| 1 | Exploring the metabolic syndrome: Nonalcoholic fatty pancreas disease显示文摘After the first description of fatty pancreas in 1933,the effects of pancreatic steatosis have been poorly investigated,compared with that of the liver. However,the interest of research is increasing. Fat accumulation,associated with obesity and the metabolic syndrome(Met S),has been defined as 'fatty infiltration' or 'nonalcoholic fatty pancreas disease'(NAFPD). The term 'fatty replacement' describes a distinct phenomenon characterized by death of acinar cells and replacement by adipose tissue. Risk factors for developing NAFPD include obesity,increasing age,male sex,hypertension,dyslipidemia,alcohol and hyperferritinemia. Increasing evidence support the role of pancreatic fat in the development of type 2 diabetes mellitus,Met S,atherosclerosis,severe acute pancreatitis and even pancreatic cancer. Evidence exists that fatty pancreas could be used as the initial indicator of 'ectopic fat deposition',which is a key element of nonalcoholic fatty liver disease and/or Met S. | 2 | Immune response in COVID-19:addressing a pharmacological challenge by targeting pathways triggered by SARS-CoV-2显示文摘To date,no vaccines or effective drugs have been approved to prevent or treat COVID-19 and the current standard care relies on supportive treatments.Therefore,based on the fast and global spread of the virus,urgent investigations are warranted in order to develop preventive and therapeutic drugs.In this regard,treatments addressing the immunopathology of SARS-CoV-2 infection have become a major focus.Notably,while a rapid and well-coordinated immune response represents the first line of defense against viral infection,excessive inflammatory innate response and impaired adaptive host immune defense may lead to tissue damage both at the site of virus entry and at systemic level.Several studies highlight relevant changes occurring both in innate and adaptive immune system in COVID-19 patients.In particular,the massive cytokine and chemokine release,the so-called“cytokine storm”,clearly reflects a widespread uncontrolled dysregulation of the host immune defense.Although the prospective of counteracting cytokine storm is compelling,a major limitation relies on the limited understanding of the immune signaling pathways triggered by SARS-CoV-2 infection.The identification of signaling pathways altered during viral infections may help to unravel the most relevant molecular cascades implicated in biological processes mediating viral infections and to unveil key molecular players that may be targeted.Thus,given the key role of the immune system in COVID-19,a deeper understanding of the mechanism behind the immune dysregulation might give us clues for the clinical management of the severe cases and for preventing the transition from mild to severe stages. | Michele Catanzaro Francesca Fagiani Marco Racchi Emanuela Corsini Stefano Govoni Cristina Lanni | 2020 | Signal Transduction and Targeted Therapy2020，5，1： | 11 |
| 3 | Diagnostic accuracy of enhanced liver fibrosis test to assess liver fibrosis in patients with chronic hepatitis C显示文摘BACKGROUND:The prognosis and clinical management of patients with chronic liver diseases are closely related to the severity of liver fibrosis.Liver biopsy is considered the gold standard for the staging of liver fibrosis.However,it is an invasive test sometimes related to complications.This study aimed to assess the diagnostic value of enhanced liver fibrosis(ELF) test to predict liver fibrosis in patients with chronic hepatitis C.METHODS:This study included 162 patients with liver disease and 67 healthy controls.Hyaluronic acid,tissue inhibitor of matrix metalloproteinase type 1,and amino-terminal propeptide type III procollagen were measured by enzymelinked immunosorbent assay with the ELF test ADVIA Centaur(Siemens Healthcare Diagnostics Inc.).Fibrosis stage was determined using the Metavir scoring system.RESULTS:In our study,for the diagnosis of significant fibrosis(Metavir F≥2) a cut-off value >7.72 provides a sensitivity of 93.0% and a specificity of 83.0%.The areas under the receiver operator characteristic curve,sensitivity,specificity,and positive and negative predictive values were 0.94,93.3%,81.0%,93.3%,and 81.0%,respectively(P<0.001).For the diagnosis of cirrhosis(Metavir F=4) a cut-off value >9.3 provides a sensitivity of 93.0% and a specificity of 86.0%.The areas under the receiver operator characteristic curve,sensitivity,specificity,and positive and negative predictive values were 0.94,79.1%,90.8%,75.6%,and 92.3%,respectively(P<0.001).CONCLUSIONS:The ELF test is a promising non-invasive method for assessing liver fibrosis in patients with chronic hepatitis C.It is effective in the diagnosis of both fibrosis and cirrhosis. | Roberto Catanzaro Michele Milazzo Silvia Arona Chiara Sapienza Dario Vasta Domenico Arcoria Francesco Marotta | 2013 | Hepatobiliary & Pancreatic Diseases International2013，12，5： | 3 |
| 4 | Mortality and morbidity in patients with atrial fibrillation and liver cirrhosis显示文摘BACKGROUND Atrial fibrillation(AF)is the most common cardiac arrhythmia encountered in clinical practice.However,the outcomes associated with AF in hospitalized patients with liver cirrhosis are unknown.AIM To determine the outcomes of hospitalized patients with liver cirrhosis and AF.METHODS In this study,we examined morbidity and mortality of patients with concomitant AF and liver cirrhosis from the National Inpatient Sample database,the largest publicly available inpatient healthcare database in the United States.RESULTS A total of 696937 patients with liver cirrhosis were included,45745 of whom had concomitant AF(6.6%).Liver cirrhosis patients with AF had higher rates of inhospital mortality(12.6%vs 10.3%,P<0.001),clinical stroke(1.6%vs 1.1%,P<0.001),and acute kidney injury(28.2%vs 25.1%,P<0.001),and less gastrointestinal bleeding(4.4%vs 5.1%,P<0.001)and blood transfusion(22.5%vs 23.8%,P<0.001)compared with those who did not have the arrhythmia.In addition,they had a longer length of stay(8±10 d vs 7±8 d,P<0.001)and higher hospitalization costs(20720±33210$vs 16272±24166$,P<0.001).CONCLUSION In subjects with liver cirrhosis,AF is associated with higher rates of inpatient mortality,stroke,and acute kidney injury compared to those who do not have the cardiac arrhythmia. | Yousef H Darrat Aiman Smer Claude-Samy Elayi Gustavo X Morales Fahad Alqahtani Mohamad Alkhouli John Catanzaro Jignesh Shah Mohsin Salih | 2020 | World Journal of Cardiology2020，12，7： | 2 |
