| 1 | 挪威海底隧道经验显示文摘海底隧道深埋于海水以下,因而在勘探、设计、施工和运行方面都比陆地隧道更困难。挪威在近30a来修建了40条海底隧道,积累了丰富的经验,形成了被称为“挪威海底隧道概念”的一整套技术。规划设计阶段要采用各种可能方法进行尽可能详细的地质勘探,尤其是海洋折射地震波探测,以提供基岩表面深度、基岩地质剖面等重要资料。影响隧道的稳定主要为断层和软弱带,因此,在地质勘探中确认断层和软弱带是极为重要的。施工时需采用精心设计的方法,包括减少每轮爆破的进尺及采用喷射混凝土拱肋、密集长锚桩等。典型的挪威海底隧道衬砌由作为支护的喷锚结构和自立的防水防冻的内衬砌组成。挪威海底隧道的衬砌都按排水结构设计,而不承受外水压力。渗水是海底隧道的最大威胁,施工期应随时了解掌子面前的地质和水文地质条件,超前探测孔和预灌浆是控制渗流的最佳方法,同时必须时刻准备处理可能发生的紧急情况。最小岩石覆盖厚度影响隧道安全和造价,必须慎重考虑,根据岩石条件、稳定要求、渗水量充分论证,大部分挪威海底隧道的最小岩石覆盖厚度都小于50m。海水的腐蚀性给海底隧道带来一系列的特殊问题,须认真对待。 | 吕明 Grφv E Nilsen B Melby K | 2005 | 岩石力学与工程学报2005,24,23: | 106 |

| 7 | Autoimmune pancreatitis in the context of IgG4-related disease:Review of imaging findings显示文摘Current understanding of autoimmune pancreatitis(AIP) recognizes a histopathological subtype of the disease to fall within the spectrum of IgG4-related disease.Along with clinical,laboratory,and histopathological data,imaging plays an important role in the diagnosis and management of AIP,and more broadly,within the spectrum of IgG4-related disease.In addition to the defined role of imaging in consensus diagnostic protocols,an array of imaging modalities can provide complementary data to address specific clinical concerns.These include contrast-enhanced computed tomography(CT) and magnetic resonance(MR) imaging for pancreatic parenchymal lesion localization and characterization,endoscopic retrograde and magnetic resonance cholangiopancreatography(ERCP and MRCP) to assess for duct involvement,and more recently,positron emission tomography(PET) imaging to assess for extra-pancreatic sites of involvement.While the imaging appearance of AIP varies widely,certain imaging features are more likely to represent AIP than alternate diagnoses,such as pancreatic cancer.While nonspecific,imaging findings which favor a diagnosis of AIP rather than pancreatic cancer include:delayed enhancement of affected pancreas,mild dilatation of the main pancreatic duct over a long segment,the 'capsule' and 'penetrating duct' signs,and responsiveness to corticosteroid therapy.Systemic,extra-pancreatic sites of involvement are also often seen in AIP and IgG4-related disease,and typically respond to corticosteroid therapy.Imaging by CT,MR,and PET also play a role in the diagnosis and monitoring after treatment of involved sites. | Leslie K Lee Dushyant V Sahani | 2014 | World Journal of Gastroenterology2014,20,41: | 23 |
| 8 | Comparative clinical trial of S-pantoprazole versus racemic pantoprazole in the treatment of gastro-esophageal reflux disease显示文摘AIM: To compare the effi cacy and tolerability of S-pan- toprazole (20 mg once a day) versus racemic Panto- prazole (40 mg once a day) in the treatment of gastro- esophageal reflux disease (GERD). METHODS: This multi-centre, randomized, double-blind clinical trial consisted of 369 patients of either sex suf- fering from GERD. Patients were randomly assigned to receive either one tablet (20 mg) of S-pantoprazole once a day (test group) or 40 mg racemic pantoprazole once a day (reference group) for 28 d. Patients were evaluated for reduction in baseline on d 0, GERD symptom score on d 14 and 28, occurrence of any adverse effect during the course of therapy. Gastrointestinal (GI) endoscopy was performed in 54 patients enrolled at one of the study centers at baseline and on d 28. RESULTS: Signifi cant reduction in the scores (mean and median) for heart burn (P < 0.0001), acid regurgitation (P < 0.0001), bloating (P < 0.0001), nausea (P < 0.0001) and dysphagia (P < 0.001) was achieved in both groups on d 14 with further reduction on continuing the therapy till 28 d. There was a statistically signifi cant difference in the proportion of patients showing improvement in acid regurgitation and bloating on d 14 and 28 (P = 