1Management of distal humeral coronal shear fractures显示文摘Coronal shear fractures of the distal humerus are rare,complex fractures that can be technically challenging to manage. They usually result from a low-energy fall and direct compression of the distal humerus by the radial head in a hyper-extended or semi-flexed elbow or from spontaneous reduction of a posterolateral subluxation or dislocation. Due to the small number of soft tissue attachments at this site, almost all of these fractures are displaced. The incidence of distal humeral coronal shear fractures is higher among women because of the higher rate of osteoporosis in women and the difference in carrying angle between men and women. Distal humeral coronal shear fractures may occur in isolation, may be part of a complex elbow injury, or may be associated with injuries proximal or distal to the elbow. An associated lateral collateral ligament injury is seen in up to 40% and an associated radial head fracture is seen in up to 30% of these fractures. Given the complex nature of distal humeral coronal shear fractures, there is preference for operative management. Operative fixation leads to stable anatomic reduction, restores articular congruity, and allows initiation of early range-of-motion movements in the majority of cases. Several surgical exposure and fixation techniques are available to reconstruct the articular surface fol owing distal humeral coronal shear fractures. The lateral extensile approach and fixation with countersunk headless compression screws placed in an anterior-to-posterior fashion are commonly used. We have found a two-incision approach(direct anterior and lateral) that results in less soft tissue dissection and better outcomes than the lateral extensile approach in our experience. 2Do medical students studying in the United Kingdom have an adequate factual knowledge of basic life support?显示文摘BACKGROUND: Healthcare professionals have a duty to maintain basic life support(BLS) skills. This study aims to evaluate medical students' factual knowledge of BLS and the training they receive.METHODS: A cross-sectional, closed-response questionnaire was distributed to the fi rst-and fourth-year students studying at institutions in the United Kingdom. The paper questionnaire sought to quantify respondent's previous BLS training, factual knowledge of the BLS algorithm using five multiple choice questions(MCQs), and valuate their desire for further BLS training. Students received 1 point for each correctly identifi ed answer to the 5 MCQ's.RESULTS: A total of 3,732 complete responses were received from 21 medical schools. Eighty percent(n=2,999) of students completed a BLS course as part of their undergraduate medical studies. There was a signifi cant difference(P<0.001) in the percentage of the fourth-year students selecting the correct answer in all the MCQ's compared to the fi rst-year students except in identifyingthe correct depth of compressions required during CPR(P=0.095). Overall 10.3%(95% CI 9.9% to 10.7%) of respondents correctly identified the answer to 5 MCQ's on BLS: 9% of the first-year students(n=194) and 12% of the fourth-year students(n=190). On an institutional level the proportion of students answering all MCQ's correctly ranged from 2% to 54% at different universities. Eighty-one percent of students(n=3,031) wished for more BLS training in their curriculum.CONCLUSION: Factual knowledge of BLS is poor among medical students in the UK. 4Molten Salts and Applications Ⅰ: Molten Salt History, Types, Thermodynamic and Physical Properties, and Cost显示文摘Excess energy from solar, nuclear or coal power stations can be stored in molten salts (MS) in the 565 °C range. At elevated temperature, large containers can be used to store energy for up to a week and generate eight hours or more, depending on the container size, of electricity to be used at night or during peak demand hours. A stable diurnal energy supply is made available by energy storage and the fluctuation experienced at thermal solar power stations caused by weather conditions is reduced. Supported by Office of Naval Research (ONR), this paper presents a survey of molten salt properties used in solar power storage, as well as the history of molten salt usage for energy storage and production. The history of molten salt usage includes past, current, and future developments involving molten salt usage for nuclear and solar energy storage and production. Density, melting temperature, viscosity, electric conductivity, surface tension, and heat capacity of various molten salts are discussed. Since costs are important factor in selecting a molten salt compound, salts costs are presented. Large containers can be used to store energy at excess temperatures in order to generate eight hours or more of electricity, depending on the container size, to be used during peak demand hours or at night for up to a week. Energy storage allows for a stable diurnal energy supply and can reduce the fluctuation due to weather conditions experienced at thermal solar power stations. Supported by Office of Naval Research (ONR), this paper discusses the design considerations for molten salt storage tanks. An optimal molten salt storage tank design layout is presented, as well as alternative designs for the storage tanks. 12Molten Salts and Applications Ⅲ: Worldwide Molten Salt Technology Developments in Energy Production and Storage显示文摘Supported by Office of Naval Research (ONR), this paper presents a survey of molten salt technology used in solar power storage. Excess energy from solar power stations and other baseline power production methods can be stored in molten salts (MS) in the 565°C range, therefore allowing the use of large containers to store energy for up to a week and generate eight hours of electricity or more to be used during peak demand hours, at night, or adverse weather conditions, depending on the container size. The technology could also be used to conserve the spin off energy in the grids from nuclear or coal power production. Real life examples of concentrating solar power (CSP) plants, both domestically and worldwide, are presented with details about the type of solar collection, capacity, and energy production. Commercial solar power stations have been constructed in the United States and overseas, particularly in Spain, with molten salt being considered for use in these facilities. 14Ants as architects of living,self-healing structures显示文摘We present experiments and theory on the ability of fire ants to both flow like a fluid and aggregate like a solid.During floods,fire ants link their bodies together to weave hydrophobic rafts in the manner of Gore-Tex.Upon returning to land,they construct bivouacs that are shaped such that each ant supports an equal load throughout construction.Particular attention is paid to rationalizing construction rates based upon individual behaviors and constraints of the ants.Many ant species form functional self-assemblages by linking themselves together.When viewed collectively,a fire ant self-assemblage exhibits bulk material characteristics,such as the ability to flow or support loads.Such assemblages can be constructed and disassembled quickly.IfDavid Hu Nathan Mlot Craig Tovey Paul Foster 2013医用生物力学2013,28,S1:0
