| 1 | Treatment of hepatocellular carcinoma with portal venous tumor thrombosis: A comprehensive review显示文摘The natural history of hepatocellular carcinoma(HCC)with portal vein tumor thrombosis(PVTT)is dismal(approximately 2-4 mo),and PVTT is reportedly found in 10%-40%of HCC patients at diagnosis.According to the Barcelona Clinic Liver Cancer(BCLC)Staging System(which is the most widely adopted HCC management guideline),sorafenib is the standard of care for advanced HCC(i.e.,BCLC stage C)and the presence of PVTT is included in this category.However,sorafenib treatment only marginally prolongs patient survival and,notably,its therapeutic efficacy is reduced in patients with PVTT.In this context,there have been diverse efforts to develop alternatives to current standard systemic chemotherapies or combination treatment options.To date,many studies on transarterial chemoembolization,3-dimensional conformal radiotherapy,hepatic arterial chemotherapy,and transarterial radioembolization report better overall survival than sorafenib therapy alone,but their outcomes need to be verified in future prospective,randomized controlled studies in order to be incorporated into current treatment guidelines.Additionally,combination strategies have been applied to treat HCC patients with PVTT,with the hope that the possible synergistic actions among different treatment modalities would provide promising results.This narrative review describes the current status of the management options for HCC with PVTT,with a focus on overall survival. | Kichang Han Jin Hyoung Kim Gi-Young Ko Dong Il Gwon Kyu-Bo Sung | 2016 | World Journal of Gastroenterology2016,22,1: | 29 |
| 2 | Safety and efficacy of tenofovir in chronic hepatitis B-related decompensated cirrhosis显示文摘AIM To evaluate the safety and efficacy of tenofovir disoproxil fumarate(TDF) as a first-line therapy in decompensated liver disease. METHODS We enrolled 174 chronic hepatitis B-related liver cirrhosis patients treated with 300 mg/d TDF at six Korean centers. Of the 174 cirrhosis patients, 57 were assigned to the decompensated cirrhosis group and 117 were assigned to the compensated cirrhosis group. We followed the patients for 12 mo and evaluated clinical outcomes, including biochemical, virological, and serological responses. We also evaluated changes in hepatic and renal function and compared the decompensated and compensated cirrhosis groups. RESULTS The 1-year complete virological response(CVR) and Hepatitis B e antigen(HBe Ag) seroconversion were seen in 70.2% and 14.2% in the decompensated cirrhosis group, respectively. The rates of HBe Ag seroconversion/loss and ALT normalization at month 12 were similar in both groups. TDF treatment was also effective for decreasing the level of hepatitis B virus(HBV) DNA in both groups, but CVR was higher in the compensated group(88.9% vs 70.2%, P = 0.005). Tenofovir treatment for 12 mo resulted in improved Child-Turcotte-Pugh(CTP) and model for end-stage liver disease(MELD) scores in decompensated group(P < 0.001). Of the 57 decompensated patients, 39(68.4%) achieved CTP class A and 27(49.1%) showed improvement in the CTP score of 2 points after 12 mo of TDF. The observed rate of confirmed 0.5 mg/d L increases in serum levels of creatinine in the decompensated and compensated cirrhosis group were 7.0% and 2.5%, respectively(P < 1.000).CONCLUSION TDF therapy in decompensated cirrhosis patients was effective for decreasing HBV DNA levels and improving hepatic function with relatively lower CVR than in compensated cirrhosis. | 3 | Clinicopathological features and prognosis of combined hepatocellular carcinoma and cholangiocarcinoma after surgery显示文摘BACKGROUND: Combined hepatocellular carcinoma and cholangiocarcinoma (cH CC-CC) is a rare subtype of primary liver cancer consisting of both hepatocellular carcinoma (HCC)and cholangiocarcinoma (CC). Because of the rarity of this tumor its feature is poorly understood. The present study aimed to evaluate the clinicopathological features and long-term prognosis of patients with cH CC-CC after surgery and to compare with those of the patients with stage-matched HCC and CC.METHODS: The clinicopathological features of the patients who underwent surgery for cH CC-CC at our center during the period of 2001-2010 were retrospectively analyzed and compared with those of stage-matched HCC and CC patients Cancer staging was performed according to the AJCC Cancer Staging Manual (6th ed.). Overall survival and disease-free survival were compared among the groups and prognostic factors of cH CC-CC were evaluated.RESULTS: Significant differences were observed in clinico pathological features among 42 patients with cH CC-CC, 90patients with HCC and 45 patients with CC. Similar to HCC patients, cH CC-CC patients