| 1 | Acute-on-chronic liver failure:Pathogenesis,prognostic factors and management显示文摘Acute-on-chronic liver failure(ACLF) is increasingly recognized as a complex syndrome that is reversiblein many cases. It is characterized by an acute deterioration of liver function in the background of a pre-existing chronic liver disease often associated with a high short-term mortality rate. Organ failure(OF) is always associated, and plays a key role in determining the course, and the outcome of the disease. The definition of ACLF remains controversial due to its overall ambiguity, with several disparate criteria among various associations dedicated to the study of liver diseases. Although the precise pathogenesis needs to be clarified, it appears that an altered host response to injury might be a contributing factor caused by immune dysfunction, ultimately leading to a pro-inflammatory status, and eventually to OF. The PIRO concept(Predisposition, Insult, Response and Organ Failure) has been proposed to better approach the underlying mechanisms. It is accepted that ACLF is a different and specific form of liver failure, where a precipitating event is always involved, even though it cannot always be ascertained. According to several studies, infections and active alcoholism often trigger ACLF. Viral hepatitis, gastrointestinal haemorrhage, or drug induced liver injury, which can also provoke the syndrome. This review mainly focuses on the physiopathology and prognostic aspects. | 2 | 肝的 osteodystrophy : 为在长期的肝的考虑的一件重要的事疾病显示文摘 Hepatic osteodystrophy (HO) is the generic term defining the group of alterations in bone mineral metabolism found in patients with chronic liver disease. This paper is a global review of HO and its main pathophysiological, epidemiological and therapeutic aspects. Studies examining the most relevant information concerning the prevalence, etiological factors, diagnostic and therapeutic aspects involved in HO were identified by a systematic literature search of the PubMed database. HO generically defines overall alterations in bone mineral density (BMD) (osteoporosis or osteopenia) which appear as a possible complication of chronic liver disease. The origin of HO is multifactorial and its etiology and severity vary in accordance with the underlying liver disease. Its exact prevalence is unknown, but different studies estimate that it could affect from 20% to 50% of patients. The reported mean prevalence of osteoporosis ranges from 13%-60% in chronic cholestasis to 20% in chronic viral hepatitis and 55% in viral cirrhosis. Alcoholic liver disease is not always related to osteo-penia. HO has been commonly studied in chronic cholestatic disease (primary biliary cirrhosis and primary sclerosing cholangitis). Several risk factors and pathogenic mechanisms have been associated with the loss of BMD in patients with chronic liver disease. However, little information has been discovered in relationship to most of these mechanisms. Screening for osteopenia and osteoporosis is recommended in advanced chronic liver disease. | 3 | Oxidative stress modulation in hepatitis C virus infected cells显示文摘Hepatitis C virus(HCV) replication is associated with the endoplasmic reticulum, where the virus can induce cellular stress. Oxidative cell damage plays an important role in HCV physiopathology. Oxidative stress is triggered when the concentration of oxygen species in the extracellular or intracellular environment exceeds antioxidant defenses. Cells are protected and modulate oxidative stress through the interplay of intracellular antioxidant agents, mainly glutathione system(GSH) and thioredoxin; and antioxidant enzyme systems such as superoxide dismutase, catalase, GSH peroxidase, and heme oxygenase-1. Also, the use of natural and synthetic antioxidants(vitamin C and E, N-acetylcysteine, glycyrrhizin, polyenylphosphatidyl choline, mitoquinone, quercetin, S-adenosylmethionine and silymarin) has already shown promising results as co-adjuvants in HCV therapy. Despite all the available information, it is not known how different agents with antiviral activity can interfere with the modulation of the cell redox state induced by HCV and decrease viral replication. This review describes an evidence-based consensus on molecular mechanisms involved in HCV replication and their relationship with cell damage induced by oxidative stress generated by the virus itself and cell antiviral machinery. | 4 | Surveillance and diagnosis of hepatocellular carcinoma: A systematic review显示文摘BACKGROUND Hepatocellular carcinoma (HCC) appears in most of cases in patients with advanced liver disease and is currently the primary cause of death in this population.Surveillance of HCC has been proposed and recommended in clinical guidelines to obtain earlier diagnosis,but it is still controversial and is not accepted worldwide.AIM To review the actual evidence to support the surveillance programs in patients with cirrhosis as well as the diagnosis procedure.METHODS Systematic review of recent literature of surveillance (tools,interval,cost-benefit,target population) and the role of imaging diagnosis (radiological non-invasive diagnosis,optimal modality and agents) of HCC.RESULTS The benefits of surveillance of HCC,mainly with ultrasonography,have been assessed in several prospective and retrospective analysis,although the percentage of patients diagnosed in surveillance programs is still low.Surveillance of HCC permits diagnosis in early stages allows better access to curative treatment and increases life expectancy in patients with cirrhosis.HCC is a tumor with special radiological characteristics in computed tomography and magnetic resonance imaging,which allows highly accurate diagnosis without routine biopsy confirmation.The actual recommendation is to perform biopsy only in indeterminate nodules.CONCLUSION The evidence supports the recommendation of performing surveillance of HCC in patients with cirrhosis susceptible of treatment,using ultrasonography every 6 mo.The diagnosis evaluation of HCC can be established based on noninvasive imaging criteria in patients with cirrhosis. | Sonia Pascual Cayetano Miralles Juan M Bernabé Javier Irurzun Mariana Planells | 2019 | World Journal of Clinical Cases2019，7，16： | 5 |
