| 1 | Long-term outcome of percutaneous ethanol injection therapy for minimum-sized hepatocellular carcinoma显示文摘AIM: To evaluate long-term follow-up of minimum-sized hepatocellular carcinoma (HCC) treated with percutaneous ethanol injection (PEI). METHODS: PEI was applied to 42 lesions in 31 patients (23 male and eight female) with HCC < 15 mm in diameter, over the past 15 years. RESULTS: Overall survival rate was 74.1% at 3 years, 49.9% at 5 years, 27.2% at 7 years and 14.5% at 10 years. These results are superior to, or at least the same as those for hepatic resection and radiofrequency ablation. Survival was affected only by liver function, but not by sex, age, etiology of Hepatitis B virus or Hepatitis C virus, α-fetoprotein levels, arterial and portal blood flow, histological characteristics, and tumor multiplicity or size. Patients in Child-Pugh class A and B had 5-, 7- and 10-years survival rates of 76.0%, 42.2% and 15.8%, and 17.1%, 8.6% and 0%, respectively (P = 0.025). | 3 | Elevated blood pressure: Our family's fault?The genetics of essential hypertension显示文摘AIM: To provide an updated review on current genetic aspects possibly affecting essential hypertension(EH), and to further elucidate their role in EH. METHODS: We searched for genetic and epigenetic factors in major studies associated with EH between Jan 2008-Oct 2013 using PubMed. We limited our search to reviews that discussed mostly human studies, and were accessible through the university online re-source. We found 11 genome wide association studies(GWAS), as well as five methylation and three miRNA studies that fit our search criteria. A distinction was not made between genes with protective effects or nega-tive effects, as this article is only meant to be a sum-mary of genes associated with any aspect of EH.RESULTS: We found 130 genes from the studies that met our inclusion/exclusion criteria. Of note, genes withmultiple study references include: STK39, CYP17A1, MTHFR-NPPA, MTHFR-NPPB, ATP2B1, CSK, ZNF652, UMOD, CACNB2, PLEKHA7, SH2B3, TBX3-TBX5, ULK4, CSK-ULK3, CYP1A2, NT5C2, CYP171A, PLCD3, SH2B3, ATXN2, CACNB2, PLEKHA7, SH2B3, TBX3-TBX5, ULK4, and HFE. The following genes overlapped between the genetic studies and epigenetic studies: WNK4 and BDKRB2. Several of the identified genes were found to have functions associated with EH. Many epigenetic factors were also correlated with EH. Of the epigenetic factors, there were no articles discussing siRNA and its effects on EH that met the search criteria, thus the topic was not included in this review. Among the miRNA tar-gets found to be associated with EH, many of the genes involved were also identified in the GWAS studies.CONCLUSION: Genetic hypertension risk algorithms could be developed in the future but may be of limited benefit due to the multi-factorial nature of EH. | 4 | Diagnostic sensitivity of imaging modalities for hepatocellular carcinoma smaller than 2 cm显示文摘AIM:To compare the imaging results with histology and to evaluate the diagnostic sensitivity of imaging modalities for hepatocellular carcinoma(HCC)smaller than 2 cm.METHODS:Nodules smaller than 2 cm(n=34)revealed by ultrasonography(US)in 29 patients with liver cirrhosis were analyzed.Histological diagnosis of HCC was performed by ultrasonographic guidance:moderately-differentiated HCC(n=24);well-differentiated HCC(n=10).The patterns disclosed by the four imaging modalities defined the conclusive diagnosis of HCC:(1)contrast-enhanced computed tomography(CECT),hypervascularity in the arterial phase and washout in the equilibrium phase;(2)Sonazoid contrast-enhanced US(CEUS),hypervascularity in the early vascular phase and defect in the Kupffer phase;(3)gadolinium-ethoxybenzyl-diethylenetriamine pentaacetic acid(Gd-EOBDTPA)-enhanced magnetic resonance imaging(MRI),hypervascularity in the arterial phase and/or defect in the hepatobiliary phase;and(4)CT arterioportal angiography:hypervascularity by CT during arteriography and/ or perfusion defect by CT during arterial portography.RESULTS:Overall,the sensitivity of diagnosing HCC smaller than 2 cm was 52.9%(18/34)(95%CI:35.170.2)by CECT;67.6%(23/34)(95%CI:49.5-82.6)by Sonazoid CEUS;76.5%(26/34)(95%CI:58.8-89.3) by Gd-EOB-DTPA MRI;and 88.2%(30/34)(95%CI: 72.5-96.7)by CT arterioportal angiography.The diagnostic sensitivity of detecting moderately-differentiated HCC by CECT,Sonazoid CEUS,Gd-EOB-DTPA MRI and CT arterioportal angiography was 62.5%(15/24)(95%CI: 40.6-81.2),79.2%(19/24)(95%CI:57.8-92.9),75.0% (18/24)(95%CI:53.3-90.2)and 95.8%(23/24)(95% CI:78.9-99.9),respectively.A significant difference(P< 0.05)was observed between CECT and CT arterioportal angiography in all nodules.There was no difference between Sonazoid CEUS,Gd-EOB-DTPA MRI,and CT arterioportal angiography.The combined sensitivity of Sonazoid CEUS and Gd-EOB-DTPA MRI was 94.1%(32/34).CONCLUSION:Changing the main diagnostic modality for HCC smaller than 2 cm from CT arterioportal angiography to Sonazoid CEUS and Gd-EOB-DTPA MRI is recommended. | Keiji Mita Soo Ryang Kim Masatoshi Kudo Susumu Imoto Taisuke Nakajima Kenji Ando Katsumi Fukuda Toshiyuki Matsuoka Yoko Maekawa Yoshitake Hayashi | 2010 | World Journal of Gastroenterology2010,16,33: | 7 |