had frequent hepatitis B virus antigen positivity, microscopic vessel invasion, cirrhosis and high level of serum alpha-fetoprotein. Similar to CC patients, cH CC-CC patients showed increased bile duct invasion and decreased capsule. The 1-, 3-, and 5-year overall survival and disease free survival of patients with cH CC-CC were not significantly different from those with stage-matched patients with CC;but significantly poorer than those with HCC. In subanalysis of patients with stage II, the overall survival in patients with cH CC-CC or CC was significantly poorer than that in patients , with HCC. We did not find the difference in patients with other stages. | 4 | Comparison of postpolypectomy bleeding between epinephrine and saline submucosal injection for large colon polyps by conventional polypectomy:A prospective randomized,multicenter study显示文摘AIM:To evaluate and compare the clinical outcomes of prophylactic submucosal saline-epinephrine injection and saline injection alone for large colon polyps by conventional polypectomy. METHODS:A prospective study was conducted from July 2003 to July 2004 at 11 tertiary endoscopic centers. Large colon polyps (> 10 mm in diameter) wererandomized to undergo endoscopic polypectomy with submucosal saline-epinephrine injection (epinephrine group) or normal saline injection (saline group). Endoscopic polypectomy was performed by the conventional snare method,and early (< 12 h) and late bleeding complications (12 h-30 d) were observed. RESULTS:A total of 561 polyps in 486 patients were resected by endoscopic polypectomy. Overall,bleeding complications occurred in 7.6% (37/486) of the patients,including 4.9% (12/244) in the epinephrine group,and 10.3% (25/242) in the saline group. Early and late postpolypectomy bleeding (PPB) occurred in 6.6% (32/486) and 1% (5/486) of the patients,respectively,including 4.5% (11/244),0.4% (1/244) in the epinephrine group,and 8.7% (21/242),1.7% (4/242) in the saline group. No significant differences in the rates of overall,early and late PPB were observed between the 2 groups. Multivariate stepwise logistic regression analysis revealed that large size (> 2 cm) and neoplastic polyps were independently and significantly associated with the presence of PPB. | 5 | Increased liver markers are associated with higher risk of type 2 diabetes显示文摘AIM: To investigate the association between liver markers and the risk of type 2 diabetes(T2DM) and impaired fasting glucose(IFG).METHODS: A total of 8863 participants(3408 men and 5455 women) over 30 years of age were analyzed from the fifth Korean National Health and Nutrition Examination Survey(2010-2011). The associations of serum liver markers such as aspartate aminotransferase(AST), alanine aminotransferase(ALT), AST/ALT, and gamma-glutamyltransferase(GGT) with T2 DM and IFG were analyzed using logistic regression models. Participants were divided into sex-specific quartiles on the basis of liver markers.RESULTS: The prevalence of T2 DM and IFG were 11.3% and 18.3%. Increasing quartiles of ALT and GGT were positively and AST/ALT were negatively correlated with T2 DM and IFG. Analysis of the liver marker combinations showed that if any two or more markers were in the highest risk quartile, the risks of both T2 DM and IFG increased significantly. The risk was greatest when the highest ALT and GGT and lowest AST/ALT quartile were combined, with the riskof T2 DM at 3.21(95%CI: 1.829-5.622, P < 0.001) in men and 4.60(95%CI: 3.217-6.582, P < 0.001) in women. | 7 | Mitochondria as therapeutic targets for cancer stem cells显示文摘Cancer stem cells(CSCs) are maintained by theirsomatic stem cells and are responsible for tumor initiation, chemoresistance, and metastasis. Evidence for the CSCs existence has been reported for a number of human cancers. The CSC mitochondria have been shown recently to be an important target for cancer treatment, but clinical significance of CSCs and their mitochondria properties remain unclear. Mitochondriatargeted agents are considerably more effective compared to other agents in triggering apoptosis of CSCs, as well as general cancer cells, via mitochondrial dysfunction. Mitochondrial metabolism is altered in cancer cells because of their reliance on glycolytic intermediates, which are normally destined for oxidative phosphorylation. Therefore, inhibiting cancer-specific modifications in mitochondrial metabolism, increasing reactive oxygen species production, or stimulating mitochondrial permeabilization transition could be promising new therapeutic strategies to activate cell death in CSCs as well, as in general cancer cells. This review analyzed mitochondrial function and its potential as a therapeutic target to induce cell death in CSCs. | 8 | Inhibitory effect of different