0.004 for acid regurgitation; P = 0.03 for bloating) and heart burn on d 28 (P = 0.01) between the two groups, with a higher proportion in the test group than in the refer- ence group. Absolute risk reductions for heartburn/acid regurgitation/bloating were approximately 15% on d 14 and 10% on d 28. The relative risk reductions were 26%-33% on d 14 and 15% on d 28. GI endoscopy showed no signifi cant difference in healing of esophagitis (P = 1) and gastric erosions (P = 0.27) between the two groups. None of the patients in either group reported any adverse effect during the course of therapy.CONCLUSION: In GERD, S-pantoprazole (20 mg) is more effective than racemic pantoprazole (40 mg) in improving symptoms of heartburn, acid regurgitation, bloating and equally effective in healing esophagitis and gastric erosions. The relative risk reduction is 15%-33%. Lev.显示文摘Objective:To study the morphology,biochemistry and bioactivity of the epidermal glands of the glandular morphotype of Christella parasitica(C.parasitica)(L.) H.Lev.Methods: Morphological studies on epidermal glands were earned out by using light microscope and scanning electron microscope.To prepare the extract,the shade-dried fronds of glandular morphotype were soaked in acetone.For antibacterial studies paper disc method was followed by using various pathogenic bacteria.Results:Detailed micromorphological,phytochemical and bioactivity studies on a medicinal fern C.parasitica(L.) H.Lev.showed its inlraspecific variation in antibacterial activity.The presence or absence of the epidermal glands was the key factor for antibacterial activity in the morphovariants of this species.The epidermal glands were orange-coloured,stalked and elongated ones of about 84.2μm×45μm,and distributed on the undersurface of cosla,coslules and veins in croziers,young and mature leaves.Frequency of glands varied from 15/cm on costa in mature leaves to 140/cm on costules in croziers.The acetone extract of the glands showed antibacterial activities and also toxic effect against mosquito larvae and tadpoles of frog.Preliminary phytochemical analysis and HPLC studies of the gland extract showed the presence of various kinds of terpenoids,alkaloids,tannins,saponins and flavonoids in it.Conclusions:The present study shows that epidermal glands of the glandular morphotype of C. parasitica(L.) | 13 | 钢-混凝土组合连续弯箱梁抗火性能与设计方法显示文摘为研究提高钢-混凝土组合连续弯箱梁抗火性能的策略,选取某三跨钢-混凝土组合连续弯箱梁为研究对象,利用通用有限元软件ANSYS建立了其在火灾下的三维非线性两阶段分析模型;基于已有热-结构耦合分析方法,模型考虑了钢箱梁内空腔辐射传热过程和其上翼缘与混凝土板的接触边界条件;将模型得到的预测结果与试验数据进行了比较,验证了模型的可靠性;采用建立的模型在不同纵向受火位置、火灾强度和荷载水平作用下对钢-混凝土组合连续弯箱梁跨中挠度进行了参数敏感性分析,研究了其极限承载能力和刚度衰变规律;以火灾下跨中挠度为评估指标,提出了针对钢-混凝土组合连续弯箱梁的抗火设计方法。研究结果表明:在对称火和结构荷载作用下,钢-混凝土组合连续弯箱梁外边缘挠度大于内边缘挠度,且荷载越大,火灾越严重,这一效应越显著;在油罐车等过火面积较大的火灾作用下,刚度较极限承载能力衰退更快,与常温下的钢-混凝土组合连续弯箱梁极限承载能力和刚度相比,边跨受火16 min时极限承载能力和刚度分别降低至29%和14%,中跨受火28 min时极限承载能力和刚度分别降低至31%和22%;在钢-混凝土组合连续弯箱梁抗火设计中,应首先提高外侧钢箱梁在火灾下的刚度,增多和加宽外侧钢箱梁底板纵向加劲肋可使边跨受火20 min后内外侧钢箱梁跨中挠度差分别减小23%和30%,中跨受火32 min后内外侧钢箱梁跨中挠度差分别减小22%和27%。 | 宋超杰 张岗(指导) 贺拴海 KODUR V K 黄侨 李徐阳 | 2021 | 交通运输工程学报2021,21,4: | 14 |
| 14 | 世界家庭医生组织(WONCA)研究论文摘要汇编——有氧运动缓解产后抑郁效果的系统性综述与荟萃分析显示文摘背景当前英美国家尚无运动方法治疗产后抑郁方面的专门指导,而且国际上在此方面的指导也不尽相同。目的评价有氧运动治疗产后抑郁的效果。设计与场所本研究为系统性综述及荟萃分析,无研究地点。方法使用计算机检索MEDLINE、EMBASE、Cochrane Library、Psyc INFO、Sport Discus、Scr Clinical Trials.gov以及World Health Organization International Clinical Trials Registry Platform数据库,收集评估产妇分娩后1年内抑郁状况的随机对照试验、有氧运动和常规护理或者其他护理方式改善产后抑郁效果的比较研究的摘要及全文。使用Cochrane协作组推荐使用的偏倚风险评估工具对纳入的文献进行偏倚风险评估。对纳入的文献进行荟萃分析。对纳入文献之间存在的异质性进行预设亚组分析。结果共纳入13篇文献,包括1 734例患者。整体来看,有氧运动可以显著缓解产后抑郁[SMD=-0.44,95%CI=(-0.75,-0.12)]。异质性分析发现,有氧运动干预效果在可能患有抑郁的产妇间及一般产妇间不存在统计学差异;只采用有氧运动取得的干预效果与采用有氧运动结合其他方式取得的干预效果不存在统计学差异;群体有氧运动与配置有氧运动咨询的干预效果不存在统计学差异。结论本项系统性综述为有氧运动具有缓解产后抑郁的效果提供了证据支持。群体运动、研究对象自己选择运动方式、运动结合其他干预方式均是有效缓解产后抑郁的方法。但是由于纳入文献存在显著异质性和高偏倚风险,故此研究结果应谨慎参考。 | PRITCHETT R V DALEY A J JOLLY K 本刊编辑部 | 2017 | 中国全科医学2017,20,30: | 12 |

