush Rais-Bahrami | 2017 | World Journal of Radiology2017,9,10: | 5 |

| 3 | The role of endoscopy in the management of hereditary diffuse gastric cancer syndrome显示文摘Hereditary diffuse gastric cancer(HDGC) syndrome is an inherited cancer risk syndrome associated with path-ogenic germline CDH1 variants. Given the high risk for developing diffuse gastric cancer, CDH1 carriers are recommended to undergo prophylactic total gastrectomy for cancer risk reduction. Current guidelines recommend upper endoscopy in CDH1 carriers prior to surgery and then annually for individuals deferring prophylactic total gastrectomy.Management of individuals from HDGC families without CDH1 pathogenic variants remains less clear, and management of families with CDH1 pathogenic variants in the absence of a family history of gastric cancer is particularly problematic at present. Despite adherence to surveillance protocols, endoscopic detection of cancer foci in HDGC is suboptimal and imperfect for facilitating decision-making. Alternative endoscopic modalities, such as chromoendoscopy,endoscopic ultrasound, and other non-white light methods have been utilized,but are of limited utility to further improve cancer detection and risk stratification in HDGC. Herein, we review what is known and what remains unclear about endoscopic surveillance for HDGC, among individuals with and without germline CDH1 pathogenic variants. | 4 | Antibiotic-loaded phosphatidylcholine inhibits staphylococcal bone infection显示文摘AIM: To test antibiotic-loaded coating for efficacy in reducing bacterial biofilm and development of osteomyelitis in an orthopaedic model of implant infection. METHODS: Phosphatidylcholine coatings loaded with 25% vancomycin were applied to washed and sterilized titanium wires 20 mm in length. A 10 mm segment was removed from rabbit radius(total = 9; 5 coated, 4 uncoated), and the segment was injected with 1 × 106 colony forming units(CFUs) of Staphylococcus aureus(UAMS-1 strain). Titanium wires were inserted throughthe intramedullary canal of the removed segment and into the proximal radial segment and the segment was placed back into the defect. After 7 d, limbs were removed, X-rayed, swabbed for tissue contamination. Wires were removed and processed to determine attached CFUs. Tissue was swabbed and streaked on agar plates to determine bacteriological score.RESULTS: Antibiotic-loaded coatings resulted in significantly reduced biofilm formation(4.7 fold reduction in CFUs; P < 0.001) on titanium wires and reduced bacteriological score in surrounding tissue(4.0 ± 0 for uncoated, 1.25 ± 0.5 for coated; P = 0.01). Swelling and pus formation was evident in uncoated controls at the 7 d time point both visually and radiographically, but not in antibiotic-loaded coatings.CONCLUSION: Active antibiotic was released from coated implants and significantly reduced signs of osteomyelitic symptoms. Implant coatings were well tolerated in bone. Further studies with additional control groups and longer time periods are warranted.
