| 1 | Peripheral T-lymphocyte subpopulations in different clinical stages of chronic HBV infection correlate with HBV load显示文摘AIM: To characterize the peripheral T-cell subpopulation profiles and their correlation with hepatitis B virus (HBV) replication in different clinical stages of chronic HBV infection.METHODS: A total of 422 patients with chronic HBV infection were enrolled in this study. The patients were divided into three stages: immune-tolerant stage, immune active stage, and immune-inactive carrier stage. Composition of peripheral T-cell subpopulations was determined by flow cytometry. HBV markers were detected by enzyme-linked immunosorbent assay. SerumHBV DNA load was assessed by quantitative real-time polymerase chain reaction.RESULTS: CD8+ T-cells were significantly higher in patients at the immune-tolerant stage than in patients at the immune-active and -inactive carrier stages (36.87 ± 7.58 vs 34.37 ± 9.07, 36.87 ± 7.58 vs 28.09 ± 5.64, P < 0.001). The peripheral blood in patients at the immune-tolerant and immune active stages contained more CD8+ T-cells than CD4+ T-cells (36.87 ± 7.58 vs 30.23 ± 6.35, 34.37 ± 9.07 vs 30.92 ± 7.40, P < 0.01), whereas the peripheral blood in patients at the immune-inactive carrier stage and in normal controls contained less CD8+ T-cells than CD4+ T-cells (28.09 ± 5.64 vs 36.85 ± 6.06, 24.02 ± 4.35 vs 38.94 ± 3.39, P < 0.01). ANOVA linear trend test showed that CD8+ T-cells were signif icantly increased in patients with a high viral load (39.41 ± 7.36, 33.83 ± 7.50, 31.81 ± 5.95 and 26.89 ± 5.71, P < 0.001), while CD4+ T-cells were signif icantly increased in patients with a low HBV DNA load (37.45 ± 6.14, 33.33 ± 5.61, 31.58 ± 6.99 and 27.56 ± 5.49, P < 0.001). Multiple regression analysis displayed that log copies of HBV DNA still maintained its highly signif icant coefficients for T-cell subpopulations, and was the strongest predictors for variations in CD3+, CD4+ and CD8+ cells and CD4+/CD8+ ratio after adjustment for age at HBV-infection, maternal HBV-infection status, presence of hepatitis B e antigen and HBV mutation.CONCLUSION: Differences in peripheral T-cell subpopulation profi les can be found in different clinical stages of chronic HBV infection. | 4 | Overview of the Shenzhen Emergency Medical Service Call Pattern显示文摘BACKGROUND:In Shenzhen,the Emergency Medical Service(EMS) system has been in service since 1997.This study aims to examine the operation of Shenzhen 120 EMS center and to identify the reasons of calling EMS.METHODS:In this retrospective quantitative descriptive study,the data from the Shenzhen 120 EMS registry in 2011 were analyzed.RESULTS:Shenzhen 120 EMS center is a communication command center.When the number of 120 are dialed,it is forwarded to the closest appropriate hospital for ambulance dispatch.In2011,the Shenzhen 120 EMS center received 153 160 ambulance calls,with an average of 420 calls per day.Calling emergency services was mainly due to traffic accidents.Trauma and other acute diseases constituted a majority of ambulance transports.The adult patients aged 15-60 years are the principal users of EMS.There are no recognized 'paramedic' doctors and nurses.The pre-hospital emergency service is under the operation of emergency departments of hospitals.Shenzhen at present does not have specialized pre-hospital training for doctors and nurses in posttrauma management.Moreover,specialized pre-hospital training,financial support,and public health education on proper use of EMS should be emphasized.CONCLUSION:The Shenzhen 120 EMS center has its own epidemiology characteristics.Traumatic injury and traffic accident are the main reasons for calling ambulance service.In-depth study emphasizing the distribution and characteristics of trauma patients is crucial to the future development of EMS. | Shuk Man Lo Yi Min Yu Lap Yip Larry Lee Mi Ling Eliza Wong Sek Ying Chair Edward J Kalinowski Tak Shing Jimmy Chan | 2012 | World Journal of Emergency Medicine2012，3，4： | 4 |