| 16 | Predictive value of multi-detector computed tomography for accurate diagnosis of serous cystadenoma:Radiologic-pathologic correlation显示文摘AIM:To identify multi-detector computed tomography(MDCT) features most predictive of serous cystadenomas(SCAs),correlating with histopathology,and to study the impact of cyst size and MDCT technique on reader performance.METHODS:The MDCT scans of 164 patients with surgically verified pancreatic cystic lesions were reviewed by two readers to study the predictive value of various morphological features for establishing a diagnosis of SCAs.Accuracy in lesion characterization and reader conf idence were correlated with lesion size(≤3cm or≥3cm) and scanning protocols(dedicated vs routine).RESULTS:28/164 cysts(mean size,39 mm;range,8-92mm) were diagnosed as SCA on pathology.The MDCT features predictive of diagnosis of SCA were microcystic appearance(22/28,78.6%),surface lobulations(25/28,89.3%) and central scar(9/28,32.4%).Stepwise logistic regression analysis showed that only microcystic appearance was signifi cant for CT diagnosis of SCA(P=0.0001).The sensitivity,specificity and PPV of central scar and of combined microcystic appearance and lobulations were 32.4%/100%/100% and 68%/100%/100%,respectively.The reader confidence was higher for lesions>3cm(P=0.02) and for MDCT scans performed using thin collimation(1.25-2.5mm) compared to routine 5 mm collimation exams(P>0.05).CONCLUSION:Central scar on MDCT is diagnostic of SCA but is seen in only one third of SCAs.Microcystic morphology is the most significant CT feature in diagnosis of SCA.A combination of microcystic appearance and surface lobulations offers accuracy comparable to central scar with higher sensitivity. | Anjuli A Shah Nisha I Sainani Avinash Kambadakone Ramesh Zarine K Shah Vikram Deshpande Peter F Hahn Dushyant V Sahani | 2009 | World Journal of Gastroenterology2009,15,22: | 11 |
| 17 | Rotational assisted endoscopic retrograde cholangiopancreatography in patients with reconstructive gastrointestinal surgical anatomy显示文摘AIM: To evaluate the success rates of performing therapy utilizing a rotational assisted enteroscopy device in endoscopic retrograde cholangiopancreatography(ERCP) in surgically altered anatomy patients. METHODS: Between June 1, 2009 and November 8, 2012, we performed 42 ERCPs with the use of rotational enteroscopy for patients with altered anatomy(39 with gastric bypass Roux-en-Y, 2 with Billroth Ⅱ gastrectomy, and 1 with hepaticojejunostomy associated with liver transplant). The indications for ERCP were: choledocholithiasis: 13 of 42(30.9%), biliary obstruction suggested on imaging: 20 of 42(47.6%), suspected sphincter of Oddi dysfunction: 4 of 42(9.5%), abnormal liver enzymes: 1 of 42(2.4%), ascending cholangitis: 2 of 42(4.8%), and bile leak: 2 of 42(4.8%). All procedures were completed with the Olympus SIF-Q180 enteroscope and the Endo-Ease Discovery SB overtube produced by Spirus Medical. RESULTS: Successful visualization of the major ampulla was accomplished in 32 of 42 procedures(76.2%). Cannulation of the bile duct was successful in 26 of 32 procedures reaching the major ampulla(81.3%). Successful therapeutic intervention was completed in 24 of 26 procedures in which the bileduct was cannulated(92.3%). The overall intention to treat success rate was 64.3%. In terms of cannulation success, the intention to treat success rate was 61.5%. Ten out of forty two patients(23.8%) required admission to the hospital after procedure for abdominal pain and nausea, and 3 of those 10 patients(7.1%) had a diagnosis of post-ERCP pancreatitis.
