| 2 | Low gradient ascites: A seven-year course review显示文摘AIM: To study the patients with low gradient ascites in hospitals of Guilan Province (northern Iran).METHODS: Patients admitted in hospitals of Guilan Province with low gradient ascites from 1993 to 2000 were enrolled in the study. Serum and ascitic fluid albumin levels were determined by biochemical reactions. The serum-ascitic albumin gradient (SAAG) less than 1.1 g/dL was considered low. Statistical analysis was performed with SPSS 9.0software and P<0.05 was considered statistically significant.RESULTS: Of the 148 patients enrolled in the study, 72(48.6%) were males and 76 (51.4%) were females with a mean age of 59.03±13.54 years. Tuberculous peritonitis was the most frequent cause of low gradient ascites in 68 (45.9%). Other most frequent causes were cancer in 62 (41.9%), nephrotic syndrome in 9 (6%), pancreatitis in 6 (4%). Peritoneal cancer was found in 22 (35%),ovarian and gastric cancers were found in 14 (22.5%)and 12 (19.3%), respectively. All of which were the causes of ascites. The mean SAAG was 0.68±0.19 g/dL. The mean serum and ascitic fluid albumin concentrations were higher in tuberculous patients (P<0.006), but lactate dehydrogenase (LDH) level was higher in cancer patients (P<0.0001). In peritoneal tuberculosis, mean ascitic glucose concentration was significantly lower than other patients (P<0.0001).CONCLUSION: Tuberculosis should be considered in all patients with low gradient ascites especially in developing countries (like Iran), as the first cause of ascites. | 4 | Hepatitis B and C virus-induced hepatitis: Apoptosis, autophagy, and unfolded protein response显示文摘AIM: To investigate the co-incidence of apoptosis, autophagy, and unfolded protein response(UPR) in hepatitis B(HBV) and C(HCV) infected hepatocytes.METHODS: We performed immunofluorescence confocal microscopy on 10 liver biopsies from HBV and HCV patients and tissue microarrays of HBV positive liver samples. We used specific antibodies for LC3β, cleaved caspase-3, BIP(GRP78), and XBP1 to detect autophagy, apoptosis and UPR, respectively. AntiHCV NS3 and anti-HBs antibodies were also used to confirm infection. We performed triple blind counting of events to determine the co-incidence of autophagy(LC3β punctuate), apoptosis(cleaved caspase-3), and unfolded protein response(GRP78) with HBV and HCV infection in hepatocytes. All statistical analyses were performed using SPSS software for Windows(Version 16 SPSS Inc, Chicago, IL, United States). P-values < 0.05 were considered statistically significant. Statistical analyses were performed with Mann-Whitney test to compare incidence rates for autophagy, apoptosis, and UPR in HBV- and HCV-infected cells and adjacent noninfected cells.RESULTS: Our results showed that infection of hepatocytes with either HBV and HCV induces significant increase(P < 0.001) in apoptosis(cleavage of caspase-3), autophagy(LC3β punctate), and UPR(increase in GRP78 expression) in the HCV- and HBVinfected cells, as compared to non-infected cells of the same biopsy sections. Our tissue microarray immunohistochemical expression analysis of LC3β in HBV^(Neg) and HBV^(Pos) revealed that majority of HBVinfected hepatocytes display strong positive stainingfor LC3β. Interestingly, although XBP splicing in HBVinfected cells was significantly higher(P < 0.05), our analyses show a slight increase of XBP splicing was in HCV-infected cells(P > 0.05). Furthermore, our evaluation of patients with HBV and HCV infection based on stage and grade of the liver diseases revealed no correlation between these pathological findings and induction of apoptosis, autophagy, and UPR.CONCLUSION: The results of this study indicate that HCV and HBV infection activates apoptosis, autophagy and UPR, but slightly differently by each virus. Using Mashhad city(Iran second populous city) traffic accident records as case study, this paper applied the combination of geo-information technology and spatial-statistical analysis to bring