| 5 | Genome-wide DNA hypermethylation and homocysteine increase a risk for myopia显示文摘AIM: To test for the association between genome-wide methylation and myopia in human and mice. METHODS: Long interspersed nucleotide element 1(LINE-1) methylation levels were used to surrogate genome-wide methylation level. We first tested for the association between high myopia(<-6 D) and LINE-1 methylation in leukocytes in 220 cases and 220 control subjects. Secondly, we validated the results of LINE-1 methylation in eyes from the form deprivation myopia(FDM) mice. Furthermore,we calculated the correlation of LINE-1 methylation levels between leukocyte DNA and ocular DNA in the mice. We also tested whether dopamine can alter LINE-1 methylation levels. RESULTS: The LINE-1 methylation level was significantly higher in the myopic human subjects than controls. The upper and middle tertiles of the methylation levels increased an approximately 2-fold(P≤0.002) risk for myopia than the lower tertile. Similarly, FDM mice had high LINE-1 methylation levels in the leukocyte, retina and sclera, and furthermore the methylation levels detected from these three tissues were significantly correlated. Immunohistochemical staining revealed higher levels of homocysteine and methionine in the rodent myopic eyes than normal eyes. Dopamine treatment to the cells reduced both LINE-1 methylation and DNA methyltransferase levels. CONCLUSION: LINE-1 hypermethylation may be associated with high myopia in human and mice. | 6 | Challenges and needs for China to eliminate rabies显示文摘In China,rabies is a significant public health concern where dogs remain the main reservoir of disease transmission to humans;rabies-related mortality ranks second in the world.We compiled all published articles and official documents on rabies in China's Mainland to examine challenges and needs to eliminate rabies in the country.The Chinese authorities have identified rabies as a priority,recognized rabies control in dogs as key to control rabies in humans and required intersectoral collaborations.Efforts have been made to respond effectively to the latest re-emergence of rabies,which peaked in 2007 with>3,300 cases.Despite these outcomes and the increasing volume of publications and regulations in the recent years,our review points to some major information gaps to improve rabies control activities and envisage elimination program.An emphasis on laboratory or pathogen-associated and basic epidemiology research in the literature has contrasted with the absence of information to monitor various systems in humans and animals(e.g.quality of surveillance,response and post-exposure prophylaxis).Information is also lacking to appropriately inform policymakers(e.g.economic disease burden,impact of policies)and assist program managers(e.g.comprehensive and strategic guidance for cost-effective prevention and control activities,public education and dog population management).In conclusion,strategic planning is needed to provide a sense of direction,demonstrate feasibility of elimination in China,and develop a research agenda,addressing country’s operational needs and constraints.The planning should be a multisectoral effort. | Wenwu Yin Jie Dong Changchun Tu John Edwards Fusheng Guo Hang Zhou Hongjie Yu Sirenda Vong | 2013 | Infectious Diseases of Poverty2013，2，1： | 3 |
| 7 | OPERATIONAL TROPICAL CYCLONE FORECAST VERIFICATION PRACTICE IN THE WESTERN NORTH PACIFIC REGION显示文摘This paper summarizes results of a survey of the operational status of tropical cyclone(TC) forecast verifi-cation in the western North Pacific region, covering the Members of the ESCAP/WMO Typhoon Committee, RSMC Tokyo-Typhoon Center, and JTWC. Results of the survey indicate that significant efforts have been focused on the verification of TC forecast guidance by operational forecast agencies in the region, either individually or through international cooperation. A variety of TC forecast verification products are available to the forecasters in either real time or in post-season through multiple means, with the main focus on the deterministic track and intensity forecasts. Only a few verification products are available for probabilistic forecasts. Verification of TC precipitation and high wind forecasts are also lacking sufficient attention in the region.
