| 1 | Frozen shoulder-A prospective randomized clinical trial显示文摘AIM To compare the results of arthroscopic capsular release with intra-articular steroid injections in patients of frozen shoulder.METHODS Fifty-six patients with frozen shoulder were randomised to one of two treatment groups: Group 1, complete 360 degree arthroscopic capsular release and group 2, intra-articular corticosteroid injection(40 mg methyl prednisolone acetate). Both groups were put on active and passive range of motion exercises following the intervention. The outcome parameters were visual analogue scale(VAS) score for pain, range of motion and Constant score which were measured at baseline, 4, 8, 12, 16 and 20 wk after intervention.RESULTS All the parameters improved in both the groups. The mean VAS score improved significantly more in the group 1 as compared to group 2 at 8 wk. This greater improvement was maintained at 20 wk with P value of 0.007 at 8 wk, 0.006 at 12 wk, 0.006 at 16 wk and 0.019 at 20 wk. The Constant score showed a more significant improvement in group 1 compared to group 2 at 4 wk, which was again maintained at 20 wk with P value of 0.01 at 4, 8, 12 and 16 wk. The gain in abduction movement was statistically significantly more in arthroscopy group with P value of 0.001 at 4, 8, 12, 16 wk and 0.005 at 20 wk. The gain in external rotation was statistically significantly more in arthroscopy group with P value of 0.007 at 4 wk, 0.001 at 8, 12, and 16 wk and 0.003 at 20 wk. There was no statistically significant difference in extension and internal rotation between the two groups at any time. | 4 | Validating a point of care lactate meter in adult patients with sepsis presenting to the emergency department of a tertiary care hospital of a low-to middle-income country显示文摘BACKGROUND: Timely identifi cation of high blood lactate levels in septic patients may allow faster detection of those patients requiring immediate resuscitation. Point-of-care(POC) testing is being increasingly utilized in the emergency department(ED). We examined the accuracy and timesaving effect of a handheld POC lactate device for the measurement of fingertip and whole blood lactate as compared with reference laboratory blood testing in septic ED patients.METHODS: A convenience sample of adult ED patients receiving serum lactate testing were enrolled prospectively in the ED of a multidisciplinary tertiary care hospital serving the population of one of the major cities of Pakistan. Participants underwent fi ngertip POC lactate measurement with a portable device and simultaneous whole blood sampling for analysis by both the POC lactate device and standard laboratory method. Lactate measurements were compared by intraclass correlation(ICC) and Bland and Altman plots.RESULTS: Forty-three septic patients were included in the study. The fingertip POC & whole blood POC lactate measurements each correlated tightly with the reference method(ICC=0.93 & ICC=0.92, respectively). | 6 | Big Data Analytics in Telecommunications: Literature Review and Architecture Recommendations显示文摘This paper focuses on facilitating state-of-the-art applications of big data analytics(BDA) architectures and infrastructures to telecommunications(telecom) industrial sector.Telecom companies are dealing with terabytes to petabytes of data on a daily basis. Io T applications in telecom are further contributing to this data deluge. Recent advances in BDA have exposed new opportunities to get actionable insights from telecom big data. These benefits and the fast-changing BDA technology landscape make it important to investigate existing BDA applications to telecom sector. For this, we initially determine published research on BDA applications to telecom through a systematic literature review through which we filter 38 articles and categorize them in frameworks, use cases, literature reviews, white papers and experimental validations. We also discuss the benefits and challenges mentioned in these articles. We find that experiments are all proof of concepts(POC) on a severely limited BDA technology stack(as compared to the available technology stack), i.e.,we did not find any work focusing on full-fledged BDA implementation in an operational telecom environment. To facilitate these applications at research-level, we propose a state-of-the-art lambda architecture for BDA pipeline implementation(called Lambda Tel) based completely on open source BDA technologies and the standard Python language, along with relevant guidelines.We discovered only one research paper which presented a relatively-limited lambda architecture using the proprietary AWS cloud infrastructure. | 8 | GC/GCMS analysis of the petroleum ether and dichloromethane extracts of Moringa oleifera roots显示文摘Objective:To explore the phytochemical