| 1 | Usefulness of noninvasive transient elastography for assessment of liver fibrosis stage in chronic hepatitis C显示文摘AIM:To evaluate the method of noninvasive transient elastography for assessment of histological stage of liver fibrosis in patients with chronic hepatitis C (CHC). METHODS: Two hundred and thirty-seven patients with CHC were included in this study. Liver biopsy was performed under ultrasonography on 217 of the patients, excluding twenty with clear clinical evidence of liver cirrhosis. Fifty subjects without liver disease were enrolled as a control group (stage 0). Twenty- five patients with sustained virological response (SVR) to interferon (IFN) therapy were also enrolled. These patients underwent liver biopsy before IFN therapy. Examination of liver stiffness (LS) was performed by elastography. RESULTS: Medians (50% levels) of LS were 4.1 (3.5-4.9), 6.3 (4.8-8.5), 8.8 (6.8-12.0), 14.6 (10.5-18.6), and 22.2 (15.4-28.0), respectively, in the fibrosis stages 0-4 (P < 0.001). LS was significantly correlated with four serum fibrosis markers. LS values in patients with SVR were 3.8 (3.5-5.6), 5.2 (4.4-6.8), 6.8 (6.1-7.6), and 6.1 (3.6-7.9), respectively, in the fibrosis stages 1-4. In all stages, LS for patients with SVR was significantly lower than that for patients who did not undergo IFN therapy. LS was significantly correlated with serum concentrations of hyaluronic acid, type Ⅳ collagen, type Ⅳ collagen 7S, and type Ⅲ procollagen N peptide. CONCLUSION: LS correlated well with the histologicalstage of fibrosis. | 8 | Evaluation of seismic behavior of box culvert buried in the ground through centrifuge model tests and numerical analysis显示文摘Japan has yet to establish a seismic design for road box culverts(RBCs)because past major earthquakes did not damage them.In recent years,structures with enlarged sectional dimensions(the purpose of which is to optimize the internal space in RBCs)have become common.However,the unknown seismic strength of such large RBCs makes RBC seismic designs increasingly necessary.Several seismic design methods have been developed for and applied to rectangular underground structures,e.g.,cut-and-cover tunnels,which are structurally similar to RBCs.Although these methods are applicable to RBCs,it is uncertain whether they can be applied directly because there are currently no evaluation results from model tests on RBCs,which have unique structural features,e.g.,no haunch at the bottom of the sidewalls.Therefore,we verify a seismic behavior of an RBC and develop a method for evaluating it in order to establish a seismic design for RBCs.We conducted centrifuge model tests subjected to seismic force and a numerical analysis using an elastoplastic finite analysis method,in addition,we validated this analysis by comparing the test and analytical results.The test results show RBCs will most likely develop rocking rotation when the ground strain exceeds approximately 0.08%.A comparison of the numerical and experimental results shows that this analysis can estimate the shear deformation behavior with approximately 90%accuracy in square cross-section cases.Meanwhile,this analysis has tendency to underestimate the axial forces in each case and to overestimate the bending moments of some members in the case of wide cross-sections. | Hitoshi Yatsumoto Yasuo Mitsuyoshi Yasuo Sawamura Makoto Kimura | 2019 | Underground Space2019,4,2: | 1 |

| 16 | Right recumbent position on gastric emptying of water evidenced by ^(13)C breath testing显示文摘AIM:To compare the impact of the right recumbent position with the sitting position on gastric emptying of water. METHODS:In eight healthy male volunteers,the 13C acetate breath test was performed twice to assess gastric emptying of 100 mL tap water.Subjects were seated in one test and lying on their right side in the other.In both positions,pulmonary 13CO2 exhalation curves were obtained by plotting breath data against time.Percent gastric retention curves were created by analyzing data using the Wagner-Nelson protocol. RESULTS:No significant posture effect was found in pulmonary 13CO2 output curves(P=0.2150),whereas a significant effect was seen in gastric retention curves (P=0.0315).The percent retention values at 10 min and 15 min were significantly smaller when subjects were in the right recumbent position compared with the seated position(P<0.05).Our results verified the accelerating effect of the right recumbent position on gastric emptying of non-nutritive solutions.Concerning clinical implications,this study suggests that placing patients with acute pain on their right side after oral administration of analgesic drugs in solution is justified as an effective practice for rapid pain relief.For patients with gastrointestinal reflux symptoms,sleeping in the right recumbent position may reduce nocturnal symptoms,as delayed gastric emptying can cause reflux symptoms. | 19 | Treatment strategy for pancreatic head cancer with celiac axis stenosis in pancreaticoduodenectomy:A case report and review of literature显示文摘BACKGROUND During pancreaticoduodenectomy in patients with celiac axis(CA)stenosis due to compression by the median arcuate ligament(MAL),the MAL has to be divided to maintain hepatic blood flow in many cases.However,MAL division often fails,and success can only be determined intraoperatively.To overcome this problem,we performed endovascular CA stenting preoperatively,and thereafter safely performed pancreaticoduodenectomy.We present this case as a new preoperative treatment strategy that was successful.CASE SUMMARY A 77-year-old man with a diagnosis of pancreatic head cancer presented to our department for surgery.Preoperative assessment revealed CA stenosis caused by MAL.We performed endovascular stenting in the CA preoperatively because we knew that going into the operation without a strategy could lead to ischemic complications.Double-antiplatelet therapy(DAPT)–which is needed when a stent is inserted–was then administered in parallel with neoadjuvant chemotherapy(NAC).This allowed us to administer DAPT for a sufficient period before the main pancreaticoduodenectomy procedure while obtaining therapeutic effects from NAC.Subtotal stomach-preserving pancreaticoduodenectomy was thenperformed.The operation did not require any unusual techniques and was performed safely.Postoperatively,the patient progressed well,without any ischemic complications.Histopathologically,curative resection was confirmed,and the patient had no recurrence or complications due to ischemia up to six months postoperatively.CONCLUSION Preoperative endovascular stenting,with NAC and DAPT,is effective and safe prior to pancreaticoduodenectomy in potentially resectable pancreatic cancer. | Eiji Yoshida Yasutoshi Kimura Takuro Kyuno Ryoko Kawagishi Kei Sato Tsuyoshi Kono Takehiro Chiba Toshimoto Kimura Hitoshi Yonezawa Osamu Funato Makoto Kobayashi Kenji Murakami Akinori Takagane Ichiro Takemasa | 2022 | World Journal of Gastroenterology2022,28,8: | 1 |