Dendrobium species on LPS-induced inflammation in macrophages via suppression of MAPK pathways显示文摘Dendrobii Caulis(DC), named ‘Shihu' in Chinese, is a precious herb in traditional Chinese medicine. It is widely used to nourish stomach, enhance body fluid production, tonify 'Yin' and reduce heat. More than thirty Dendrobium species are used as folk medicine. Some compounds from DC exhibit inhibitory effects on macrophage inflammation. In the present study, we compared the anti-inflammatory effects among eight Dendrobium species. The results provided evidences to support Dendrobium as folk medicine, which exerted its medicinal function partially by its inhibitory effects on inflammation. To investigate the anti-inflammatory effect of Dendrobium species, mouse macrophage cell line RAW264.7 was activated by lipopolysaccharide. The nitric oxide(NO) level was measured using Griess reagent while the pro-inflammatory cytokines were tested by ELISA. The protein expressions of inducible NO synthase(iNOS), cyclooxygenase-2(COX-2) and mitogen-activated protein kinases(MAPKs) phosphorylation were evaluated by Western blotting analysis. Among the eight Dendrobium species, both water extracts of D. thyrsiflorum B.S.Williams(DTW) and D. chrysotoxum Lindl(DCHW) showed most significant inhibitory effects on NO production in a concentration-dependent manner. DTW also significantly reduced TNF-α, MCP-1, and IL-6 production. Further investigations showed that DTW suppressed iNOS and COX-2 expression as well as ERK and JNK phosphorylation, suggesting that the inhibitory effects of DTW on LPS-induced macrophage inflammation was through the suppression of MAPK pathways. | 11 | Optimum condition by mechanical characteristic evaluation in friction stir welding for 5083-O Al alloy显示文摘The mechanical characteristies for friction stir welding (FSW) of 5083-O Al alloy were evaluated. The results show that in FSW at 800 r/min and 124 mm/min, a weld defect is observed at the start point. However, the button shape at the end point is good and the stir zone has a soft appearance. At 267 mm/min, a void occurs at the button. A slight weld defect and rough stir zone are seen both at the start and end points at 342 mm/min. Moreover, at the bottom, a tunnel-type void is observed from an early stage to the end point, and at 1 800 r/min, a weld defect can be found from an early stage to the end point. These defects are rough with imperfect joining due to excessive rotation speed and high physical force. Weld fractures relative to rotational and travel speeds are observed at the stir zone. | 16 | Skin diseases in the Da Qing Diabetes Study:a cross-sectional study显示文摘Background:The prevalence of skin diseases and diabetes mellitus(DM)are prominent around the world.The current scope of knowledge regarding the prevalence of skin diseases and comorbidities with type 2 DM(T2DM)is limited,leading to limited recognition of the correlations between skin diseases and T2DM.Methods:We collected 383 subjects from the Da Qing Diabetes Study during the period from July 9th to September 1st,2016.The subjects were categorized into three groups:Normal glucose tolerance(NGT),impaired glucose tolerance(IGT),and T2DM.The prevalence and clinical characteristics of skin diseases were recorded and investigated.Results:In this cross-sectional study,383 individuals with ages ranging from 53 to 89-year-old were recruited.The overall prevalence of skin diseases was 93.5%,and 75.7%of individuals had two or more kinds of skin diseases.Additionally,there were 47 kinds of comorbid skin diseases in patients with T2DM,of which eight kinds of skin diseases had a prevalence>10%.The prevalence of skin diseases in NGT,IGT,and T2DM groups were 93.3%,91.5%,and 96.6%,respectively;stratified analysis by categories showed a statistically significant difference in'disturbances of pigmentation'and'neurological and psychogenic dermatoses'.The duration of T2DM also significantly associated with the prevalence of'disturbances of pigmentation'and'neurological and psychogenic dermatoses'.Subsequently,the prevalence of'disturbances of pigmentation'was higher in males than females in NGT(P<0.01)and T2DM(P<0.01)groups.In addition,the difference in the prevalence of'disturbances of pigmentation'was also significant in NGT and T2DM groups(P<0.01).Conclusions:There was a high prevalence of skin diseases in the Da Qing Diabetes Study.To address the skin diseases in the Da Qing Diabetes Study,increased awareness and intervention measures should be implemented. | Chang-Bing Shen Xin Qian Rui-Xing Yu Xue-Lei Ji Yin-Juan Shi Jing Gao Cheng-Xu Li Ke-Ke Li Wen-Min Fei Xue Shen Zi-Yi Wang Yang Han Xiao-Li Ning Randy Ko Yi-Hsiang Hsu Xian-Yong Yin Guang-Wei Li Yong Cui | 2021 | Chinese Medical Journal2021,,10: | 2 |