| 5 | Application of Natural Antioxidants from Strawberry Tree(Arbutus unedo L.) and Dog Rose(Rosa canina L.) to Frankfurters Subjected to Refrigerated Storage显示文摘The effect of the addition of natural antioxidants from strawberry tree(Arbutus unedo L.;AU)and dog rose(Rosa canina L.;RC),in frankfurters elaborated with or without the addition of antioxidant additives(sodium ascorbate and nitrite)was studied.Six different types of experimental frankfurters were prepared depending on the addition of phenolic-rich extracts from RC and AU and the presence(P)or absence(C)of antioxidant additives.Thiobarbituric acid reactive substances(TBARS)-numbers significantly increased during chilled storage of C-frankfurters while additives and fruit phenolics inhibited lipid oxidation in P-frankfurters.The amount of protein carbonyls significantly increased in all treatments except in P-AU frankfurters.The discoloration process that occurred during the chilled storage was reduced by the addition of substances with proven antioxidant activity(P-frankfurters).Texture characteristics as hardness,springiness,cohesiveness and gumminess also suffered a significant deterioration in C-frankfurters.The use of phenolic fruit extracts in combination with traditional antioxidant additives is a successful strategy to enhance the oxidative stability of frankfurters without modifying their color and texture properties. | Mónica Armenteros David Morcuende Sonia Ventanas Mario Estévez | 2013 | Journal of Integrative Agriculture2013，12，11： | 4 |
| 6 | The European registry of quality outcomes for cataract and refractive surgery(EUREQUO):a database study of trends in volumes,surgical techniques and outcomes of refractive surgery显示文摘Background:A European web-based registry for refractive surgery was established in 2008;The European Registry of Quality Outcomes for Cataract and Refractive Surgery(EUREQUO).The aim of the registry was to improve treatment and standards of care for refractive surgery.Further aims were to offer a tool for benchmarking by establishing a reference database and for surgeons to enter and analyze their own outcomes.The purpose of this study was to characterize the registry and analyze the data collected during its first decade.Methods:The characteristics of the web-based registry are described.Data collected from February 4^(th) 2004 until June 30th 2014 are included in the analysis.The database is analyzed in terms of surgical technique,indications for surgery,complications,and refractive and visual outcomes.Results:Data have been reported from 47 centers in 14 countries until mid-2014.About 4,000 procedures were reported annually.The most frequent procedure was laser-assisted in-situ keratomileusis(LASIK)with 11697 reported surgeries.Over time in the database,LASIK declined(p<0.001)while photorefractive keratectomy(PRK)and refractive lens exchange(RLE)increased(p<0.001 for both procedures).The indications for surgery,in terms of preoperative refraction and age,were stable over time,for all types of procedures.Surgical complications were reported infrequently and with a well-known relationship to the type of surgical procedure.The reported refractive outcomes were good.The visual outcomes indicate a significant increase of visual acuity after high myopia treatment by phakic intraocular lens in the anterior(phakic IOL AC)and the posterior(phakic IOL PC)chamber and a poorer visual outcome,after both myopia and hyperopia treatment,by epithelial LASIK(Epi-LASIK).Conclusions:We describe the establishment of a European registry for refractive surgery.The database increases at a rate of approximately 4000 refractive procedures per year.The most frequent procedure is LASIK,but both PRK and RLE are an increasing part of the reported procedures.The indications for surgery have been stable over time.Surgical complications and visual outcome vary,depending on the type of surgery. | Mats Lundström Sonia Manning Peter Barry Ulf Stenevi Ype Henry Paul Rosen | 2015 | Eye and Vision2015，2，1： | 3 |

| 9 | Evidence for colorectal sarcomatoid carcinoma arising from tubulovillous adenoma显示文摘Sarcomatoid carcinomas of the colorectum are rare tu- mors that display both malignant epithelial and stromal components. Clinically, they are aggressive tumors with early metastasis. Due to their infrequent occurrence, the pathogenesis is poorly understood. We report a case of a 52-year-old woman who presented with a rectal mass and intermittent hematochezia. Superficial biopsies during colonoscopy revealed a tubulovillous adenoma with high-grade dysplasia. Endoscopic ultra- sonography confirmed an invasive nature of the mass, and deeper biopsies revealed the presence of neoplasm with mixed histological components. The surgically- excised specimen demonstrated the presence of poorly differentiated spindle cells underneath the tubulovillous adenoma and an intermediate stage of invasive adeno- carcinoma. Based on the histological appearance and immunohistochemical studies, a diagnosis of sarcoma- toid carcinoma was made. Only nine cases of sarcoma- toid carcinomas of the colorectum have been reported to date. As a result, the terminology and pathogenesis of sarcomatoid carcinoma remain speculative. To the best of our knowledge, this is the first report of co- existence of sarcomatoid carcinoma and invasive ad- enocarcinoma with tubulovillous adenoma; all stages represented within the same tumor.