out the influence of spatial factors in their formation. The aim of the study is to examine 4 clustering analyses to have a better understanding of traffic accidents patterns in complex urban network. In order to deploy the clustering technique in urban roads, 9331 point features for inner city traffic accidents during 12 months have been registered according to their x and y location in geographic information system(GIS). The mentioned areas were analyzed by kernel density estimation(KDE) using ARCMAP and two other analyses using SANET 4th edition software so that the results of network analysis can be compared with traditional KDE method. In addition, this research introduces five classifications for determining the eventfulness of the under study area based on standard deviation and to make priority in creating security in the area.The nearest neighbor and K-function output analysis consist of four curves and regarding the fact that for all fatal, injury and property damage only crashes, the observed value curve is above the 5% confidence interval. Accidents in the study region are more clustered than expected by random chance. | 15 | Efficacy of saccharomyces boulardii with antibiotics in acute amoebiasis显示文摘AIM: To compare the efficacy of antibiotics therapy alone with antibiotics and saccharomyces boulardii in treatment of acute amebiasis.METHODS: In a double blind, random clinical trial on patients with acute intestinal amoebiasis, 57 adult patients with acute amoebiasis, diagnosed with clinical manifestations (acute mucous bloody diarrhea) and amebic trophozoites engulfing RBCs found in stool were enrolled in the study.Regimen 1 induded metronidazole (750 mg Tid) and iodoquinol (630 mg Tid) for 10 days. Regimen 2 contained capsules of lyophilized saccharomyces boulardii (250 mg Tid) orally in addition to regimen 1. Patients were re-examined at two and four weeks after the treatment, and stool examination was performed at the end of week 4. Student′s t-test, χ2and McNemar′s tests were used for statistical analysis.RESULTS: Three patients refused to participate. The other 54 patients were randomized to receive either regimen 1 or regimen 2 (Groups 1 and 2 respectively, each with 27patients). The two groups were similar regarding their age,sex and clinical manifestations. In Group 1, diarrhea lasted 48.0±18.5 hours and in Group 2,12.0±3.7 hours (P<0.0001).In Group 1, the durations of fever and abdominal pain were 24.0±8.8 and 24.0±7.3 hours and in Group 2 they were 12.0±5.3 and 12.0±3.2 hours, respectively (P<0.001).Duration of headache was similar in both groups. | 16 | Essential amino acid ratios and mTOR affect lipogenic gene networks and miRNA expression in bovine mammary epithelial cells显示文摘Background: The objective of this study was to study how changing the ratio of Lys to Thr, Lys to His, and Lys to Val affects the expression of lipogenic genes and micro RNA(mi RNA) in bovine mammary epithelial cells.Results: Triplicate cultures with the respective 'optimal' amino acid(AA) ratio(OPAA = Lys:Met 2.9:1; Thr:Phe 1.05:1;Lys:Thr 1.8:1; Lys:His 2.38:1; Lys:Val 1.23:1) plus rapamycin(OPAARMC; positive control), OPAA, Lys:Thr 2.1:1(LT2.1),Lys:Thr 1.3:1(LT1.3), Lys:His 3.05:1(LH3.0), or Lys:Val 1.62:1(LV1.6) were incubated in lactogenic medium for 12 h.The expression of 15 lipogenic genes and 7 mi RNA were evaluated. Responses to LT2.1, LT1.3, LH3.0, and LV1.6relative to the control(OPAARMC) included up-regulated expression of ACSS2, FABP3, ACACA, FASN, SCD, LPIN1,INSIG1, SREBF1, PPARD, and NR1H3(commonly known as LXR-α). Furthermore, LV1.6 up-regulated expression of ACSL1, DGAT1, and RXRA and down-regulated PPARG expression. Although no effect of OPAA on expression of PPARG was observed, compared with the control, OPAA up-regulated expression of the PPAR targets ACSS2, FABP3,ACACA, FASN, SCD, LPIN1, INSIG1, and SREBF1. Compared with the control, the expression of the anti-lipogenic MIR27 AB was down-regulated by OPAA, LT2.1, LT1.3 and LH3.0.
