| 1 | Superior mesenteric artery first approach versus standard pancreaticoduodenectomy: a systematic review and meta-analysis显示文摘BACKGROUND: The superior mesenteric artery(SMA) first approach was proposed recently as a new modification of the standard pancreaticoduodenectomy. Increasing evidence showed that a periadventiceal dissection of the SMA with early transection of the inflow during pancreaticoduodenectomy associates better early perioperative results, and setup the scene for long-term oncological benefits. The objectives of the current study are to compare the operative results and long-term oncological outcomes of SMA first approach pancreaticoduodenectomy(SMA-PD) with standard pancreaticoduodenectomy(S-PD).DATA SOURCES: Electronic search of the PubM ed/MEDLINE, EMBASE, Web of Science and Cochrane Library was performed until July 2015. We considered randomized controlled trials(RCTs) and non-randomized comparative studies(NRCSs) comparing SMA-PD with S-PD to be eligible if they included patients with periampullary cancers.RESULTS: A total of one RCT and thirteen NRCSs met the inclusion criteria, involving 640 patients with SMA-PD and 514 patients with S-PD. The SMA-PD was associated with less intraoperative bleeding, less blood transfusions and higher rate of associated venous resections. The pancreatic fistula and delayed gastric emptying had a significantly lower rate in the SMA-PD group. There were no differences between the two approaches regarding overall complications, major complication rates and in-hospital mortality. There was no difference regarding R0 resection rate, and one-, two-or three-year over-all survival. The SMA-PD was associated with a lower local, hepatic and extrahepatic metastatic rate.CONCLUSIONS: The SMA-PD is associated with better perioperative outcomes, such as blood loss, transfusion requirements, pancreatic fistula, and delayed gastric emptying. | 4 | Self-stabilized chitosan and its complexes with carboxymethyl starch as excipients in drug delivery显示文摘This study focuses on the behavior of chitosan(CHI)and its polyelectrolyte complexes with carboxymethyl starch(CMS)used as monolithic matrices with acetaminophen as drug tracer.Two different chitosan grades were tested alone or associated in various ratios with CMS as excipients for tablets obtained by direct compression.The degree of deacetylation(DDA)of CHI,estimated from 1H NMR and FTIR data,was correlated with X-ray diffraction and scanning electron microscopy(SEM)to evaluate structural organization of the monolithic matrices.In vitro drug dissolution assays showed major differences in CHI kinetic profiles between tablets exposed to acidic medium for 2h(to mimick gastric passage)prior to dissolution in simulated intestinal fluid(SIF),and those administered directly to SIF.Prior exposure to acidic SGF conducted to longer dissolution profiles(release completed after 16 h)and preservation of tablet shape,whereas tablets directly incubated in SIF were rapidly disintegrated.The improved properties of chitosan matrices exposed to SGF may be related to an outer compact coating layer(visible in SEM).The effect of self-stabilization of chitosan in acidic medium was compared to that due to formation of polyelectrolyte complexes(PEC)in co-processed polymeric systems(CHI:CMS).The self-formed membrane following exposure to gastric acidity appears to help maintaining tablet integrity and allows higher drug loading,recommending CHI and its complexes with CMS as excipients for drug delivery. | Mihaela Leonida Pompilia Ispas-Szabo Mircea Alexandru Mateescu | 2018 | Bioactive Materials2018，3，3： | 4 |
| 5 | Using experimental data to reduce the single-building sigma of fragility curves:case study of the BRD tower in Bucharest,Romania显示文摘The lack of knowledge concerning modelling existing buildings leads to significant variability in fragility curves for single or grouped existing buildings.This study aims to investigate the uncertainties of fragility curves,with special consideration of the single-building sigma.Experimental data and simplified models are applied to the BRD tower in Bucharest,Romania,a RC building with permanent instrumentation.A three-step methodology is applied:(1)adjustment of a linear MDOF model for experimental modal analysis using a Timoshenko beam model and based on Anderson’s criteria,(2)computation of the structure’s response to a large set of accelerograms simulated by SIMQKE software,considering twelve ground motion parameters as intensity measurements(IM),and(3)construction of the fragility curves by comparing numerical interstory drift with the threshold criteria provided by the Hazus methodology for the slight damage state.By introducing experimental data into the model,uncertainty is reduced to 0.02 considering S d(f1)as seismic intensity IM and uncertainty related to the model is assessed at 0.03.These values must be compared with the total uncertainty value of around0.7 provided by the Hazus methodology. | Matthieu Perrault Philippe Gueguen Alexandru Aldea Sorin Demetriu | 2013 | Earthquake Engineering and Engineering Vibration2013，12，4： | 4 |
| 6 | Validating cone-beam computed tomography for peri-implant bone morphometric analysis显示文摘Cone-beam computed tomography(CBCT) has been recently used to analyse trabecular bone structure around dental implants. To validate the use of CBCT for three-dimensional(3D) peri-implant trabecular bone morphometry by comparing it to two-dimensional(2D) histology, 36 alveolar bone samples(with implants n527 vs. without implants n59) from six mongrel dogs, were scanned ex vivo using a high-resolution(80 mm)CBCT. After scanning, all samples were decalcified and then sectioned into thin histological sections(,6 mm)to obtain high contrast 2D images. By using CTAn imaging software, bone morphometric parameters including trabecular number(Tb.N), thickness(Tb.Th), separation(Tb.Sp) and bone volume fraction(BV/TV) were examined on both CBCT and corresponding histological images. Higher Tb.Th and Tb.Sp, lower BV/TV and Tb.N were found on CBCT images(P,0.001). Both measurements on the peri-implant trabecular bone structure showed moderate to high correlation(r50.65–0.85). The Bland–Altman plots showed strongest agreement for Tb.Th followed by Tb.Sp, Tb.N and BV/TV, regardless of the presence of implants.
