| 2 | Palliative chemotherapy for gastroesophageal cancer in old and very old patients: A retrospective cohort study at the National Center for Tumor Diseases, Heidelberg显示文摘AIM:To investigate the outcome of palliative chemotherapy in old patients with gastroesophageal cancer at the National Center for Tumor Diseases,Heidelberg.METHODS:Using a prospectively generated database,we retrospectively analyzed 55 patients≥70years under palliative chemotherapy for advanced gastroesophageal cancer at the outpatient clinic of the National Center for Tumor Diseases Heidelberg,Germany between January 2006 and December2013.Further requirements for inclusion were(1)histologically proven diagnosis of gastroesophageal cancer;(2)advanced(metastatic or inoperable)disease;and(3)no history of radiation or radiochemotherapy.The clinical information included Eastern Cooperative Oncology Group performance status(ECOG PS),presence and site of metastases at diagnosis,date of previous surgery and perioperative chemotherapy,start and stop date of first-line treatment,toxicities and consecutive dosage reductions of first-line treatment,response to first-line therapy,date of progression,usage of second-line therapies and date and cause of death.Survival times[progression-free survival(PFS),overall survival(OS)and residual survival(RS)]were calculated.Toxicity and safety were examined.Prognostic factors including ECOG PS,age and previousperioperative treatment were analyzed.RESULTS:Median age of our cohort was 76 years.86%of patients received a combination of two cytotoxic drugs.76 percent of patients had an oxaliplatin-based first-line therapy with the oxaliplatin and 5-fluorouracil regimen being the predominantely chosen regimen(69%).Drug modifications due to toxicity were necessary in 56%of patients,and 11%of patients stopped treatment due to toxicities.Survival times of our cohort are in good accordance with the major phaseⅢtrials that included mostly younger patients:PFS and OS were 5.8 and 9.5 mo,respectively.Survival differed significantly between patient groups with low(≤1)and high(≥2)ECOG PS(12.7 mo vs 3.8 mo,P<0.001).Very old patients(≥75 years)did not show a worse outcome in terms of survival.Patients receiving secondline treatment(51%)had a significantly longer RS than patients with best supportive care(6.8 vs 1.4 mo,P=0.001).Initial ECOG PS was a strong prognostic factor for PFS,OS and RS.CONCLUSION:Old patients with non-curable gastroesophageal cancer should be offered chemotherapy,and ECOG PS is a tool for balancing benefit and harm upfront.Second-line treatment is reasonable. | Anne Katrin Berger Stefanie Zschaebitz Christine Komander Dirk Jger Georg Martin Haag | 2015 | World Journal of Gastroenterology2015,21,16: | 6 |
| 3 | Epigenetics and pancreatic cancer:Pathophysiology and novel treatment aspects显示文摘An improvement in pancreatic cancer treatment represents an urgent medical goal.Late diagnosis and high intrinsic resistance to conventional chemotherapy has led to a dismal overall prognosis that has remained unchanged during the past decades.Increasing knowledge about the molecular pathogenesis of the disease has shown that genetic alterations,such as mutations of K-ras,and especially epigenetic dysregulation of tumor-associated genes,such as silencing of the tumor suppressor p16ink4a,are hallmarks of pancreatic cancer.Here,we describe genes that are commonly affected by epigenetic dysregulation in pancreatic cancer via DNA methylation,histone acetylation or miRNA(microRNA)expression,and review the implications on pancreatic cancer biology such as epithelial-mesenchymal transition,morphological pattern formation,or cancer stem cell regulation during carcinogenesis from PanIN(pancreatic intraepithelial lesions)to invasive cancer and resistance development.Epigenetic drugs,such as DNA methyltransferases or histone deactylase inhibitors,have shown promising preclinical results in pancreatic cancer and are currently in early phases of clinical development.Combinations of epigenetic drugs with established cytotoxic drugs or targeted therapies are promising approaches to improve the poor response and survival rate of pancreatic cancer patients. | Daniel Neureiter Tarkan Jger Matthias Ocker Tobias Kiesslich | 2014 | World Journal of Gastroenterology2014,20,24: | 6 |
| 4 | Targeting cell death signaling in colorectal cancer:Current strategies and future perspectives显示文摘The evasion from controlled cell death induction has been considered as one of the hallmarks of cancer cells.Defects in cell death signaling are a fundamental phenomenon in colorectal cancer.Nearly any non-invasive cancer treatment finally aims to induce cell death.However,apoptosis resistance is the major cause for insufficient therapeutic success and disease relapse in gastrointestinal oncology.Various compounds have been developed and evaluated with the aim to meet with this obstacle by triggering cell death in cancer cells.The aim of this review is to illustrate current approaches and future directions in targeting cell death signaling in colorectal cancer.The complex signaling network of apoptosis will be demonstrated and the'druggability'of targets will be identified.In detail,proteins regulating mitochondrial cell death in colorectal cancer,such as Bcl-2 and survivin,will be discussed with respect to potential therapeutic exploitation.Death receptor signaling and targeting in colorectal cancer will be outlined.Encouraging clinical trials including cell death based targeted therapies for colorectal cancer are under way and will be demonstrated.Our conceptual understanding of cell death in cancer is rapidly emerging and new types of controlled cellular death have been identified.To meet this progress in cell death research,the implication of autophagy and necroptosis for colorectal carcinogenesis and therapeutic approaches will also be depicted.The main focus of this topic highlight will be on the revelation of the complex cell death concepts in colorectal cancer and the bridging from basic research to clinical use. | Bruno Christian Koehler Dirk Jger Henning Schulze-Bergkamen | 2014 | World Journal of Gastroenterology2014,20,8: | 4 |

| 11 | Linking molecular deadwood-inhabiting fungal diversity and community dynamics to ecosystem functions and processes in Central European forests显示文摘Fungi play vital roles in the decomposition of deadwood due to their secretion of various enzymes that break down plant cell-wall complexes.The compositions of woodinhabiting fungal(WIF)communities change over the course of the decomposition process as the remaining mass of wood decreases and both abiotic and biotic conditions of the wood significantly change.It is currently not resolved which substrate-related factors govern these changes in WIF communities and whether such changes influence the deadwood decomposition rate.Here we report a study on fungal richness and community structure in deadwood of Norway spruce and European beech in temperate forest ecosystems using 454 pyrosequencing.Our aims were to disentangle the factors that correspond to WIF community composition and to investigate the links between fungal richness,taxonomically-resolved fungal identity,and microbial-mediated ecosystem functions and processes by analyzing physico-chemical wood properties,lignin-modifying enzyme activities and wood decomposition rates.Unlike fungal richness,we found significant differences in community structure between deadwood of different tree species.The composition of WIF communities was related to the physico-chemical properties of the deadwood substrates.Decomposition rates and the activities of ligninmodifying enzymes were controlled by the succession of the fungal communities and competition scenarios rather than fungal OTU richness.Our results provide further insights into links between fungal community structure and microbialmediated ecosystem functions and processes. | Björn Hoppe Witoon Purahong Tesfaye Wubet Tiemo Kahl Jürgen Bauhus Tobias Arnstadt Martin Hofrichter François Buscot Dirk Krüger | 2016 | Fungal Diversity2016,,2: | 2 |