constituents from petroleum ether and diehloromethane extracts of Moringa oleifera(M.oleufera)roots using GC/GC—MS.Methods:A total of 5.11 kg fresh and undried crashed root of M.oleifera were cut into small pieces and extracted with petroleum ether and diehloromethane(20 L.each) at room temperature for 2 d.The concentrated extracts were subjected to their GC—MS analysis.Results:The GC-MS analysis of the petroleum ether and diehloromethane extracts of M.oleifern roots,which showed promising biological activities,has resulted in the identification 102 compounds.These constituents belong to 15 classes of compounds including hydrocarbons,fatty acids,esters,alcohols,isolhioeyanate.thiocyanale,pyrazine,aromalics.alkamides.cyanides,steroids,halocompounds.urea and N-hydroxyimine derivatives,unsaturated alkenamides.alkyne and indole.GC/GC-MS studies on petroleum ether extraet of the roots revealed that it contained 39 compounds,belonging to nine classes.Cyclooctasulfur S8 has been isolated as a pure compound from the extract.The major compounds identified from petroleum ether extract were trans-13-clocosene(37.9%).nonacosane(32.6%).cycloartenol(28.6%) nonadecanoic acid(13.9%) and cyclooctasulfur S8(13.9%).Dichloromethane extract of the roots was composed of 63 compounds of which nasimizinol(58.8%) along with oleic acid(46.5%),N—benzyl-N-(7—cyanato heptanamide(38.3%),N—benzyl-N—(1—chlorononyl) amide(30.3%),bis[3—benzyl prop-2-ene]-1-one(19.5%) and N.N-dibeuzyl—2-ene pent 1.5—diamide(11.6%) were the main constituents.Conclusions:This study helps to predict the formula and structure of active molecules which can be used as drugs.This result also enhances the traditional usage of M.oleifera which possesses a number of bioactive compounds. | Shaheen Faizi saima sumbul Muhammed Ali Versiani Rubeena Saleem Aisha Sana Hira Siddiqui | 2014 | Asian Pacific Journal of Tropical Biomedicine2014，4，8： | 2 |

| 10 | Curcumin preconditioning enhances the efficacy of adipose-derived mesenchymal stem cells to accelerate healing of burn wounds显示文摘Background:Following recent findings from our group that curcumin preconditioning augments the therapeutic efficacy of adipose-derived stem cells in the healing of diabetic wounds in rats,we aimed to investigate the regenerative effects of curcumin preconditioned adipose-derived mesenchymal stem cells(ASCs)for better recovery of acid inflicted burns in this study.Methods:ASCs were preconditioned with 5μM curcumin for 24 hours and assessed for proliferation,migration,paracrine release potential and gene expression comparative to naive ASCs.Subsequently,the healing capacity of curcumin preconditioned ASCs(Cur-ASCs)versus naive ASCs was examined using acidic wounds in rats.For this,acid inflicted burns of 20 mm in diameter were made on the back of male Wistar rats.Then,2×10^(6) cells of Cur-ASCs and naive ASCs were intradermally injected in the wound periphery(n=6)for comparison with an untreated saline control.Post-transplantation,wounds were macroscopically analysed and photographed to evaluate the percentage of wound closure and period of re-epithelization.Healed wound biopsies were excised and used for histological evaluation and expression analysis of wound healing markers at molecular level by quantitative PCR and western blotting.Results:We found that Cur-ASCs exhibited greater proliferation,migration and paracrine potential in vitro.Further,Cur-ASCs showed more effective recovery than naive ASCs as exhibited by gross morphology,faster wound closure and earlier re-epithelialization.Masson’s trichrome and hematoxylin and eosin staining demonstrated the improved architecture of the healing burns,as evidenced by reduced infiltration of inflammatory cells,compact collagen and marked granulation in Cur-ASC treated rats.Corroborating these findings,molecular assessment showed significantly reduced expressions of pro-inflammatory factors(interleukin-1 beta,interleukin-6,tumor necrosis factor alpha)a with striking upsurge of an oxidative marker(superoxide dismutase 1),proangiogenic factors(vascular endothelial growth factor,hepatocyte growth factor,hypoxia-inducible factor-1 alpha)and collagen markers(transforming growth factor beta 1,fibroblast growth factor-2,collagen type 1 alpha 1),verifying that Cur-ASCs modulate the regulation of pro-inflammatory and healing markers at burn sites.Conclusions:Treatment with Cur-ASCs resulted in faster re-epithelization of acid inflicted burns compared to the treatment with naive ASCs.Based on observed findings,we suggest the transplantation of Cur-ASCs is a valuable therapy for the potent clinical management of acidic burns. | Maryam Azam Hafiz Ghufran Hira Butt Azra Mehmood Ramla Ashfaq Asad M.Ilyas Muhammad R.Ahmad Sheikh Riazuddin | 2021 | Burns & Trauma2021，9，1： | 2 |
