| 2 | Autoimmune hepatitis:Appraisal of current treatment guidelines显示文摘Autoimmune hepatitis affects patients of all ages and gender, across all geographic regions. Although still rare, its incidence and prevalence are increasing. Genetic predisposition conveyed by human leucocyte antigen is a strong risk factor for the disease and may be responsible in part for the wide variation in presentation in different geographic regions. Our understanding of the underlying pathogenic mechanisms is evolving and may lead to development of more targeted immunotherapies. Diagnosis is based on elevated levels of serum aminotransferases, gamma globulins, autoantibodies and characteristic findings on histology. Exclusion of other causes of chronic hepatitis is important.Although undiagnosed disease is associated with poor outcomes, it is readily treatable with timely immunosuppressive therapy in the majority of patients.International guidelines are available to guide management but there exists a disparity in the standard treatment regimens. | 3 | Variability in permeability and integrity of cell membrane and depletion of food reserves in neem(Azadirachta indica) seeds from trees of different age classes显示文摘We quantified cell membrane permeability(electrical conductivityEC,water soluble sugar-WSS,and amino acids-AA)and integrity(phospholipids,α-tocopherol and lipid peroxidation)along with food reserve deterioration(total proteins,total sugar,and total starch)of neem seeds collected from various mother tree age classes and stored for 65days in airtight plastic containers at ambient room temperature(35±5°C).Results show that the activities were higher in fresh seeds(EC267.56-2950.01μS/g,WSS 19.96-19.48 mg/g and AA 5.40-5.35 mg/g)and declined with increasing duration of storage period(EC153.37-195.17μS/g,WSS 3.13-4.17 mg/g and AA 4.29-4.49 mg/g after35 days and EC 144.02-161.56μS/g,WSS 2.06-2.40 mg/g and AA3.98-4.27 mg/g after 65 days of storage).Phospholipids andα-tocopherol were higher in fresh seed(0.073-0.093 OD at 710 nm and0.080-0.105 OD,respectively)and declined as storage duration increased(0.033-0.042 OD at 710 nm and 00.0010-0.0020 OD,respectively).Dead seeds showed reduced amounts of phospholipids and minimum activity ofα-tocopherol(antioxidants).The level of MDA was lower in fresh seeds(0.0066-0.0087 OD at 600-535 nm)and increased as storage duration increased(0.0248-0.0268 OD after 65 days of storage).The higher amount of MDA indicated that seeds died due to rancidity of the oil inside the seed.Neem seed cake was assessed for deterioration of food reserves(total proteins,total sugar,and total starch),concentrations of which were higher in fresh seed and declined as storage duration increased.Germination was higher in fresh seeds and after 65days,no germination was received perhaps due to deterioration of biochemicals in seeds.Patterns of seed deterioration were similar across all seed lots. | evendra Kumar Dhruv Kumar Mishra | 2014 | Journal of Forestry Research2014，25，1： | 4 |

| 5 | Healthcare utilization and costs associated with gastroparesis显示文摘AIM To use a national database of United States hospitals to evaluate the incidence and costs of hospital admissions associated with gastroparesis.METHODS We analyzed the National Inpatient Sample Database(NIS)for all patients in whom gastroparesis(ICD-9 code:536.3)was the principal discharge diagnosis during the period,1997-2013.The NIS is the largest publicly available all-payer inpatient care database in the United States.It contains data from approximately eight million hospital stays each year.The statistical significance of the difference in the number of hospital discharges,length of stay and hospital costs over the study period was determined by regression analysis.RESULTS In 1997,there were 3978 admissions with a principal discharge diagnosis of gastroparesis as compared to16460 in 2013(P<0.01).The mean length of stay for gastroparesis decreased by 20%between 1997 and 2013from 6.4 d to 5.1 d(P<0.001).However,during this period the mean hospital charges increased significantly by 159%from$13350(after inflation adjustment)per patient in 1997 to$34585 per patient in 2013(P<0.001).The aggregate charges(i.e.,'national bill') for gastroparesis increased exponentially by 1026%from$50456642±4662620 in 1997 to$568417666±22374060 in 2013(P<0.001).The percentage of national bill for gastroparesis discharges(national bill for gastroparesis/total national bill)has also increased over the last 16 years(0.0013%in 1997 vs 0.004%in2013).During the study period,women had a higher frequency of gastroparesis discharges when compared to men(1.39/10000 vs 0.9/10000 in 1997 and 5.8/10000vs 3/10000 in 2013).There was a 6-fold increase in the discharge diagnosis of gastroparesis amongst type 1 DM and 3.7-fold increase amongst type 2 DM patients over the study period(P<0.001).CONCLUSION The number of inpatient admissions for gastroparesis and associated costs have increased significantly over the last 16 years.Inpatient costs associated with gastroparesis contribute significantly to the national healthcare bill.Further research on cost-effective evaluation and management of gastroparesis is required. | Vaibhav Wadhwa Dhruv Mehta Yash Jobanputra Rocio Lopez Prashanthi N Thota Madhusudhan R Sanaka | 2017 | World Journal of Gastroenterology2017，23，24： | 3 |