| 6 | Scirrhous hepatocellular carcinoma displaying atypical findings on imaging studies显示文摘We describe a 15-mm scirrhous hepatocellular carcinoma(HCC) in a 60-year-old man with B-type cirrhosis.Ultrasound disclosed a 15-mm hypoechoic nodule in segment 7.Contrast-enhanced US revealed heterogeneous,not diffuse,hypervascularity in the early phase and a defect in the Kupffer phase.Contrast-enhanced computed tomography(CT) revealed a heterogeneous hypervascular nodule in the early phase and a low-density area in the late phase.Magnetic resonance imaging(MRI) revealed iso-to hypointensity at T1 and high intensity at T2-weighted sequences.Contrast-enhanced MRI also revealed a heterogeneous hypervascular nodule in the early phase and washout in the late phase.Super-paramagnetic iron oxide-MRI revealed a hyperintense nodule.CT during hepatic arteriography and CT during arterial portography revealed heterogeneous hyperattenuation and a perfusion defect,respectively.Based on these imaging findings the nodule was diagnosed as a mixed well-differentiated and moderately-differentiated HCC.Histologically,the nodule was moderately-differentiated HCC characterized by typical cytological and structural atypia with dense fibrosis.Immunohistochemically,the nodule was positive for heterochromatin protein 1 and alpha-smooth muscle actin,and negative for cytokeratin 19.From the above findings,the nodule was diagnosed as scirrhous HCC.Clinicians engaged in hepatology should exercise caution with suspected scirrhous HCC when imaging studies reveal atypical findings,as shown in our case on the basis of chronic liver disease. | Soo Ryang Kim Susumu Imoto Taisuke Nakajima Kenji Ando Keiji Mita Katsumi Fukuda Ryo Nishikawa Yu-ichiro Koma Toshiyuki Matsuoka Masatoshi Kudo Yoshitake Hayashi | 2009 | World Journal of Gastroenterology2009,15,18: | 4 |

| 9 | Inhibitory effect of voglibose and gymnemic acid on maltose absorption in vivo显示文摘AIM To determine whether diabetic care can be improved by combination of voglibose and gymnemic acid ( GA ), we compared the combinative and individual effects of voglibose and GA on maltose absorption in small intestine.METHODS The small intestine 30 cm long from 2 cm caudal ward Treitz's ligament of Wistar rat was used as an in situ loop, which was randomly perfused in recircular mode with maltose (10 mmol/L) with or without different dosages of voglibose and/or GA for an hour. To compare the time course, perfusion of 10 mmol/L maltose was repeated four times.Each time continued for 1 hour and separated by 30 minutes rinse. In the first time, lower dosages of GA (0.5g/ L) and/ or voglibose (2μmol/L) were contained except control.RESULTS Absorptive rate of maltose was the lowest in combinative group (P< 0.05, ANOVA),for example, the inhibition rate was about 37%during the first hour when 0.5g/L GA and 2μmol/L voglibose with 10mmol/L maltose were perfused in the loop. The onset time was shortened to 30 minutes and the effective duration was prolonged to 4 hours with the combination; therefore the total amount of maltose absorption during the effective duration was inhibited more significantly than that in the individual administration (P<0.05, U test of Mann-Whitney). The effect of GA on absorptive barriers of the intestine played an important role in the combinative effects.CONCLUSION There are augmented effects of voglibose and GA. | 14 | Inhibitory effect and mechanism of acarbose combined with gymnemic acid on maltose absorption in rat intestine显示文摘AIM To compare the combinative andindividual effect of acarbose and gymnemic acid(GA) on maltose absorption and hydrolysis insmall intestine to determine whether nutrientcontrol in diabetic care can be improved bycombination of them.METHODS The absorption and hydrolysis ofmaltose were studied by cyclic perfusion ofintestinal loops in situ and motility of theintestine was recorded with the intestinal ring invitro using Wistar rats.RESULTS The total inhibitory rate of maltoseabsorption was improved by the combination ofGA (0.1 g/L 1.0 g/L) and acarbose(0.1 mmol/L 2.0 mmol/L) throughout theireffective duration (P<0.05, U test of Mann-Whitney), although the improvement only couldbe seen at a Iow dosage during the first hour.With the combination, inhibitory duration of acarbose on maltose absorption was prolonged to 3 h and the inhibitory effect onset of GA was fastened to 15min. GA suppressed the intestinal mobility with a good correlation (r-0.98) to the inhibitory effect of GA on maltose absorption and the inhibitory effect of 2mmol/L (high dose) acarbose on maltose hydrolysis was dual modulated by 1 g/L GA in vivo indicating that the combined effects involved the functional alteration of intestinal barriers.CONCLUSION There are augmented effects of acarbose and GA, which involve pre-cellular and paracellular barriers.
