| 3 | Laparoscopic vs open total gastrectomy for gastric cancer:A meta-analysis显示文摘AIM:To conduct a meta-analysis comparing laparoscopic total gastrectomy(LTG)with open total gastrectomy(OTG)for the treatment of gastric cancer.METHODS:Major databases such as Medline(PubMed),Embase,Academic Search Premier(EBSCO),Science Citation Index Expanded and the Cochrane Central Register of Controlled Trials(CENTRAL)in The Cochrane Library were searched for studies comparing LTG and OTG from January 1994 to May 2013.Evaluated endpoints were operative,postoperative and oncological outcomes.Operative outcomes included operative time and intraoperative blood loss.Postoperative recovery included time to first fatus,time to first oral intake,hospital stay and analgesics use.Postoperative complications comprised morbidity,anastomotic leakage,anastomotic stenosis,ileus,bleeding,abdominal abscess,wound problems and mortality.Oncological outcomes included positive resection margins,number of retrieved lymph nodes,and proximal and distal resection margins.The pooled effect was calculated using either a fixed effects or a random effects model.RESULTS:Fifteen non-randomized comparative studies with 2022 patients were included(LTG-811,OTG-1211).Both groups had similar short-term oncological outcomes,analgesic use(WMD-0.09;95%CI:-2.39-2.20;P=0.94)and mortality(OR=0.74;95%CI:0.24-2.31;P=0.61).However,LTG was associated with a lower intraoperative blood loss(WMD-201.19 mL;95%CI:-296.50--105.87 mL;P<0.0001)and overall complication rate(OR=0.73;95%CI:0.57-0.92;P=0.009);fewer wound-related complications(OR=0.39;95%CI:0.21-0.72;P=0.002);a quicker recovery of gastrointestinal motility with shorter time to frst fatus(WMD-0.82;95%CI:-1.18--0.45;P<0.0001)and oral intake(WMD-1.30;95%CI:-1.84--0.75;P<0.00001);and a shorter hospital stay(WMD-3.55;95%CI:-5.13--1.96;P<0.0001),albeit with a longer operation time(WMD 48.25 min;95%CI:31.15-65.35;P<0.00001),as compared with OTG.CONCLUSION:LTG is safe and effective,and may offer some advantages over OTG in the treatment of gastric cancer. | Jun-Jie Xiong Quentin M Nunes Wei Huang Chun-Lu Tan Neng-Wen Ke Si-Ming Xie Xun Ran Hao Zhang Yong-Hua Chen Xu-Bao Liu | 2013 | World Journal of Gastroenterology2013，19，44： | 11 |
| 4 | Effects of Topography and Land Use Change on Gully Development in Typical Mollisol Region of Northeast China显示文摘Due to high intensity agricultural exploitation since the middle of the 20 th century, farmland gullies have become a pervasive form of water erosion in Northeast China. Yet few researches are concentrated on how topography and land use affect long-term gully development in this region. In this study, gully distribution in a village with an area of 24.2 km^2 in the central Mollisols area of Northeast China in different times were compared by Aerial photography(1968), Quickbird image(2009) and field survey, and factors affecting gully development including land use and topography were analyzed. The results showed that the total gully number decreased from 104 to 69, while occupying area rose from 34.8 ha to 78.4 ha from 1968 to 2009. Fundamental gully distribution had been formed by 1968 as most of 2009′s gullies were evolved from 1968′s gullies′ merge and width expansion process, and new gullies those initiated after 1968 occupied only 7% of total gully area in 2009. Gully area increasing ratio in grassland was the highest and that in forestland was the lowest. The threshold catchment area between simple and complex gully development was around 15 ha to 25 ha. This threshold value sets apart catchment areas that will develop simple or complex gullies in areas with similar environmental conditions. | 7 | Utilization of longitudinal ultrasound to quantify joint soft-tissue changes in a mouse model of posttraumatic osteoarthritis显示文摘To assess the utility of longitudinal ultrasound(US) to quantify volumetric changes in joint soft tissues during the progression of posttraumatic osteoarthritis(PTOA) in mice, and validate the US results with histological findings. A longitudinal cohort of 3-month-old wild-type C57 BL/6 male mice received the Hulth-Telhag surgical procedure on right knee to induce PTOA, and sham surgery on their left knee as control. US scans were performed on both knees before, 2, 4, 6, and 8 weeks post-surgery. Joint space volume and Power-Doppler(PD) volume were obtained from US images via Amira software. A parallel cross-sectional cohort of mice was killed at each US time point, and knee joints were subjected to histological analysis to obtain synovial soft-tissue area and OARSI scores. The correlation between US joint space volume and histological synovial soft-tissue area or OARSI score was assessed via linear regression analysis. US images indicated increased joint space volume in PTOA joints over time, which was associated with synovial inflammation and cartilage damage by histology. These changes started from 2 weeks post-surgery and gradually became more severe. No change was detected in sham joints. Increased joint space volume was significantly correlated with increased synovial soft-tissue area and the OARSI score(P o 0.001). However, instances of alveolar remodeling in reptiles are rare. A remodeled alveolus in the maxilla of the Chinese theropod Sinosaurus (Lower Jurassic Lower Lufeng Formation) is the first confirmed example of such dental pathology in a dinosaur. Given the known relationship between feeding behavior and tooth damage in theropods (teeth with spalled enamel, tooth crowns embedded in bone) and the absence of dentary, maxillary, and premaxillary osteomyelitis, traumatic loss of a tooth is most likely the cause of alveolar remodeling.
