1Risk factors for cervical lymph node metastasis in papillary thyroid microcarcinoma: a study of 1,587 patients显示文摘Objective: The purposes of this study were to identify risk factors for cervical lymph node metastasis and to examine the association between BRAF^(V600E) status and clinical features in papillary thyroid microcarcinoma(PTMC).Methods: A total of 1,587 patients with PTMC, treated in Tianjin Medical University Cancer Institute and Hospital from January2011 to March 2013, underwent retrospective analysis. We reviewed and analyzed factors including clinical results, pathology records, ultrasound results, and BRAF^(V600E) status.Results: Multivariate logistic regression analyses demonstrated that gender(male) [odds ratio(OR) = 1.845, P = 0.000], age(< 45 years)(OR = 1.606, P = 0.000), tumor size(> 6 mm)(OR = 2.137, P = 0.000), bilateralism(OR = 2.011, P = 0.000) and extrathyroidal extension(OR = 1.555, P = 0.001) served as independent predictors of central lymph node metastasis(CLNM).Moreover, CLNM(OR = 29.354, P = 0.000) served as an independent predictor of lateral lymph node metastasis(LLNM). Among patients with a solitary primary tumor, those with tumor location in the lower third of the thyroid lobe or the isthmus were more likely to experience CLNM(P < 0.05). Univariate analyses indicated that CLNM, LLNM, extrathyroidal extension, and multifocality were not significantly associated with BRAF^(V600E) mutation.Conclusions: The present study suggested that prophylactic neck dissection of the central compartment should be considered in patients with PTMC, particularly in men with tumor size greater than 6 mm, age less than 45 years, extrathyroidal extension, and tumor bilaterality. Among patients with PTMC, BRAF^(V600E) mutation is not significantly associated with prognostic factors. 2Serum N-glycan markers for diagnosing liver fibrosis induced by hepatitis B virus显示文摘BACKGROUND Hepatitis B virus (HBV) infection is the primary cause of hepatitis with chronic HBV infection,which may develop into liver fibrosis,cirrhosis and hepatocellular carcinoma.Detection of early-stage fibrosis related to HBV infection is of great clinical significance to block the progression of liver lesion.Direct liver biopsy is regarded as the gold standard to detect and assess fibrosis;however,this method is invasive and prone to clinical sampling error.In order to address these issues,we attempted to find more convenient and effective serum markers for detecting HBV-induced early-stage liver fibrosis.AIM To investigate serum N-glycan profiling related to HBV-induced liver fibrosis and verify multiparameter diagnostic models related to serum N-glycan changes.METHODS N-glycan profiles from the sera of 432 HBV-infected patients with liver fibrosis were analyzed.Significant changed N-glycan levels (peaks)(P <0.05) in differentfibrosis stages were selected in the modeling group,and multiparameter diagnostic models were established based on changed N-glycan levels by logistic regression analysis.The receiver operating characteristic (ROC) curve analysis was performed to evaluate diagnostic efficacy of N-glycans models.These models were then compared with the aspartate aminotransferase to platelet ratio index (APRI),fibrosis index based on the four factors (FIB-4),glutamyltranspeptidase platelet albumin index (S index),GlycoCirrho-test,and GlycoFibro-test.Furthermore,we combined multiparameter diagnostic models with alanine aminotransferase (ALT) and platelet (PLT) tests and compared their diagnostic power.In addition,the diagnostic accuracy of N-glycan models was also verified in the validation group of patients.RESULTS Multiparameter diagnostic models constructed based on N-glycan peak 1,3,4and 8 could distinguish between different stages of liver fibrosis.The area under ROC curves (AUROCs) of Model A and Model B were 0.890 and 0.752,respectively differentiating fibrosis F0-F1 from F2-F4,and F0-F2 from F3-F4,and surpassing other serum panels.However,AUROC (0.747) in Model C used for the diagnosis of F4 from F0-F3 was lower than AUROC (0.795) in FIB-4.In combination with ALT and PLT,the multiparameter models showed better diagnostic power (AUROC=0.912,0.829,0.885,respectively) when compared with other models.In the validation group,the AUROCs of the three combined models (0.929,0.858,and 0.867,respectively) were still satisfactory.We also applied the combined models to distinguish adjacent fibrosis stages of 432patients (F0-F1/F2/F3/F4),and the AUROCs were 0.917,0.720 and 0.785.CONCLUSION Multiparameter models based on serum N-glycans are effective supplementary markers to distinguish between adjacent fibrosis stages of patients caused by HBV,especially in combination with ALT and PLT.Xi Cao Qing-Hua Shang Xiao-Ling Chi Wei Zhang Huan-Ming Xiao Mi-Mi Sun Gang Chen Yong An Chun-Lei Lv Lin Wang Yue-Min Nan Cui-Ying Chen Zong-Nan Tan Xue-En Liu Hui Zhuang 2020World Journal of Gastroenterology2020,26,10:12

