1	WONCA研究论文摘要汇编(一百二十七)——全科医疗中腰痛分层治疗(IMPaCT Back)的效果:前瞻性-群组序列对照研究显示文摘目的此研究将明确在全科医疗中腰痛风险-层化治疗对医生的临床行为、患者的治疗效果及成本的作用。方法腰痛分层治疗研究(通过靶向疗法以改善患者的治疗)按照前瞻性-群组序列对照研究方法,将接受全科医疗的患者分为两组,即干预前期组(常规治疗6个月)与干预后期组(层化治疗12个月),此研究有64名全科医生参与并结合了理疗服务,纳入了1 647例患有腰痛的成人。利用风险层化工具将患者按功能障碍分为:低、中、高风险组,并提供风险-匹配治疗。使用罗兰-摩理斯腰背痛生活障碍问卷(Roland-Morris Disability Questionnaire)对治疗6个月患者的功能改善进行初步评估。研究过程将关注医生根据理疗、诊断性检查、用药处方和疾病证明而做出的与风险相应的行为改变。成本效应分析评估了所增加的质量调整寿命年数及腰部-相关医疗的成本。并分析治疗意向。结果 922例患者(干预前期368例,干预后期554例)符合基线标准。在治疗6个月组的随访中,罗兰-摩理斯腰背痛生活障碍问卷(Roland-Morris Disability Questionnaire)分数均差0.7〔95%CI(0.1,1.4)〕,提示与常规治疗相比,层化治疗均差小但获益显著;高危组均差大,为2.3〔95%CI(0.8,3.9)〕,是临床重要差异。而在干预后期组,患者不能工作的平均时间相较于干预前期组要少50%(4 vs 8 days,P=0.03);被给予疾病证明的患者比例也要低30%;医疗成本也有所减少。结论全科医疗中腰痛分层治疗方案的实施,在未增加医疗成本的前提下,显著改善了患者功能障碍的治疗效果,并使其不能工作的时间减半。建议推广应用。	Foster NE Mullis R Hill JC 周淑新	2014	中国全科医学2014，17，19：	2