4A spatial-explicit dynamic vegetation model that couples carbon,water,and nitrogen processes for arid and semiarid ecosystems显示文摘Arid and semiarid ecosystems,or dryland,are important to global biogeochemical cycles.Dryland's community structure and vegetation dynamics as well as biogeochemical cycles are sensitive to changes in climate and atmospheric composition.Vegetation dynamic models has been applied in global change studies,but the complex interactions among the carbon(C),water,and nitrogen(N) cycles have not been adequately addressed in the current models.In this study,a process-based vegetation dynamic model was developed to study the responses of dryland ecosystems to environmental changes,emphasizing on the interactions among the C,water,and N processes.To address the interactions between the C and water processes,it not only considers the effects of annual precipitation on vegetation distribution and soil moisture on organic matter(SOM) decomposition,but also explicitly models root competition for water and the water compensation processes.To address the interactions between C and N processes,it models the soil inorganic mater processes,such as N mineralization/immobilization,denitrification/nitrification,and N leaching,as well as the root competition for soil N.The model was parameterized for major plant functional types and evaluated against field observations.Chi ZHANG ChaoFan LI Xi CHEN GePing LUO LongHui LI XiaoYu LI Yan YAN Hua SHAO 2013Journal of Arid Land2013,5,1:8
5Relationship between settlements and topographical factors:An example from Sichuan Province,China显示文摘Terrain can influence the spatial distribution of settlements. Studies on the terrain characteristics of settlements can help to understand the effects of the environment on human activities. This paper provides a quantitative analysis of the relationship between settlements and topographical factors. A statistically significant sample of residential locations and ASTER GDEM V2 were used to investigate terrain traits and settlements distributions. We selected eight topographical factors and introduced a practical concept, distributive entropy, into assessing the aggregation extent of the settlements' spatial distribution. The study showed that topography varies within the study area, and distributive entropy indicates that settlements have distinctive distribution tendency in statistic approach. According to the results of this study, mountain inhabitants prefer to settle in valleys. Additionally, with distributive entropy, residential suitability was divided to three levels: suitable, normal, and unsuited. 9Transumbilical enterostomy for Hirschsprung’s disease with a two-stage laparoscopy-assisted pull-through procedure显示文摘BACKGROUND A one-stage laparoscopic operation has recently been considered a favorable option for the management of patients with Hirschsprung's disease(HD)due to its superior cosmetic results.One-stage transanal endorectal pull-through for the treatment of rectosigmoid HD has been widely used in newborns without complications.However,enterostomy is required in some HD cases for enterocolitis and dilated colon.Our transumbilical enterostomy(TUE)and twostage laparoscopy-assisted anorectoplasty were effective and achieved a similar cosmetic effect to one-stage laparoscopy on the abdominal wall in patients with anorectal malformation,but the effect in patients with HD is unclear.AIM To evaluate the safety,efficacy and cosmetic results of TUE in two-stage laparoscopy-assisted pull-through for HD.METHODS From June 2013 to June 2018,53 patients(40 boys,13 girls;mean age at enterostomy:5.5±2.2 mo)who underwent enterostomy and two-stage laparoscopy-assisted pull-through for HD with stoma closure were reviewed at our institution.Two enterostomy approaches were used:TUE in 24 patients,and conventional abdominal enterostomy(CAE)in 29 patients.Eleven patients with rectosigmoid HD had severe preoperative enterocolitis or a dilated colon.26 patients had long-segment HD,and 16 patients had total colonic aganglionosis(TCA).The patients with left-sided HD underwent the two-stage laparoscopic Soave procedure,and the patients with right-sided HD and TCA underwent the laparoscopic Duhamel procedure.Demographics,enterostomy operative time,complications and cosmetic results were respectively evaluated.RESULTS There were no differences between the groups with respect to gender,age at enterostomy,weight and clinical type(P>0.05).No conversion to open technique was required.Two patients experienced episodes of stomal mucosal prolapse in the TUE group and 1 patient in the CAE group(8.33%vs 3.45%,P>0.05).No parastomal hernia was observed in either of the two groups.Wound infection at the stoma was seen in 1 case in the TUE group,and 2 cases in the CAE group(4.17%vs 6.90%,P>0.05).No obstruction was noted in any of the patients in the TUE group,whereas obstruction was found in 1 patient in the CAE group.Enterocolitis was observed in 3 and 5 patients in the TUE and CAE group,respectively(12.50%vs 17.24%,P>0.05).There was no significant difference between the TUE group and CAE group in terms of the incidence of soiling and constipation(P>0.05).The cosmetic result using the scar score in the TUE group was better than that in the CAE group(6.83±0.96 vs 13.32±1.57,P<0.05).CONCLUSION TUE is a safe and feasible method for the treatment of HD,and the staged enterostomy and two-stage laparoscopy-assisted pull-through achieved a similar cosmetic effect to the one-stage laparoscopic procedure.Pei-Pei Xu Xiao-Pan Chang Xi Zhang Shui-Qing Chi Guo-Qing Cao Shuai Li De-Hua Yang Xiang-Yang Li Shao-Tao Tang 2019World Journal of Gastroenterology2019,25,46:3
10Enhanced optical Kerr nonlinearity of MoS2 on silicon waveguides显示文摘A quasi-two-dimensional layer of MoS2 was placed on top of a silicon optical waveguide to form a MoS2–silicon hybrid structure. Chirped pulse self-phase modulation measurements were carried out to determine the optica Kerr nonlinearity of the structure. The observed increase in the spectral broadening of the optical pulses in the MoS2–silicon waveguide compared with the silicon waveguides indicated that the third-order nonlinear effect in MoS2 is about 2 orders of magnitude larger than that in silicon. The measurements show that MoS2 has an effective optical Kerr coefficient of about 1.1 × 10-16m2∕W. 12Solar cycle dependence of the seasonal variation of auroral hemispheric power显示文摘Although much has been done on the hemispheric asymmetry (or seasonal variations) of auroral hemispheric power (HP), the dependence of HP hemispheric asymmetry on solar cycle has not yet been studied. We have analyzed data during 1979-2010 and investigated the dependence of HP hemispheric asymmetry/seasonal variation for the whole solar cycle. Here we show that (1) the hemispheric asymmetry of HP is positively correlated to the value of solar F10.7 with some time delay; (2) it is closely related to the coupling function between the solar wind and magnetosphere; and (3) the winter hemisphere receives more auroral power than the summer hemisphere for Kp ~0 to 6. The statistic results can be partly understood in the framework of the ionospheric conductivity feedback model. 18High energy and high brightness laser compton backscattering gamma-ray source at IHEP显示文摘Based on the LINAC of BEPCII, a high-polarized, high bightness, energy-tunable, monoenergetic laser compton backscattering (LCS)gamma-ray source is under construction at IHEP. The gamma-ray energy range is from 1 MeV to 111 MeV. It is a powerful and hopeful researchplatform to reveal the underlying physics of the nuclear, the basic particles and the vacuum or to check the exist basic physical models, quantumelectrodynamic (QED) theories. In the platform, a 1.064 mm Nd:YAG laser system and a 10.6 mm CO_(2) laser system are employed. All the triggersignals to the laser system and the electron control system are from the only reference clock at the very beginning of the LINAC to make sure thetemporal synchronization. Two optical transition radiation (OTR) targets and two charged-couple devices (CCD) are used to monitor and to alignthe electron beam and the laser beam.
